In accordance with the Continuing Contract for On Call Professional Architectural and
Engineering Services made and entered on the 19th day of December 2018, between
Monroe County hereinafter referred to as the "County" and Cardno, Inc. hereinafter
referred to as "Consultant" where professional services are allowed if construction costs
do not exceed $2,000,000.
AU terms and conditions of the referenced Contract for On Call Professional Architectural
and Engineering Services apply to the Task Order, unless the Task Order modifies an Article
of the Agreement of which will be specifically referenced in this Task Order and the
modification shall be precisely described.
This Task Order is effective on the 1511' Day of September 2021 and shall expire 60 days after
construction is complete.
In accordance with Article II Scope of Basic Services, Paragraph 2.1, the Scope of this
Task Order is amended as follows:
The project scope will include, but not be limited to, GPR scan of the roadway, soil
borings, geotechnical evaluation, evaluation of roadway rehabilitation alternatives at three
critical locations near US 1, and preparation of a roadway rehabilitation plans and cost
estimate for those three locations.
Details of the scope of services are included in the Consultant's proposal that is attached
to this task order as Exhibit A.
In accordance with Article VII, Paragraph 7.1 Payment Sum,the Payment Sum of this
Task Order is as follows:
The Consultant shall be paid monthly based on hourly rates per the continuing services
contract and estimated staff hours and the following not to exceed amounts:

PROJECT DESCRIPTION
Key Deer Blvd. is an approximately four-mile-long roadway in Big
Pine Key that begins at Wilder Road, directly north of US1, and
continues northwest to the intersection with Bryant Ave. The majority
of Key Deer Blvd. is located within the National Key Deer Refuge.
The project scope will include, but not be limited to, GPR scan of the
roadway, soil borings, geotechnical evaluation, evaluation of roadway
rehabilitation alternatives at three critical locations near US 1, and
preparation of a roadway rehabilitation plans and cost estimate for
those three locations.

SCOPE OF SERVICES
The COUNTY may request changes that would increase, decrease, or otherwise modify the
Scope of Services. Such changes must be contained in a written change order executed by the
parties in accordance with the provisions of the Continuing Engineering Services Agreement,
prior to any deviation from the terms of this Project Specific Agreement, including the initiation
of any extra work.
For KEY DEER BLVD.(as outlined)
The Consultant shall provide and perform the following services, which shall constitute the
General Scope of the Basic Services under the covenants, terms, and provisions of this Service
Provider Agreement.
Certain assumptions have been made in developing the scope for services and related fees. To
the extent reasonably possible, they are stated. If changes to the project result in changes in the
level of effort presented in this scope of services, the scope of services and related fee will be
revised by mutual agreement.
SCOPE
The scope includes the Ground Penetrating Radar (GPR) scan, geotechnical explorations,
geotechnical analysis, roadway rehabilitation alternatives analysis, cost estimating, and
preparation of a roadway rehabilitation plans for three critical locations. The limits of the project
extend approximately four miles from Key Deer Blvd.'s intersection with Wilder Road, directly
north of USI, to its intersection with Bryant Ave for the GPR scan and geotechnical
investigation. The County has identified three locations for the roadway rehabilitation analysis
and construction plans which include north of the U.S. 1 intersection, south of the Drinka Court
intersection and south of the 22"d Avenue(Church Lane)intersection.
Task 1: Roadway Repair
1.1 Geotechnical Exploration with GPR Scan
• Perform a geophysical survey to identify existing shallow voids or cavities below the
existing pavement.This will be performed with a MALA GPR system with a 1.6 GHz
(upper 12 to 18-inches)and 800 MHz antenna(3 to 4 feet).
• Perform 12 pavement cores and 12 Standard Penetration Test (SPT) Borings to
depths of 10 feet below ground surface.

11 Roadway Analysis
Evaluate geotechnical conditions, provide alternatives for rehabilitation of the roadway,
and provide recommendation based upon durability, impacts, and County's budget
constraints for the three locations identified. One field review and three meetings will be
included with this analysis.
1.3 Roadway Plans
Develop construction plans that implement the approved remediation recommendations
for the three locations identified. There will 60%, 100%and Final Plans provided to the
County for review. The plans will include the following items:
a. Key Sheet
b. Summary of Pay Items
c. Typical Section with Pavement Design
d. Plan Sheets(3 locations)
e. Temporary Traffic Control Plan near the US 1 intersection.
1.4 Cost Estimate
Prepare cost estimate of 60%, 100%and Final Plans.
1.5 Permitting(FDOT)
Conduct a Pre-Application meeting with FDOT and prepare a FDOT Connection Permit.
The Consultant will coordinate with the FDOT and provide up to three submittals in
responses to FDOT comments.
1.6 Environmental Constraints
Perform a desktop review of existing permits and environmental conditions to confirm
project will be constructed within existing environmental permit requirements.
ASSUMPTIONS
• No survey is required for the construction plans. The design of the rehabilitation will be
based on available aerial photos.
• No utility coordination is required or will be handled by the County.
• Roadway improvements are isolated to the three locations identified.
• The specifications will be based on FDOT Specification for Road and Bridge
Construction.
