MONROE COUNTY
THIS 5th AMENDMENT to Agreement for Disposal of Remains is made and entered
into this 15th day of September, 2021, by and between Monroe County, a political subdivision
of the State of Florida, whose address is The Historic Gato Building, 1100 Simonton Street,
Suite 2-257, Key West, Florida 33040 (hereafter, "County"), and Dean-Lopez Funeral Home,
a Florida business whose address is 418 Simonton Street, Key West, Florida 33040 (hereafter
"Contractor").
WHEREAS, on the 17th of September 2014 the County and the Contractor entered into
an Agreement for disposal of remains (hereafter"Original Agreement"); and
WHEREAS, Dean-Lopez Funeral Home is willing and able to provide for disposal of
remains and perform related services; and
WHEREAS, the parties have found the original agreement to be mutually beneficial
now therefore,
IN CONSIDERATION of the following mutual promises and benefits,the parties agree
as follows:
Section 1. Paragraph 1 of the Original Agreement is amended to read as follows:
1. TERM. The term of this Amendment is two years beginning October 16,
2021 and terminating on October 15, 2023.
Section 2. All other provisions of the September 17, 2014 Original Agreement not
t o sistent herewith, shall remain in full force and effect.
