Dear Gentlemen,
At this time I am requesting that you reimburse me for the amount of $180.80 from permit
# 12300729 dated 02/23/2012. 145 Fairwich Ct. Tavernier, Fl. 33070
1 have not done any work under this permit. The owner and myself cannot reach an
understanding for my payment schedule.
I also ask that no one may reactivate this permit nor use my drawings to request another
permit.
I have informed the owner to hire someone else and pull another permit.

after the effective date of the permit.
4. Once commenced, (with an approved inspection) this permit will remain active as long as
there is an approved inspection within one -hundred and eighty (180) days of the last
approved inspection.
5. No "partial' inspections of any sort will count toward satisfying the 180 day requirement.
6. Certificate of Occupancy must be secured before this building can be used or inhabited for
any purpose.
7. Do not remove this card until issuance of a Certificate of Occupancy.
