AGENDA ITEM WORDING: Approval of Resolution authorizing the write off of EMS Accounts Receivable,
in the total amount of$1,050.67.
ITEM BACKGROUND: The attached Resolution requests to write off the balances of the patient accounts that
are listed on Exhibit A, in the total amount of $1 ,050.67.
In the case of Mr. McCleery, due to recent surgery, he required the ambulance crew to assist him down the
stairs, and transport him from home to his doctor's office. He had been assured that if the physician completed
a Certificate of Medical Necessity form, Medicare would pay for the ambulance transport. However, Medicare
denied payment of the claim, stating that ambulance transportation was not necessary.
In the case of Ms. Phelps, she had been transported by ambulance to Mariners Hospital's Emergency Room.
The ambulance crew saw her waiting later in the day, and offered to take her back home. She thought it was a
courtesy service. Patient is willing to pay the mileage charge in the amount of$14.28, plus an additional
$50.00. This would cover the cost of the basic transportation, since the patient did not require any assistance
from the EMS personnel.

LOWER AND MIDDLE KEYS FIRE AND AMBULANCE
DISTRICf OF MONROE COUNTY, FLORIDA
AUTHORIZING THE WRITE-OFF OF EMS ACCOUNTS
RECEIVABLE
WHEREAS, Monroe County Emergency Medical Services provides ambulance service~ and
and
WHEREAS, attempts to collect the full amoWlt from the service recipients have been made;
WHEREAS, it has been determined that further collection efforts are fruitless and would only
incur additional costs to continue to bill the recipient as the insurance coverage afforded said recipients
is not sufficient to cover all recipient's medical damages, and recipient continues to suffer great
financial hardship; now, therefore
BE IT RESOLVED BY THE BOARD OF GOVERNORS nm LOWER AND MIDDLE
KEYS FIRE AND AMBULANCE DISTRICT OF MONROE COUNTY, FLORIDA:
Service recipient listed on attached Exhibit A accounts receivable in the amount of $1,050.67
be deleted from the County's Emergency Medical Services Department accOWlts receivable list.
