PREVIOUS REVELANT BOCC ACTION: Amendment #3 was on the Agenda of the 12/13/00,
BOCC Meeting, Item C13. The Amendment was approved. By letter dated April 2, 2001, the Alliance
Advised the document is null and void since the Amendment was received after the contract ended,
after the closeout report was submitted, and the signing a letter of intent.

THIS AMENDMENT, entered into between the Alliance for Aging, Inc., hereinafter referred to as the
"Alliance", and Monroe County Board of Commissioners, hereinafter referred to as the "provider", amends
contract AA 029.
The purpose of this amendment is to reflect a transfer of $9,000 from Title III C-2 to Title III C-1.
1. Attachment I, Section III, D., Method of Payment, is hereby amended to read:
The Alliance shall make payment to the provider for provision of services up to a maximum number of
units of service and at the rate(s) stated below:

2. This amendment shall begin on the date on which the amendment is signed by both parties.
All provisions in the contract and any attachments thereto in conflict with this amendment shall be and
are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level
specified in the contract and the Master Agreement.
This amendment and all its attachments are hereby made a part of the contract.
IN WITNESS WHEREOF, the parties hereto have caused this amendment to be executed by their officials
thereunto duly authorized.

Amendment 003 to Contract AA-029
Dear Louie:
I am returning the copy of the above-mentioned document originally
marked by your legal department as "Clerk's Original" copy. You will
undoubtedly notice that it has not been signed by our board President.
The reasons for it are the following:
1.
The contract in question ended on December 31,2000.
Any amendments to it should have been carried out prior
to the end date indicated. Regrettably, the amendment
sent to me was not mailed from your nutrition office until
February 26, 2001.
2.
Your grants management office has already submitted to
us its final report on Contract AA-029 and we have
transmitted the closeout report to the State of Florida.
This further precludes any possibility of further action on
this particular contract.
3.
You will recall that subsequent to Amendment 003, a
letter of intent was mailed to you on December 12, 2000
informing you of the need to decrease your total contract
amount by $12,500.00 under Title III C-1. You signed
said letter on December 19. This document, in fact,
makes the provisions of Amendment 003 null and void.
Please call me if I can be of any further assistance.

BULK APPROV ALS - COMMISSIONER MCCOY
1. Approval to remove surplus equipment from inventory via
disposal/destruction and advertising for bids and accepting bids
2. Approval of a Resolution amending Resolution No. 136-2000,
concerning fees for animal control services, in order to increase the
County License Fee for unspayed/unneutered animals from $20.00
to $35.00
3. Approval of a Contract Renewal with Barnes' Alarm Systems, Inc.
for alarm system annual certification, maintenance, and monitoring
and authorization for Mayor to execute same, and to adjust each
cost by the percentage change in the Consumer Price Index for all
urban consumers (CPI-V) for the most recent 12 months available
4. Approval of a Contract Renewal with G & K Services to provide
uniform service to the Division of Public Works, and authorization
for Mayor to execute same, and to adjust each cost by the
percentage change in the Consumer Price Index for all urban
consumers (CPI-V) for the most recent 12 months available
5. Approval to award bid and enter into a contract with Windswept
NC & Appliances for Central Air Conditioning Maintenance and
Repair for the Middle Keys facilities, and authorization for Mayor
to execute same
6. Approval of Lease Amendment with L. H. Kephart d/b/a Eaton
Street Professional Center, to increase cost of dumpster service and
to include an annual CPI computation on rent, and authorization
for Mayor to execute same
7. Approval to extend Lease Agreement with Doug Baetz on a
month-to-month basis to operate the Beach ChairlUmbrella
Concession at Higgs Beach for an amount of$150.00 per month or
10% whichever is greater, and Approval to advertise for bids to
operate the Beach ChairlUmbrella Concession at Higgs Beach
8. Acceptance of monthly report on Change Orders reviewed by the
County Administrator
9. Approval to enter into a written agreement with City Electric
System to maintain traffic signals and street lights from Stock
Island to Big Pine Key and authorization for Mayor to execute
same
10. Approval of a Road Adoption Agreement with Keyholz for Long
Beach Road, Big Pine Key, and authorization for Mayor to execute
same
11. Approval to award bid and enter into a contract with the lowest
responsive bidder for the construction of two picnic shelters at
Sombrero Beach, Marathon and authorization for the Mayor to

Dear Louie:
By means of this letter you are being notified of our intention to decrease your
Older Americans Act Title III C-1 allocation by $12,5000.00 in order to avert
the return of unused contract funds to the Department of Elder Affairs. As
previously explained during our telephone conversation with you on the week
of November 27, this will be a one time reduction which will not affect
adverseiy the total amount of your OM allocation for the 2001 contract year.
Under normal circumstances this type of revision would call for a formal
amendment to your contract. However, given the time constraints we would
be working under in trying to obtain signatures to a written amendment, we
need to rely on this letter as the official document of record.

The amended award for Title III C-1 shall be $116,887 and it shall be
budgeted as follows:
