Horton County Administrator Airport Director May 2009 Prepared by: REQUEST FOR QUALIFICATIONS KEY WEST INTERNATIONAL AIRPORT IMPLEMENTATION OF NOISE INSULATION PROGRAM Monroe County is soliciting Statements of Qualifications (SOQ) from Professional Firms who would like to be considered for program management, design, and construction administration work for all remaining phases of the airport's Noise Insulation Program. The scope of services includes, but is not limited to, program management; analysis of design alternatives; construction design; preparation and distribution of bid documents, plans and specifications; pre- and post-acoustical testing; construction administration; construction observation and inspection necessary to improve the noise insulation of individual residential dwellings in the vicinity of Key West International Airport. There are approximately 206 residential condominium units and 16 individual residences potentially eligible for participation in the Noise Insulation Program. Modifications to residential condominium units and individual residences involved in the Noise Insulation Program may include, but will not necessarily be limited to the following: • Replacement of existing doors and windows with acoustically-rated units; • Replacement of existing screen/storm doors with acoustically-rated units; • Installation of acoustically-rated storm doors over existing doors or over acoustically-rated replacement doors; • Installation of code-compliant storm protection; • Provision or enhancement of thermal insulation in attics, walls, crawl spaces; • Provision of air conditioning for residential units without a system or where the present system is obsolete, or retrofit of existing central air conditioning systems to achieve requisite Noise Level Reduction. Firm / Team Qualifications: Qualifications of the firm, sub-consultants, and personnel assigned to perform the work. Use only SF 330 for this section. 2. Project Organization & Management: Include project organization chart, expanded resume of project manager, and amount of time the project manager will be committed to this project. 3. Project Approach: Concise discussion of your approach to performing the Scope of Services including specific responsibilities of key team members and sub-consultants. 4. Experience: Specific related experience of all proposed key personnel, including subconsultants. 5. DBE Participation: Identification of DBE firms proposed for use in performing the work and copies of their DBE certificates(Florida Statewide& Inter-Local Certification). EVALUATION CRITERIA All SOQ received will be evaluated regarding compliance with the RFQ, FAA Advisory Circular 150/5100-14C "Architectural, Engineering, Planning Consultant Services for Airport Grant Projects," and Florida statutes and laws. One or more of the highest rated firms may be requested to submit additional information and/or participate in an interview. Consultant Selection will be based on the following criteria: 1. Qualifications of the Firm/Team (weighted 16% in selection criteria): Demonstrate ability and experience of the principal consultant and its key personnel. Firmrream should demonstrate its familiarity with the Florida Building Code and industry-accepted policies and procedures. 2. Project Similarity (weighted 16% in selection criteria): SOQ should include specific examples of similar projects performed for similar clients. References and points of staff contact should be included. SOQ should include the approach and methodology taken to complete similar projects for other clients. 3. Past Performance(weighted 16% in selection criteria): SOQ should demonstrate the Firm/Team's ability to meet schedule demands and complete projects by agreed-upon deadlines, and to complete work on or under budget. References should be included that help Key West International Airport ascertain and determine the quality of the projects completed, and the satisfaction of clients where projects have previously been undertaken by the proposing Firm/Team. Firm/Team should disclose all arbitration, litigation, lawsuits, and other legal actions associated with projects Firm/Team was involved with in the past five years. Page 2 of 13 4. Project Manager to be Assigned to this Project (weighted 20% in selection criteria): A key element in the success of providing professional services is the Project Manager. SOQ should include resume and detail of experience relevant to the provision of these services. SOQ shall also include longevity with proposed Firm/Team, where Project Manager will be based, and his/her ability and proposed mode of travel to Key West International Airport. SOQ should also demonstrate the Project Manager's understanding of the challenges and opportunities of this project, the Sponsor's concerns and expectations, and Project Manager familiarity with Key West International Airport's Noise Insulation Program, 5. Approach to Performing Work (weighted 17% in selection criteria): SOQ should include approach to performing the work, based upon the following Preliminary Scope of Services. This Preliminary Scope of Services is not intended to be all inclusive of the Work to be performed. This Scope has been prepared so the replies will be written using a common basis. Your own experience should allow you to amplify on the Scope in your response to the RFQ. Also, your response will serve as a guide to an eventual "Scope of Services"to be included in the Contract. 