OG ASSISTA tD ,A, E Date 6 1/12/22 Exhibit A Key West International Airport(EYW) Monroe County, Florida Access Road Full-Time Construction Administration and Resident Inspection Scope of Work—Amendment A PROJECT DESCRIPTION The proposed project is a two-lane landside access road that provides connectivity from S. Roosevelt Boulevard to Key West International Airport (AIRPORT). The proposed access road is located on the west side of EYW adjacent to the current Department of Motor Vehicles (DMV) office and one-hour parking lot and will require reconfiguration of the impacted parking areas and commercial transportation staging area. This project consists of construction of the new access road, fuel delivery route, reconfigured commercial transportation parking at the AIRPORT and conversion of Gate #5 to a Sally Port gate. The project includes the construction of an approximately 550' long two-lane landside access road that provides connectivity from S. Roosevelt Boulevard to EYW's fuel farm, fixed base operator (FBO), overflow parking, commercial transportation and airport support areas (see Figure 1). The proposed access road will separate airport support services from airport passenger traffic on Faraldo Circle, reducing congestion in the terminal area. The proposed access road will also eliminate fuel truck and other large vehicle traffic from ingress/egress to the passenger terminal area,as well as reconfiguring impacted parking and staging areas to provide enhanced customer parking and hotel shuttle and Transportation Network Company(TNC)staging away from the congested terminal curb. Figure 1 +' � uIIIIIIIIMi � '' wm«rmmmma�uu�L� r McFarland Johnson Key West International Airport Access Road—Full-Time Construction Administration and Resident Inspection im McFarland Johnson Amendment A—Scope of Services PSO 2021-03A SCOPE OF WORK The CONSULTANT (McFarland-Johnson, Inc.) will provide the following additional professional services under this scope of work as related to the above-described project including the design and permitting of an increased sized (from 6 in to 8 in diameter) water line from South Roosevelt Blvd. along the access road to the FBO building area, additional site design/permitting associated with a new fuel containment pad within the project limits and construction administration services for access road signage installation. 1. Administration/Project Management The following items of work shall be completed under the Administration/Project Management task: A. Coordination and design reviews of Signature Terminal Building and Fuel Tank Expansion for impacts to existing Access Road design and construction documents. B. Design revisions to existing construction documents to accommodate items in letter A above for permit modifications. C. Revisions to Submission of revised construction documents and a utility permit application to the Florida Keys Aqueduct Authority. D. Coordination efforts with the Florida Keys Aqueduct Authority to discuss and prepare an amended FKAA Utility Permit. E. Coordination and submission to the Southwest Florida Water Management District for site plan revisions F. Coordination and submission to Monroe County for site plan revisions. G. Sign submittal review and construction administration. 2. Final Design(100%) A. Incorporate revised water line, and fire hydrant into conformed plans and specifications for construction purposes. B. Obtain permits, as required, to allow construction of the project. Any changes to the following assumptions will be considered and change in project scope and may require a change in the project budget: • The water line will be increased from 6 in diameter to 8 in diameter and a terminating valve and end cap near the FBO buildings will be provided for future connection for the terminal addition project. Construction Administration Services Utilizing the CONSULTANT's previously developed architectural and electrical engineering drawings and specifications incorporated into CLIENT'S overall construction document deliverable, CONSULTANT will answer the Contractor's Request for Interpretation, review three construction submittals, attend one virtual meeting, and coordinate services with CLIENT. Once the construction is complete, CONSULTANT will generate record CAD files for CLIENT's use. SERVICES EXCLUDED FROM THE SCOPE OF THIS PROJECT: The following items and tasks are excluded from this agreement. These services may be provided, if requested by the CLIENT, under an amendment or other contract and may require additional compensation. • Permits and review of regulatory requirements. • Reproduction and distribution of printed documents, as deliverables will be issued electronically; • Supplemental information and design for unknown conditions or substantive field changes; • Value Engineering, Re-Design or Re-Bid of the Project in the event of lack of funding, contractor pricing, or other issues; • As it is assumed all meeting will be held virtually via Microsoft Teams, travel costs are excluded. PROJECT SCHEDULE Schedule for the work of this Project is proposed as follows. Exact dates are to be determined and may be adjusted within reason. Each phase of services included in this proposal will begin upon receipt of approval of prior phase and Notice to Proceed from AIRPORT. Services requested outside of this time frame may require additional time and/or compensation. • Construction: 10 weeks COMPENSATION The work described in this proposal of services will be performed on a lump-sum basis. CONSULTANT would be compensated as follows: • Total Lump Sum cost to provide design services (Task A) is $6,445.00 including expenses. See attached spreadsheet for breakdown of fees by task.
