JBB Structure Assessment & Parapet Demo 08/12/2021 TASK ORDER FOR PROFESSIONAL ARCHITECTURAL AND ENGINEERING SERVICES BETWEEN MONROE COUNTY AND CPH, INC. FOR JEFFERSON B. BROWNE BUILDING STRUCTUAL ASSESSMENT AND PARAPET DEMOLITION In accordance with the Consultant Agreement for Professional Architectural and Engineering Services, made and entered into on December 19, 2018 between MONROE COUNTY, ("County") and CPH, INC., ("Consultant"), where professional services are allowed if construction costs do not exceed $4,000,000.00, or for study activity if the fee for professional services for each individual study under the contract does not exceed$500,000.00 ("Agreement"), as provided in Subsection 287.055(2)(g),Florida Statutes, as amended by Section 2, Chapter 2020- 127, Laws of Florida, pursuant to which Attachment A, Subsection 1.01 of the Agreement is hereby modified, the parties enter into this Task Order. All terms and conditions of the Agreement apply to this Task Order, unless this Task Order amends, adds, or modifies a provision or an Article of the Agreement of which will be specifically referenced in this Task Order and the amendment, addition, or modification shall be precisely described. This Task Order is effective on the 12th day of August 2021. WHEREAS, the Jefferson B. Browne building at Jackson Square has had spalling issues for many years necessitating the emergency demolition of the parapet which has become a safety hazard; and WHEREAS, the County desires to contract with the Consultant to perform Construction Administration oversight of the demolition of the parapet in additional to having a Structural Assessment done on the entire building to ensure there are no additional immediate safety hazards. NOW, THEREFORE, in consideration of the mutual promises and covenants set forth below, the parties agree as follows: 1. In accordance with Article II, SCOPE OF BASIC SERVICES of the Agreement, the Consultant will provide Construction Administration oversight during the demolition of the parapet and also perform a Structural Assessment of the entire building. The scope of work shall include but not be limited to the following: • Task A—Construction Administration of Parapet Demolition o Consultant shall review shop drawings, respond to RFIs, issue field change exhibits, and produce minor plan addendums to address RFIs. RFIs shall be responded to in a timely and efficient manner. Consultant will coordinate with Contractor on progress of project and perform site visits as needed to verify field conditions. Consultant shall review, evaluate, and issue responses to all change order requests. Consultant shall perform substantial walk, generate punch list, and perform final walk of site to certify completion of construction. Consultant will review as-builts, prepare close-out documents, and coordinate with permitting agencies to finalize construction activities, as needed. Site visits are limited to up to three total visits. Page 1 of 2 • Task B — Structural Assessment — Consultant will perform a structural assessment for life safety of the existing Jefferson B. Browne Building, along with roof assessment for the relocation of the existing tower. o Consultant shall complete visual inspection of the Jefferson B. Browne Building, including the original 1965 building and the 1985 building addition. Consultant will review previous reports, collect all required data, and make visual inspections, as required by the Monroe County Code, for life safety. Consultant will prepare one report for each building. Scope also includes visual inspection of the Freeman Justice Building roof, along with visual inspection of the existing tower of the Jefferson B. Browne Building. Consultant will prepare report and recommendations for tower relocation to the Freeman Justice Building. Scope includes minor adjustments to reports based on Client comments. Scope includes up to two (2) site visits. 2. In accordance with Article VII paragraph 7.1.1 of the Agreement,the County shall pay the Consultant a not-to-exceed total of Twenty-Eight Thousand and 00/100 dollars ($28,000.00)paid on a percent complete basis for the following phases: a. Task A — Construction Administration - $8,500.00 not-to-exceed upon review and approval by the Director of Project Management. b. Task B — Structural Assessment- $19,500.00 lump sum upon document review and approval by the Director of Project Management All other Terms and Conditions of the Agreement remain unchanged and in full forces and effect.