6. Proximity (weighted 15% in selection criteria): SOQ should indicate the accessibility of principal firm to Key West International Airport, and proximity of any firms proposed as sub-consultants. SOQ should include Firm/Team's proposed method of travel between their office and Key West International Airport, describing the cost of such travels, time requirement, and ability to respond on short notice. 7. Disadvantaged Business Enterprise (DBE) Program (weighted 3% in selection criteria): SOQ should describe the firm's willingness, commitment, and ability to meet DBE program goals established with the FAA for Key West International Airport. The DBE goal is 11% of the total fee. SOQ should include DBE performance on past projects, as compared to the DBE goals of those projects. GENERAL NOTES Monroe County and the Key West International Airport reserve the right to reject any and all SOQ submitted in response to this RFQ for any reason. Page 3 of 13 PRELIMINARY SCOPE OF SERVICES KEY WEST INTERNATIONAL AIRPORT IMPLEMENTATION OF NOISE INSULATION PROGRAM This Scope of Services will describe tasks necessary to implement the Noise Insulation Program for approximately 206 residential condominium units and XX homes located in the Key West International Airport area. TASK 1 PROJECT MANAGEMENT The Consultant will perform general project-related management and coordination with the airport and the FAA. This task includes preparation of monthly invoices and progress reports, FAA coordination, project management plan updates, sub-consultant coordination and agreements, and schedule updates. This task also includes weekly conference calls with the Consultant Team and related documentation. URS Corporation is the County's Airport Consultant. URS will be the Airport Noise Program Coordinator(ANPC)for the duration of the NIP. TASK 2 MAINTAIN PROJECT OFFICE AND PROVIDE INFORMATION TO THE PUBLIC The Consultant should closely coordinate all NIP activities with the ANPC. The Consultant will attend meetings of the airport's Ad Hoc Committee on Noise to provide status reports on the progress of the Noise Insulation Program. They will also participate in and/or conduct up to two other meetings as appropriate (e.g., presentations to Homeowner's Associations, etc.) to disseminate information about the Noise Insulation Program and progress in noise mitigation efforts. These meetings will coincide with other project-related trips to Key West. The Airport Noise Program Coordinator shall communicate meeting dates and times with the Consultant Team. The Consultant will be available to discuss technical issues with Homeowners via telephone, or in person via the Airport Noise Program Coordinator, as necessary. Examples of noise insulation materials, such as different window and door assemblies, will be available in the Project Office for inspection along with photographs of completed work, product brochures and application forms. The Consultant will schedule appointments as needed to meet with Homeowners in the Project Office to view and discuss the window and door samples, product displays, etc. If these appointments include participation by the Consultant Team then these appointments will coincide with other project-related trips to Key West. Page 4 of 13 TASK 3 UPDATE PROGRAM POLICIES The Consultant will update the Policy and Procedures Manual and the Architectural Treatment Policies and Procedures that describes the step-by-step process for the Key West International Airport Noise Insulation Program. The Policy and Procedures Manual and the Architectural Treatment Policies and Procedures will guide the implementation process and will continue to be modified as the airport and Ad Hoc Committee review the ongoing development of the program. Copies of the updated manual will be discussed with and distributed to the airport, Ad Hoc Committee, BOCC, and FAA, as appropriate. TASK 4 LEGAL DOCUMENT ADMINISTRATION The Consultant's Project Manager will obtain the title commitments for each of the participating properties from a Florida licensed Title Company. The Title Company will provide an opinion of title, which would identify the vesting of ownership for the property. It would also identify any and all mortgages, liens, and encumbrances on the property, which would require signature of consent to the avigation easement and subordination of the easement. The Consultant's Project Manager will coordinate the activities in obtaining the consents and subordination agreements. This entails identifying the lien holder, providing the lien holder with the consent and subordination