A RESOLUTION OF THE BOARD OF COUNTY
COMMISSIONERS OF MONROE COUNTY, FLORIDA,
AUTHORIZING THE SUBMISSION OF A GRANT
APPLICATION TO THE FLORIDA DEPARTMENT OF
COMMUNITY AFFAIRS SMALL CITIES COMMUNITY
DEVELOPMENT BLOCK GRANT PROGRAM,
AUTHORIZING THE MAYOR AND/OR COUNTY
ADMINISTRATOR TO EXECUTE THE APPLICATION,
AWARD AGREEMENT, PROGRAM DOCUMENTS,
REPORTS, AND CLOSE OUT DOCUMENTATION
WHEREAS, the Florida Small Cities Community Development Block Grant (CDBG) program is a
competitive grant program administered by the Florida Department of Community Affairs (DCA) that
address critical housing and infrastructure needs of the elderly, disadvantaged, and low to moderate
income populations; and
WHEREAS, Monroe County is eligible to apply to DCA for funding from the Small Cities Community
Development Block Grant program; and
WHEREAS, Monroe County has identified a need for physical housing improvements to benefit low-to-
moderate income persons; and
WHEREAS, the Board of County Commissioners of Monroe County conducted a first Public Hearing
on May 19, 2010, to obtain citizen input as to the type of grant application that it should submit to the
DCA under the FFY 2009 Small Cities CDBG Program; and
WHEREAS, the Board of County Commissioners of Monroe County conducted a second Public
Hearing on June 16, 2010, to allow citizens to review and comment on a proposed 2009 Small Cities
CDBG Housing Rehabilitation grant application; and
WHEREAS, as a result of the second public hewing, the Commission agreed to submit a 2009 Small
Cities CDBG Application;
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Monroe
County, Florida, that the County hereby:
I. Authorizes the submission of FFY2010 Small Cities Community Development Block
Grant application to the Florida Department of Community Affairs;
2. Authorizes the Mayor and/or County Administrator to execute the application, award
agreement, program documents and reports including close-out documentation; and
1 Adopts the County's Comprehensive Plan as the County's Community Development Plan;
This Resolution shall go into effect immediately upon its passage and adoption by the signatures of
the Commission and the Clerk of the Commission.
PASSED AND ADOPTED by the Board of County Commissioners of Monroe County, Florida, at
a regular meeting of said Board held on the 20th day of October, 2010.

MODIFICATIONU 1 TO SUBGRANT AGREEMENT BETWEEN
THE DEPARTMENT OF ECONOMIC OPPORTUNITY
(MONROE COUNTY)
This Modification is made and entered into by and between the State of Florida, Department of
Economic Opportunity, ("the Department"), and Monroe County, ("the Recipient"), to modify
DEOIDCA Contract Number 12DB-05-11-54-01-H15, award dated September 30, 2011 ("the
Agreement").
WHEREAS, the Department and the Recipient entered into the Agreement, pursuant to which the
Department provided a subgrant of $750,000 to Recipient under the Small Cities Community
Development Block Grant("CDBG") Program as set forth in the Agreement;
WHEREAS,the Department and the Recipient desire to modify the Agreement;
WHEREAS, pursuant to the provisions of Chapter 2011-142, Laws of Florida, the DCA Division
of Housing and Community Development was transferred to the Department of Economic Opportunity
effective October 1, 2011; and the parties wish to reflect the new name.
NOW, THEREFORE, in consideration of the mutual promises of the parties contained herein,
the parties agree as follows:
Reinstate Agreement
1. The Agreement is hereby reinstated as though it had not expired.

2. Paragraph 3, Period of Agreement is hereby revised to reflect an ending date of June 29, 2014.
® Revise Activity Work Plan
3. Attachment I, Activity Work Plan, of the Subgrant Agreement is hereby deleted and is

Request for Amendment Number 1
Dear Mr. Dennis:
Monroe County is requesting Amendment 1 to Contract Number 12DB-05-11-54-01-H15 dated
September 30, 2011. The purpose of this amendment is to request a time extension from the current end
date of September 29, 2013 to June 29, 2014.
We are responding to an August 26, 2013 letter from Mr. Bob Dennis recommending that we
resubmit Amendment 1 solely as an extension of the end date. Additional time is required to complete
other documentation and actions requested by the Department of Economic Opportunity.
On behalf of Monroe County, I would like to extend my thanks for the assistance that has been
provided by the Department of Economic Opportunity in the implementation of this Community
Development Block Grant Program. If you have any questions regarding this amendment, please
