C-3, Amendment 4002 to Contract US-1251 between the Alliance For Aging, Inc. (AAA) and the
Monroe County Board of County Commissioners (Social Services/In home and nutrition programs).
ITEM BACKGROUND: Board approved item is being rescinded due to never being fully executed
by both parties (BOCC and AAA) prior to the contract's expiration date. Because Monroe County
Social Services did deliver the agreed upon service, the AAA is replacing this redundant amendment
with a Settlement Agreement, and reimbursing the expenses as agreed upon.

ITEM BACKGROUND: This contract allows for reimbursement to the County for congregate and
home delivered meals provided to clients of the Nutrition Program. The Alliance for Aging
periodically adjusts the contract amount based on the official funding allocation published by the
Department of Elder Affairs, as noted on Page I of the contract.

THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance", and Monroe County Board of Commissioners.
The purpose of this amendment is to decrease the contract amount by $849.62, for a total
contract of $ 28,937.38. Payments will be made based on availability of funds.
All provisions in the contract and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the contract are hereby amended to conform with this amendment.
This amendment and all its attachments are hereby made a part of the contract.