agreements, explain the program and the lien holders' rights, and respond to any questions or concern the lien holder(s) may have with the consent or subordination agreement. The Consultant's Project Manager will manage the receipt of consents and subordination agreements by tracking it in the database. Following completion of Final Inspection, the Consultant will submit the Avigation Easement to the Clerk of the Circuit Court to be recorded. The Consultant will prepare the necessary paperwork and pay the fees associated with recording the Avigation Easements. The Consultant's Project Manager will record the avigation easement upon execution by the Homeowner. The Consultant's Project Manager will forward all recorded documents to the Title Company for issuance of the final Title Policy. The Consultant's Project Manager will maintain the title policies in the parcel file. TASK 5 DOCUMENT ARCHITECTURAL, MECHANICAL, AND ELECTRICAL CONDITIONS The Consultant will collect sufficient architectural, mechanical, and electrical data on each home to document the existing conditions for proposed treatment recommendation. At this time, general building metrics, unusual building conditions, and relevant building code conditions will be documented as a basis for future treatment recommendations and construction detailing. The Consultant will photograph and prepare written documentation of the existing conditions at each residence. Detailed floor plans will be developed showing the number and location of all windows, doors, perimeter penetrations, and potential treatment areas (e.g., wall, ceiling, attic, soffit, skirting, exposed framing, etc.). Door and window schedules will elaborate types, sizes, and styles of these elements. The mechanical and electrical information will indicate the existing HVAC systems configuration and capacity. Page 5 of 13 Structural inspections are required for homes that are proposed to receive ceiling treatments. The Consultant will schedule appointments with each eligible participant for on-site visits a minimum of two weeks in advance, allowing approximately 3 hours per home. The Consultant Team will conduct on-site visits to collect data on each of the homes pre-selected for each phase, over the course of a three-week period. The Consultant will review the steps to participation, explain all the requisite paperwork the Homeowner must fill out in order to participate in the program, secure title and mortgage information, and answer specific Homeowner questions. The Consultant Team will collect architectural data on the home, discuss treatment options, technical issues, and other noise insulation measures with the Homeowners at the on-site visits. TASK 6 PERFORM PRE-MODIFICATION NOISE AUDIT Immediately following the architectural survey, a pre-modification noise audit of selected homes will be conducted. Measurements will be used to verify the necessity for modifications identified during the design process. Pre-modification data will be compared with post-modification measurements to document the achieved increase in noise reduction, and as a check on quality control. Pre-modification measurements will be used to consult with the Consultant Team on treatment protocols. The number of homes to be measured will be approximately 20%to 25% of the total number of homes being insulated in the phase. The sample will be selected to represent the range of housing types expected. Usually, each home that is measured before construction will be measured afterwards as well. The Consultant will schedule appointments with the selected Homeowners. The Consultant will conduct one on-site visit to collect data on the selected homes. The noise level reduction (NLR) measurements essentially consist of placing a microphone inside each of the rooms that will be measured. The electrical signal of a pink noise source is fed through the amplification/loudspeaker system and is filtered to simulate aircraft noise. The technician will measure the average interior sound level within the room and the sound level directly outside each exterior element (i.e., doors, windows, and walls). An overall NLR will be mathematically computed based on the measured test data and the acoustical conditions of each room. TASK 7 DEVELOP ACOUSTIC DESIGN PACKAGE The Acoustic Design Package documents existing conditions, floor plans, and miscellaneous items that are to be incorporated into the design documents. It also contains the recommended acoustical treatments to the windows, doors, ceilings, and walls. The acoustical designs will be aimed at reducing interior noise levels to meet the FAA AIP Handbook goal of an interior noise level no greater than 45 dB DNL. In addition, the modifications will be designed to achieve an average 5-dB improvement. The Consultant will develop the noise insulation designs in full compliance with the policies, standards, and specifications issued by the FAA for AIP projects. Page 6 of 13 The Acoustic Design Package will include the following information: • General Requirements, • Elevation