AGENDA ITEM WORDING: Approval of the appointment of Debra Premaza to the Health Council
of South Florida to serve a term commencing on December 1,2000 and concluding on June 30, 2002.
ITEM BACKGROUND: Debra Premaza will be replacing Carter Long Hannah in the category of
Purchaser. Mr. Hannah has resigned. The Board opening was advertised in accordance with Article III,
Section I of the Health Council of South Florida Bylaws.

Dear Mr. Roberts:
The Health Council of South Florida (HCSF) Inc. is pleased to submit
the attached application for consideration for appointment to the
Board of Directors in the placement category of Purchaser. This is for
a term commencing on December 1, 2000 and concluding on June
30, 2002. The Board member who held this seat resigned from the
Board of Directors.
This Board opening was advertised in accordance with Article III,
Section 1 of the Health Council of South Florida Bylaws. Ms. Premaza
is the sole applicant and is highly qualified. If possible, we would like
to have this name submitted to the Commission~rs for appointment at
the next County Commission Meeting.
