AGENDA ITEM WORDING: Approval to renew current Life Insurance and Accidental Death &
Dismemberment (AD&D) policy with The Hartford Life Insurance for one additional year. Rate
increase of .01 per $1000 for Basic Life; no increase in AD&D rate.
ITEM BACKGROUND: Last RFP done June 2010. Next RFP scheduled per Purchasing Policy in
2013.
PREVIOUS RELEVANT BOCC ACTION: Policy originally secured in 2000; RFP's done in 2001,
2007 and 2010. RFP done in 2010 resulted in recommendation and BOCC approval of The Hartford
Life Insurance for a two year term policy.
CONTRACT/AGREEMENT CHANGES: Increase in Basic Life premium to .51 per $1000; AD&D
premium .02 per $1000. Rate guaranteed until October 1, 2013.

This item requests approval to renew the current basic life and Accidental Death & Dismemberment Insurance
Policy with Hartford Life Insurance for the period of October 1, 2012 through September 30, 20I3. This renewal
will be the third renewal of the current policy and therefore, in accordance with purchasing policies and procedures,
will be put out for bid next year.
This renewal represents a rate increase of .01 per $1,000 composite for the basic life coverage for both employees
and retirees and includes the same terms and conditions. The annual approximate difference is a $4,000 increase
which is based on the current enrollment as of the date of this memo.