Photographs, • Special Requirements, • Structural Description, • Contractor General Notes, • Homeowner Requirements, • Schematic Architectural Plans, • Schematic Mechanical Plans, • Schematic Electrical Plans, • Product Selection Icons, and • Door and Window Schedules. The Consultant will develop and distribute the Design Packages (for all residences included in the phase) to the Consultant Team for internal review and discussion. Following internal review and revision as necessary, the Consultant will meet with the Airport Manager and the Airport Noise Program Coordinator to review the Acoustic Design Package and discuss any outstanding issues. Any necessary revisions to the Acoustic Design Package will be completed prior to presentation to the Homeowners. TASK 8 PRESENT DESIGN PACKAGE TO HOMEOWNERS The Consultant will review the Acoustic Design Package with each Homeowner, including all treatment recommendations and product selections such as color, style, and material. The Consultant Team will review changes proposed by the Homeowner prior to approval. Issues arising from Homeowners' refusal of proposed treatments will be addressed according to Program policies. The Consultant will schedule appointments with the Homeowners to present the Acoustic Design Package and the review of legal documents. TASK 9 DEVELOP BID DOCUMENTS The Consultant will develop a complete set of construction documents that meet all FAA requirements and the requirements of the local authorities. The construction documents will be based on the products and treatments in Phases 1 through 7, and will include written specifications that describe the products, suppliers, materials, and workmanship that are acceptable and which meet the technical requirements of the project. The construction documents will also include the plans and details referenced to the specifications sections. The documents will be of sufficient detail to be used for the formal bid process as well as for the actual construction phase of the work. Once all of the Homeowners in a particular phase have signed-up (or declined to participate) the Consultant will compile all of the Concept Packets for those properties included in the bid package. Page 7 of 13 The bid package set will contain at least the following information for each residence: • Floor plan that shows the existing construction and modifications that shall be made for each habitable level and the roof plan; • Door and window schedules that are keyed to identifying marks on the floor plans and that call out the specific modifications that are required, complete with elevation drawings of typical doors and windows; • HVAC upgrade requirements including ventilation capacity and electrical capacity; and • Detailed drawings of special installation techniques required for windows, doors, and other elements. The Consultant will provide the general requirements, wage rates, labor policies, and other legally required contractual documents in accordance with Monroe County protocols for inclusion in the Project Manual, and will develop construction documents that meet applicable requirements, including bid forms, special provisions, technical specifications, house plans, equipment schedules, and details. The Consultant will provide copies of all Monroe County— Homeowner Noise Insulation Contracts and Avigation Easements signed by the Homeowners and properly notarized. The Consultant will prepare a cost estimate for the construction of all participating residences once the bid package is completed for each phase. Up to 20 sets of the Project Manual (including all volumes) will be produced and distributed by the Consultant. The Project Manual (including all volumes) will also be provided electronically on CD. The Consultant will submit one copy of the Project Manual (including all volumes)to each of the following agencies for review and comments: the County Attorney and FAA. Any required revisions to the Project Manual (including all volumes)will be finalized prior to distribution to the Contractors. TASK 10 PREPARE ADVERTISEMENT AND REQUEST BIDS The Program Manager will prepare and advertisement, in compliance with Monroe County Purchasing Department requirements, briefly describing the Noise Insulation Program and calling for bids. The Monroe County Purchasing Department will advertise the Request for Bids in local newspapers and in Demandstar by Onvia. The Program Manager will transmit one copy of the Project Manual (including all volumes) to Monroe County Purchasing Department on a CD, as required. Page 8 of 13 TASK 11 CONDUCT PRE-BID CONFERENCE The Consultant will hold a mandatory Pre-Bid Conference prior to the Bid openings. This will provide all pre-qualified contractors the opportunity to ask questions regarding the project and discuss the site conditions. Any necessary revisions and/or addenda to the Bid Documents will be prepared, along with responses to questions, and will be distributed to all bidders in accordance with the procedures described in the Bid Documents. Bid