SUBJECT: Proposed Change Order to be Approved
Attached is proposed Change Order#1 Professional Assessment of Per/Polyfluoroalkyl Substances (PFS)
Services for Joe London Fire Academy project. This change order increases the final contract sum by
$29,200.00. The substantial completion date is to be determined.
Monroe County Ordinance 027-1998 and subsequent amendments provide authority for the
Administrator to approve change orders within specified limits after providing Board of County
Commissioners with at least 24 hours to review them and pose any questions or objections that they
may have.
It is the Administrator's intent to approve the attached change order under that authority on Friday,
May 27, 2022.

The PFOS plume was not fully delineated in the previous testis . Additional wells must be installed and

Dear Kevin:
Cardno was issued a second amendment to Task Order for Professional Assessment of
Per- and Polyfluoroalkyl Substances at the Joe London Fire Training Academy on
January 21, 2022. This Task Order Amendment included remediation of impacted soils,
quarterly groundwater monitoring and reporting, and installation of six (6) groundwater
monitor wells at the referenced site. The request for six (6) wells was made in error due
to confusion regarding multiple comment letters from FDEP regarding the prior
assessment report results. In fact, four (4) wells were being requested by FDEP.
Cardno installed and sampled the four (4) wells requested by FDEP in February 2022,
and has determined that the PFAS impacts to groundwater extend beyond the recently
installed monitor well network to the south, and that additional wells will be required to
delineate these impacts. Based on discussions with Monroe County staff, Cardno is
proposing three (3) additional wells at the locations shown on the attached Figure, and is
adding these wells to the proposed quarterly sampling events included in the Task Order
(Amendment 2) under Task 4.
Change Order Request
This Change Order (CO) request is for installation of three (3) additional monitor wells,
and the addition of these wells to the three future quarterly groundwater sampling events
included in the previous Task Order assignment (including sampling, laboratory analysis,
and reporting). The CO request for Task 1 (well installation) is $20,600 and the CO
request for Task 4 is $8,600 (Total CO request of$29,200). This change order will
bring the total Amendment 2 Task Order amount to $185,160.
This CO request takes into account the remaining budget within Task 1. Specifically, the cost
proposal for installation of six (6) wells in the Amendment 2 Task Order was $26,620, of which
approximately $5,000 remains in that budget. The estimated cost for installation of three wells

included in this change order was $25,600. However, subtracting $5,000 results in a CO
request of$20,600 for Task 1.
Schedule
As with the last round of well installations, sonic drilling Contractors are backlogged many weeks.
In anticipation of CO approval, we have requested this work be added to the Contractors
schedule for the first available opening. At this time, that would be the later part of June 2022.
In the meantime, Cardno will reach out to FDEP with a modified proposal for well installation and
sampling, including a modified schedule (pending approval to proceed by Monroe County).
We appreciate the opportunity to provide this proposal for environmental assessment of this site.
