I have enclosed the Application and Certification for Change Orders #1 and #2 for the Noise Insulation
Program - Phase 2 Construction from H.I.P.P. Industries, Inc. I have also enclosed the following back-up
materials for this payment application:
1. AIA Document G701 — 2001, one for each Change Order.
2. Descriptions of Change Order Proposals for each Residence.
3. Detailed Air Conditioning and Electrical Proposals for each Residence in the Change Order.
4. Unit Price Form 00432 for each Change Order.
These documents have been reviewed and approved by C&S. I have also reviewed them, and find them
satisfactory.