forms will be simplified as much as possible without compromising the ability to evaluate the Bids received. TASK 12 EVALUATE BIDS AND RECOMMEND AWARD The Bids will be opened by the County in accordance with their established procedure. The County Attorney will review and certify the Bid process. The County will provide copies of all bids to the Consultant for review and recommendation for award. The Consultant will attend the bid opening. The Consultant will analyze the Bids for compliance with the Bid Documents and to determine the lowest responsive bidder. The results of the analysis will be tabulated and a recommendation for award of the contract will be submitted to the County and the FAA. Following FAA concurrence, the recommendation for award of the contract will be presented to the Monroe County BOCC. The Consultant will attend all BOCC meetings dealing with construction Bid award. The Consultant will work with the Airport Noise Program Coordinator to ensure the Contractors contract is complete and provided to the BOCC for signature. TASK 13 CONDUCT PRE-CONSTRUCTION CONFERENCE The success of the project depends on the Homeowners being satisfied with the noise control treatments. Quality workmanship is absolutely essential in this regard. The proper execution of noise insulation modifications ultimately rests with the building trades. The Consultant will conduct and document the Pre-Construction Conference to review the scope of work and the coordination plan. Administrative procedures and performance requirements will be reviewed with the contractors. The Consultant will acquaint contractor personnel with the detailed requirements of noise insulation design. The Pre-Construction Conference will cover the following material: • Purpose of the project; • Goals to be achieved; • Homeowner interaction and sensitivity training; • Simplified description of noise transmission into buildings; • Noise insulation treatments; • Submittal requirements; • Scheduling and phasing of trades; • Level of workmanship; • Coordination requirements; Page 9 of 13 • Progress payments; • Requisitions and change orders; and • Final closeout documentation requirements. TASK 14 CONDUCT CONTRACTOR WALK-THROUGH Many tasks occur during the Contractor walk through sessions. These include: • Product Measurements — The Contractor and manufacturer representatives, if present, will measure the window and door openings for ordering and manufacturing the designated products. (The contract documents note the nominal dimensions of these products for bid purposes only). Contractors are responsible for taking their own measurements for product fabrication. • Review of Installation Details— The Contractor must review the existing condition of the window and door openings. • Verification of Homeowner Pre-Work Completion (if applicable) —The Consultant will inspect items listed in the contract documents as Homeowner pre-work for satisfactory resolution. Where necessary, the Consultant will address these issues with the Homeowner. • Verification of Finishes, Colors, and Styles - The Consultant will verify with the Homeowner the product selection styles, colors, and finishes, the window grille configuration (if applicable), and the location of the air condensing unit(if applicable). The Airport Noise Program Coordinator will follow up with Homeowners as necessary to assist in collecting this information. • Construction Survey Form - The Consultant will complete the Construction Surrey with the Homeowner. The Construction Survey provides written information for the contractor as to any special needs required at the project site. Such needs include assisting the Homeowner in moving furniture, storage areas required for items moved, special hours where the house is not accessible to the contractor. The Consultant, with assistance from the Contractor, will schedule appointments with the Homeowners for the Contractor Walk-Through. TASK 15 PROVIDE CONSTRUCTION PHASE SERVICES Throughout the course of construction, the Consultant will provide construction observation services and conduct daily field inspections to help ensure quality control. If change orders are necessary, the Consultant will secure the approval of the airport and the FAA prior to approving any major changes to the contract. The tasks involved in construction administration include the following: • Review and process all submittals and shop drawings that are required by the Specifications. Page 10 of 13 • Review materials before they are installed to ensure that they comply with approved submittals. • Review work covered by shop drawings to ensure that it complies with Contract Documents. • Conduct daily inspections at each site that is under construction. • Coordinate with the Contractors to insure all appropriate building permits have been secured. • Observe work in progress to verify that it meets the requirements and intent of the Contract Documents. • Attend, conduct, and document weekly construction progress meetings with the contractors and