I September 24,2001 To: Suzanne Hutton From: Rich Jones Re: Permit Transfers Please find enclosed the one page form for changing the permittee on the DEP mooring field permit. This is very straight forward. As far as the FFWCC permit goes- my contact in Tallahassee said that I can just send them a letter requesting the change and they will send a new permit to the City of Marathon. I haven't been able to get ahold of the ACOE representative yet, but I'm sure the process is just as simple for them. {;""\ ~ ,;c~; Yf,vi; ,. .~ \.~ t.9J ;.~, ,.-J'-, SEP 2 5 200' APPLICATION FOR TRANSFER OF ENVIRONMENTAL RESOURCE PERMIT AND NOTIFICATION OF SALE OF A FACILITY OR SURFACE WATER MANAGEMENT SYSTEM Permit No. 44-0164121-003-EI Date Issued May 4, 2001 Date Expires May 4, 2006 FROM (Name of Current Permit Holder): Monroe County Board of County Commissionem % Geoge Garrett, Dept. of Marine Resources, 2798 Overseas Highway Mailing Address: City: Telephone: ( 305) 289-2507 Marathon State: Florida Zip Code: 33050 Identification or Name of Facility/Surface Water Management System: Boot Key Mooring Field Phase of Facility/Surface Water Management System (if applicable): The undersigned hereby notifies the Department of the sale or legal transfer of this facility, or surface-water management system, and further agrees to assign all rights and obligations as permittee to the applicant in the event the Department agrees to the transfer of permit.
