ITEM BACKGROUND: On June 22, 2006 All Keys Construction was issued a purchase order for emergency repair on Bridge 904120. Further inspection has revealed additional work will be required to repair the bridge. PREVIOUS RELEVANT BOCC ACTION: On June 21, 2006 BOCC granted approval to waive purchasing policies and procedures and issue a purchase order for emergency repairs to Bridge 904120 on Boca Chica Road. CONTRACT/AGREEMENT CHANGES: Increase contract by $45,000.00-Due to increased surface area, new and larger quantity reinforcing steel, more form material and more of all used products such as concrete and bonding agent. STAFF RECOMMENDATIONS: Approval as stated above. TOTAL COST: $144,800.00 BUDGETED: Yes..lL.. Spedalizillg in CfJ"a'~re Repair ami Pahrfillfj July 26, 2006 Monroe County Re: Geiger Key Bridge - Addendum I Scope of Work: To furnish all labor, materials, equipment, taxes, and insurance's: Repair an additional 26 linear ft of damaged tie beam, and approximately 216 sq ft of Deteriorated slab bottom Steps of Repair Method: 1. Clean and remove all unsound concrete, rust, grease, oil, paint, and other foreign materials that will inhibit performance. 2. Remove I' concrete around the rusted rebar to effectively treat the rust with a phosphoric acid wash to stop further rusting and coat all exposed rebar with an organic zinc epoxy primer. 3. Apply bonding agent, and replace concrete using a polymer-modified grout reinforced with glass fibers and extended aggregate. 4. Topcoat with a polymer modified stucco mix to match existing stucco. Note: reinforced steel shall be removed and replaced if deteriorated by 25% or more by cutting out old, and replace by drilling, and epoxying into sound concrete, treat as above prior to placement of grout. Proposal Amount: $45,000.00 Forty Five Thousand dollars, and no/cent., Exclusions: Permits, additional Engineer Services, and Army Corp of Engineering Terms: 25% mobilization, Bi-weekly payment based on progress, balance at completion.