subcontractors. • Notify the General Contractor of any part of the work in progress that does not conform to approved submittals, shop drawings, or Contract Documents. • Consider and evaluate the Contractor's suggestions for modifications to the Contract Documents. • Maintain files and records of all pertinent documents including submittals, shop drawings, addenda, minutes of meetings and reports. • Review applications for payment to ensure that they agree with the work actually done and materials actually received. • Prepare punch lists at the appropriate time for each unit, distribute the lists to the General Contractor for execution, verify that punch list items are complete, and then certify the General Contractor's final invoice for payment. The Consultant's Resident Project Representative (RPR) will be in the field daily to monitor Homeowner satisfaction throughout the construction process. The Consultant will conduct periodic field inspections to maintain quality control in the construction installations. The Airport Noise Program Coordinator will be available to Homeowners, Contractors, and the Consultant to assist in communication and coordination. TASK 16 PERFORM POST-MODIFICATION NOISE AUDIT Once construction is substantially complete in a sufficient number of homes, post-modification noise audit of selected homes will be conducted. The post-modification data will be compared with pre-modification measurements to document the achieved increase in noise reduction, and as a check on quality control. The Consultant will schedule appointments with the selected Homeowners. The Consultant will conduct one on-site visit to collect data on the selected homes. TASK 17 CONDUCT FINAL INSPECTIONS Once the outstanding punch list items are completed, the Consultant will conduct a final inspection. The Consultant will verify that all items identified on the punch list have been acceptably completed, that all work has been performed in accordance with the contract plans and specifications, and that the work meets project standards for workmanship and quality. At the final inspection, after the Consultant has verified that the work is complete, the Airport Noise Program Coordinator will verify Homeowner satisfaction and document that the Homeowner has received their warranty manuals. The manuals will list the length of workmanship and product warranties, and the names and telephone numbers of whom to call should warranty service be required. The Consultant's RPR will schedule and attend the final inspections with the Homeowners. TASK 18 EVALUATE AND CLOSE-OUT PROJECT The Consultant will prepare a full technical assessment of performance and achievements, including the following: • The measured noise reduction benefits achieved by the application of the Noise insulation plan to each of the houses. • The adequacy of technical specifications. • The response of Homeowners to pre- and post-modification attitudinal surveys on interference from aircraft noise. • Revisions required, if necessary, to the noise insulation plan and/or the technical specifications in order to achieve the Project objectives. • An evaluation of the predicted versus the actual costs for modifications. • Contract schedule compliance. • Full cost accounting including Bid cost and Change Order Request evaluation. • Necessary FAA closeout documentation forms. Page 12 of 13 TASK 19 PROVIDE WARRANTY SERVICE For a one-year period following each phase program acceptance, the Consultant will evaluate warranty issues. For the first year, all warranty requests made by the Homeowners and reported to the Airport Noise Program Coordinator will be documented and forwarded to the Contractor for resolution. Should the Consultant be required to make an on-site evaluation, the Airport Noise Program Coordinator, whenever possible, will coordinate with the Homeowner to schedule appointments that coincide with other project-related trips. After this one year period, Homeowners will be instructed to contact the manufacturer or manufacturer's representative directly for warranty issues. Page 13 of 13 Monroe County, owner and operator of Key West International Airport, is soliciting statements of qualifications until <INSERT TIME AND DATE> from Professional Firms who would like to be considered for program management, design, and construction administration work for the airport's Noise Insulation Program — Design and Construction, for all remaining phases. The scope of services includes, but is not limited to, program management; analysis of design alternatives; construction design; preparation and distribution of bid documents, plans and specifications; pre- and post-acoustical testing; construction administration; construction observation and inspection necessary to improve the noise insulation of individual residential dwellings in the vicinity of Key West International Airport. There are approximately 206 residential condominium units and 16 individual residences potentially eligible for participation in the Noise Insulation Program.
