ITEM BACKGROUND: Occasionally patients require transportation to other hospitals outside of Monroe County for medical consultation and treatment not available at Fisherman's Hospital. Since private secondary transport services may not be available in emergency situations, it is necessary for the Hospital to seek resources to assist in interfacility patient transfers by ground and air. The Hospital and Monroe County Fire Rescue desire to enter into an agreement in order to facilitate the participation of the nurse during occasional interfacility critical care patient transfers. WHEREAS, Monroe County Fire Chief, on behalf of the Board of County Commissioners Monroe County, desires to execute a memorandum of understanding with Fishermen's Hospi regarding the use of registered nurses in interfacility air and ground transfers; and WHEREAS, the Board of County Commissioners desires to have the Chief execute such memorandum on behalf of the Board; now, therefore BE IT RESOLVED THAT THE BOARD OF COUNTY COMMISSIONERS OF MONR COUNTY, FLORIDA hereby authorizes the Monroe County Fire Chief to execute, on behalf of - Board of County Commissioners of Monroe County a Memorandum of Understanding for Use Registered Nurses Between Fishermen's Hospital and Monroe County Fire Rescue. Since private secondary transport services may not be available in emergency situations, it is necessary for the Hospital to seek resources to assist in interfacility patient transfers by ground and air. The Hospital and MCFR desire to enter into this agreement in order to facilitate the participation of the nurse during occasional interfacility critical care patient transfers, subject to the following terms and conditions. The Hospital shall maintain a group of registered nurses trained to manage the care of critically ill or injured patients during interfacility air or ground transfers. The Hospital will make every effort, but not guarantee 24/7 availability of a transport nurse. These nurses shall have completed flight nurse orientation to the satisfaction of the Monroe County Sheriffs Aviation Unit commander. On the first day of each January, April, July, and October the Hospital's Director of Nursing shall provide an updated roster of critical care flight nurses to the Aviation Unit commander who shall verify that each member listed is current with training required for helicopter operations. The Aviation unit commander shall sign off on the roster and forward copies to the Monroe County Fire Chief s office, who will forward it to the MCFR Medical Director. 2. During patient interfacility transfers, the Hospital's nurse shall operate under the medical direction of the transferring physician and shall administer care subject to the sending physician's orders. During ground or air transfers, the MCFR paramedic shall assist the nurse in rendering appropriate care to the patient, but will have ultimate authority with regard to command decisions surrounding the mission. During air transfers, the helicopter pilot shall have ultimate authority in all matters relative to aircraft safety and flight operations. 3. The Hospital assumes all risk associated with the nurse's participation during air and ground transfers. The flight nurse remains an employee of Fishermen's Hospital and all issues of compensation due the nurse rest with the Hospital. 4. Secondary interfacility transfers will be done on a case by case basis depending upon resources available and consultation with MCFR Medical Director. This memorandum of understanding does not commit MCFR to perform interfacility transfers beyond the capability of personnel and equipment resources, as MCFR has the primary responsibility of emergency response in the community. 5. Either paw may terminate this agreement with Afteen days' written notice to the other panty. 6.
