G. , I 9 <; 9 ,by and between the Florida Department ofHeath/Monroe County Health Department (DOH) and the Board of County Commissioners of Monroe County (County). WHEREAS, the County and DOH are contracted to provide public health services to the citizens of Monroe County under Florida Statute 154.01, and WHEREAS, the present facility at 148 Georgia Avenue, Tavernier, does not adequately ensure patient and staff safety, provide privacy during eligibility determination and examination, and enable an efficient movement of patients through service areas, NOW THEREFORE, the parties agree to the following provisions: 1. The County has included $400,000 in the fiscal year 1999-2000 capital improvement plan for construction activities to build the DOH portion of an Upper Keys facility that will remain committed for this purpose for the duration of project. 2. The DOH through the Monroe County Health Department Trust Fund has included $200,000 in the Special Project category of the DOH Core Contract for construction activities to build the DOH portion of an upper Keys facility. 3. The DOH has been provided through Legislative Budget Item 528A from fiscal year 98-99 a Fixed Capital Outlay of $400,000 for improvement activities for the DOH portion of an upper Keys facility, including, but not limited to, planning, permitting and future contingencies. 4. Under a separate agreement, the County and DOH will define the responsibilities of both parties concerning the procurement and payment for services for construction activities to build the DOH portion of an upper Keys facility. 5. The term of this agreement shall be until the completion of the construction project. 6. The County and DOH shall cooperate in the selection of the appropriate location for the project.o -n :x 0 0 7.
