AGENDA ITEM WORDING: Approval to purchase one Type I, F-450 Ambulance by "Piggybacking"
on our own previously awarded competitive bid from Wheeled Coach Industries, Inc. in the amount of
$100,217.
ITEM BACKGROUND: Monroe County is the recipient of a rural emergency medical services
matching grant in the amount of $81,000.00 in State funds, awarded by the Florida Department of Health,
Bureau of Emergency Medical Services. These funds are to support the purchase of one ambulance to be used
in District-1. Further, all costs that exceed the limits of the grant award, in accordance with Section 401.113
(2)(b), Florida Statutes, are the sole responsibility of the grantee. Funding for the County's match, in the
amount of $19,217, is proposed to come from Fire and Ambulance District-1, Reserves 141.
Wheeled Coach Industries agrees to abide by the terms and conditions of Monroe County's initial Purchase
Order #197350 issued on 2/10/99, and subsequent Purchase Orders #211772, 218380, and 218906 issued on
2/17/00, 12/21/00, and 1/22/01 respectively for the purchase of additional Type I, F-450 Ambulances at a unit
cost of $100,217.
PREVIOUS RELEVANT BOCC ACTION: Approved a request to resubmit a grant application to
Florida Department of Health, Bureau of EMS, to fund an Emergency Transport Vehicle Project, at a regularly
scheduled Board of County Commissioners meeting on July 26, 2000.

Dear Mayor Neugent:
On December 14, 2000, you were informed by Secretary Brooks of the approval of your rural
emergency medical services matching grant in the amount of $81,000.00 in State funds. These
funds are to be used to support the purchase of one ambulance as revised by the department. A
copy of the approved revised budget is attached.
Your grant began on December 14, 2000, and will end on January 3, 2002. No costs may be
incurred before or after these dates. Should additional time be required to complete the project
you must submit a written extension request prior to the ending date listed above. Further, all
costs that exceed the limits of the grant award, in accordance with Section 401.113 (2)(b), Florida
Statutes, are the sole responsibility of the grantee. Your acceptance of all the grant terms and
conditions is acknowledged when funds are drawn or otherwise obtained through the
department's payment system. As Secretary Brooks indicated, no funds will be released until the
Bureau receives a copy of a preventive/scheduled maintenance plan and a five year plan for
replacement of ambulances and other program activities. Upon receipt and review of these
documents we will submit a request for release of the funds
One of the major requirements of the grant is that you must submit periodic financial reports and
a narrative report on the activities accomplished during the specified period. The report due
dates and the period each covers is as follows:
1. Beginning date of grant through April 30, 2001, report due May 15, 2001;
2. May 1, 2001 through August 31, 2001, report due September 15, 2001;
3. September 1, 2001 through January 3, 2002, report due January 18, 2002.
The final report is due 40 days after the ending date of the grant and must include copies of all
invoices, receiving reports and cancelled checks pertaining to the grant expenditures. If the grant
activities and expenditures are completed prior to the ending date, a final report may be
submitted at that time and no further reports will be required. Failure to meet these reporting
requirements will jeopardize the funding of any future grant applications submitted by your
organization.
