Notice of Substantial Completion M1t M1 Key West International Airport 3491 South Roosevelt Blvd. Key Vilest, Florida 33040 December 7, 2021 Mr. Robert Lopez General Asphalt 4850 NW 72"d Avenue Miami, FL 33166 Subject: Substantial Completion Rehabilitate Taxiway A Key West International Airport Monroe County Project No. —GAKAP159 Mr. Lopez, The work identified in the contract documents for the above contract dated September 181h, 2019 has been reviewed and found, to the Engineer's best knowledge, information, and belief, to be substantially complete. The date of Substantial Completion is November 11'h, 2021. The Contractor will complete or correct the Work as approved by the Owner's authorized engineer on the list of items within 75 days from the above date of Substantial Completion. A list of items to be completed or corrected is attached. The failure to include any items on such list does not alter the responsibility of the Contractor to complete all Work in accordance with the Contract Documents. This list may be amended based on the results of the final inspection. Unless otherwise agreed to in writing, the date of commencement of warranties for items on the attached list will be the date of issuance of the final Certificate of Payment or JWdate of final payment, whichever occurs first. P/easecoondinateas necessary to ensure that these issues are addressed prior to demobilization. 1. General items: a. Cleanup/restore upper air site and Contractor stag/nc8avdownareas. b. Cleanup/restore haul routes. o. Ensure drainage grates/inlets are clear of sediment,grass and construction debris, d. inspect,cleanup/remove any remaining debris, i.e, pallet material remnants,etc.from site. e. Restore sod in selected areas disturbed by construction. Z. Complete final pavement marking and pavement marking removal with high pressure water. 3. Repair/replace inoperable display screen on Voltage Regulator#2(East circuit), 4. Complete "Homerun"conduit terminations, associated pavement repairs,and placement of cable. S. Leve I/re-com pact conduit trenches at wind cone feeds(from runway to W/C). 6. Complete all required electric circuit tesdng/meggin8. 7. Complete pavement repairs as indicated in separate correspondence.
