The Health Council of South Florida (Council) Inc. is pleased to
submit the attached application for consideration for a two-year
appointment to the Board of Directors in the placement category of Officers
purchaser. The seat became vacant due to the resignation of Michael
Paul Gluck, MD
Lannon who relocated outside of Monroe County. The Council's crldJr
Nominating Committee reviewed the applications received and Liz Kern, RN
recommends that Kerry Shelby be appointed to the Council Board of Vice Chalf

If possible, we respectfully request to have Mr. Shelby's application
submitted to the Commissioners for appointment at the next County Board Members
Commission Meeting. We would like to have him participate in the Albert Collazo

Kerry G. Shelby, CP A
Kerry G. Shelby, CPA is the Assistant member of the Key West Art & Historical
Director of Administration for the Florida Society and has served as a volunteer for the
Keys Aqueduct Authority. Originally from Key West Literary Seminar, Reef Relief,
Tennessee, Kerry has lived in Key West for and the Monroe Association for Retarded
over twelve years. After graduation from Citizens. He and his wife Diane live with
the University of Tennessee, Kerry spent their pug Samson on Von Phister Street in
eight years with the accounting firm of Key West.
