DESCRIPTION OF FACILITY/EQUIPMENT:
This document, issued under the provisions of Chapter 24, Miami-Dade County Code (Dade County Pollution
Control Ordinance), shall be valid from June 1, 1998 through May 31, 1999. The above named permittee is
hereby authorized to operate the pollution control facility at the above location which consists of the
following:
A 120,000 gpd pretreatment system for an industrial laundry with:
1. A lint & sand interceptor with 120 mesh screen, a 15,000 gallon equalization tank and a 15,000 gallon
emergency tank.
2. Wastewater pit with high/low level controls, two trash pumps and a magnetic flow meter.
3. Chemical treatment system with two flocculation cells and one coagulation cell, including pH monitor and
chemical feed system.
4. DAF unit, 200 ft. sq. surface area; 1.5 gpm/ft. sq.
5. A 2,000 gallon Sludge storage tank with alarm.
6. A RVP vacuum filter, 3' diameter x 6'.
7. Ancillary equipment and controls.
Subject to specific and general conditions listed below and in the following pages of this document (if
any) .
SPECIFIC CONDITIONS:
01 Hazardous waste, Sludge and other industrial wastes and/or wastewater must be transported by DERM
approved transporters. Analytical data must be submitted to DERM, for all waste and/or wastewater that
is disposed of locally. Failure to gain approval from DERM, for disposal, will result in rejection of
your waste at the disposal facility.
02 A monthly report of operations shall be submitted to this Department on or before the 15th of the
following month in accordance with the enclosed monitoring program. Said monitoring program is a
condition of this operating permit and is made a part hereof by reference. The information in the
report shall include:
1. Average daily wastewater flow (gals). Number of batches treated.
2. Analytical data of Cu, Ni, Cr (TOT), Zn, pH, Phenols, oil and grease, TTO and dissolved hydrocarbons
in final effluent. Include sampling date, sample type, sample location, and analytical method used.
3. Amount of sludge/hazardous waste disposed of, date, name of hauler and final destination.
03 A Quarterly report shall be submitted to this Department on, or before the 15th of the following month,
including:
1. Analytical data, from a certified laboratory for locally disposed Sludge and/or wastewater, for TCLP
Pb, Cd, and Cr, EPA Series 601 and 602 Organics, TRPH.
2. Amount of sludge/hazardous wastes disposed of, date, name of hauler and final destination.
Failure to submit this information may cause rejection of your waste for local disposal. Sludge and
other industrial waste shall only be transported ,by DERM approved haulers.
