Additional information on contractor requirements and worker protections under the EO is available at www. dol . gov/whd/govcontracts . Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5 . 5 (a) (1) (ii) ) . ----------------------------------------------------------- Ln The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type (s) of construction in the area covered by the wage determination . The classifications are listed in alphabetical order of ""identifiers"" that y indicate whether the particular rate is a union rate (current union negotiated rate for local) , a survey rate (weighted average rate) or a union average rate (weighted union average rate) . i Union Rate Identifiers A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than ""SU"" or ""UAVG"" denotes that the union classification and rate were prevailing for that Page 34 of 37 Packet Pg.865 D25.a classification in the survey. Example : PLUM0198-005 07/01/2014 . PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers . 0198 indicates the local union number or district council number where applicable, i . e . , Plumbers Local 0198 . The next number, 005 in the example, is an internal number used in processing the wage determination . 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014 . 0 Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate . Survey Rate Identifiers Classifications listed under the ""SU"" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in LL the survey for that classification . As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates . Example : SULA2012-007 5/13/2014 . SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates . LA indicates the State of Louisiana. 2012 is the year of survey on which these Ln classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination . 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier. y Survey wage rates are not updated and remain in effect until a new survey is conducted. i Union Average Rate Identifiers Classification (s) listed under the UAVG identifier indicate 2 that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data . EXAMPLE : UAVG-OH-0010 08/29/2014 . UAVG indicates that the rate is a weighted Page 35 of 37 Packet Pg.866 D25.a union average rate . OH indicates the state . The next number, 0010 in the example, is an internal number used in producing the wage determination . 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier. A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based. c 0 ----------------------------------------------------------- WAGE DETERMINATION APPEALS PROCESS 1 . ) Has there been an initial decision in the matter? First a comprehensive field or phone conference is done with the client to ensure that the scope of work is accurately understood by both parties. Once the scope of work is finalized a schedule to complete the work is proposed to the client. Florida Keys Land Surveying, LLC understands that time is of the essence for many projects and does its best to complete all projects within the allowable time schedule. 0 All available information is gathered by the project manager, ensuring that the field crew can be fully prepared for the site conditions and have all the necessary data required to begin the survey project. A planning session is then scheduled, involving the project manager and the field crew party chief. Survey methods are discussed and selected based on the site conditions and project 0 scope. The methodology agreed upon is then peer reviewed by another project manager, professional land surveyor or the responsible land surveyor in charge. The field work is completed by the field crew under the direct supervision of the project party chief. y All the field work is completed utilizing the agreed upon methodology including check shots 0. throughout the project, such as RTK or conventional radial ties to independent control points. If any of these checks show results outside the error tolerance for the project at hand, all work is stopped until the problem can be found and rectified. Party chiefs are directed to contact the project 2 manager if any such issues are discovered, so that the project manager can help to quickly resolve the issue. FKLS utilizes field data collectors that have sim cards with email capacity and I-Pads which allow for direct communication of data from the field to the project manager in the office. After the project field work is completed, project managers discuss the field results with the party chief. Once the field work is downloaded the project manager post processes all the field data and compares the results to the project scope to insure that the required data has been collected, and that all of the scope requirements are being met. 0 Drafting is accomplished on all projects by a draftsman who is experienced in the type of work being performed, and who has been kept informed of any special circumstances or challenges of the particular project. The party chief and the field crew examine all map products at every step of the mapping process to assure that the final product represents the actual field conditions. - 0 Final map products are then carefully reviewed by the project manager, and verified for accuracy before the product is considered finalized. A final check of the entire project process including all field notes, field data, drafting and the final product is then performed using a peer review process by another project manager and/or professional land surveyor. Once the peer review process is completed and signed off on by the reviewing party the final product and all the supporting project data is sent to the responsible surveyor in charge for an additional comprehensive final review and signature of the deliverable. c Packet Pg.875 i D.25.b FLORIDA KEYS LAND SURVEYING 19960 OVERSEAS HIGHWAY, SUGARLOAF KEY, FL 33042 PHONE: (305) 394-3690 EMAIL : FKLSemail(jatgmail.eom www.,floridakeyslandsurveying.net 0 o - 4- RELEVANT EXPERIENCE, REFERENCES AND PAST PERFORMANCES ON SIMILAR 0 0 0 0 0. 0 PROJECTS CL 0 0 0 0 0 0 0 Packet Pg.876 i D.25.b FLORIDA KEYS LAND SURVEYING 19960 OVERSEAS HIGHWAY, SUGARLOAF KEY, FL 33042 PHONE: (305) 394-3690 EMAIL : FKLSemail(jtt 1nyil.com www.,floridakeyslandsurveying.net 0 0 - 4- RELEVANT EXPERIENCE Florida Keys Land Surveying, LLC is a state of Florida registered professional land surveying firm located on Sugarloaf Key.
