ITEM BACKGROUND: Approval of Amendment 003 to the HCE Contract will adjust the unit rate
for services under contract as a result of the 2011 CCE RFP. New rates are effective March 1, 2012
PREVIOUS RELEVANT BOCC ACTION: Prior approval granted for Amendment 2 to the Home
Care for the Elderly (HCE) Grant Contract #KH- 1172 on 12-11 -11.
CONTRACT/AGREEMENT CHANGES: Adjustment of Case Management Rates from $47.86 to
$50.48. Contract amount stays the same. There is no decrease or increase in the total of the contract
SUM.

THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance", and Monroe County Board of Commissioners.
The purpose of this amendment is to adjust the unit rate for services under contract as a

Amendment 002 CONTRACT KH 1172
THIS AMENDMENT, entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance", and Monroe County Board Of Commissioners.
IIA. The purpose of this amendment is to increase the amount allocated under the
extended contract by $6,347.00 for Case Manaaement As a matter of reference, the
Alliance projects to process and pay the Basic Subsidy to qualified caregivers
participating in the HCE program of approximately $9,757.00.
At the time of execution of this amendment, $4,752 are projected to be available for §p2gial
Subsidy including, Specialized Medical Equipment and Supplies —Vendor (SCSV) and
Specialized Medical Equipment and Supplies -Consumer reimbursement (SCSM). These
funds are subject to change depending on funding availability. The Providers will be notified
in writing of any anticipated changes in funding availability.
METHOD OF PAYMENT:
