ITEM BACKGROUND: As per the attached letter from Morganti, in the course of demolition work on the old terminal building, they discovered mold and asbestos. The mold was minor, and was removed for less than $5000.00. However, the asbestos removal price was $14,138.00. Due to the emergency nature of this removal we immediately authorized Lang Environmental to proceed with the work. Please see attached e-mail from John King stating that Lang Environmental is a reputable company that has done much work for the County in the past. Today's floor covering removal activities in the future holding concourse revealed underlying layers of vinyl tile which raise concerns that asbestos tile might be present. Because the building was constructed at a time when materials were used that have now been identified as potentially causing health hazards we are requesting that a survey be conducted immediately to determine that the tile is not asbestos and the current demolition activities have not caused suspension of fibers in the air. If a survey was performed showing the discovered tile is not of concern please forward those results to this office for review and project documentation. We also have concern about what appears to be mold in the temporary holding area return air vents as well as at several of the HVAC feeder grilles. We advise that an air quality review and inspection of the substance be preformed prior to any demolition actives begin in this area. Mr. Capraro discussed this situation with you in our last construction meeting and I believe you stated that the airport had a firm contracted to perform this type of inspection. We do not want construction activities to be delayed over these issues but it is of critical importance to the welfare of workers and the general public that these concerns be addressed as we move forward with construction of the existing terminal. (LE) is pleased to submit this proposal to perform professional asbestos abatement services for the above- referenced project. As we understand, Key West International Airport is seeking a proposal to perform complete asbestos abatement of the designated first floor office area as part of a planned renovation project. The scope of work for this project is based upon information provided and prepared by Mr. Richard GrupenhoffofEE&G, on April ]5,2009. In this proposal, we have included a brief description of our scope of work along with the associated cost required for completion of this project in accordance with our existing "Continuing Contract for Asbestos Abatement, Demolition & Removal Services" between Lang Environmenta], Inc. and The School Board of Hernando County, Florida", dated October 2],2009 (copy of contract included under separate cover). Scone of Work: ] . Submit a "Courtesy" EP A "Notice of Asbestos Removal Project" form and corresponding fee to the local Monroe County FDEP South District Branch Office. 2. Provide a copy of all applicable "Pre-Job" submittals including, but not limited to the following; copies of LE' s State of Florida Asbestos Abatement Contractor license, insurance certificates, EP A AHERA Asbestos Supervisor and Worker training certifications, and copies of OSHA-required medical surveillance/personnel clearances, respirator training, and fit testing documentation. 3. Post "Asbestos Warning" signs and barrier tape around the perimeter of each work area to notify all personnel of asbestos abatement. Supply and install critical barriers consisting of two (2) layers of 6-mil polyethylene sheeting sealed with duct tape to seal all penetrations and openings into the work area. 5. Supply and install splashguards consisting of one (1) layer of of 6-mil polyethylene sheeting sealed with duct tape up to 4' high on all affected walls (to serve as) or one (1) layer on all perimeter roof abatement areas (to serve as protections). 6. Supply and install a two-stage decontamination unit consisting of a clean room and equipment room, as appropriate, for the designated work area. 7. Supply and install sufficient HEPA-filtered negative air pressure machines to change out the air in the work area and vent the exhaust outside the building. 8. Remove and dispose of the approximately 4,000 square feet of existing Category 1 nonfriable asbestos-containing vinyl floor tile and adhesive mastic located within the approximately 2,300 square foot (former)n first floor offices area. Includes up to two (2) to three (3) separate layers of floor tile over an approximately 2,000 square foot affected area. 9. After removal is completed, the work areas will be HEP A vacuumed and/or wet wiped as appropriate and encapsulated to lock down any remaining fibers. 10. Perform personal exposure monitoring ofa minimum of2S% of the designated workforce for compliance with OSHA 29 CFR 1926.1101. 11. All designated bags of asbestos waste will be stored in lockable dumpsters on-site and then transported to a State of Florida licensed "Asbestos Landfill" facility. 12. Provide a copy of all applicable "Post-Job" submittals including, but not limited to the following; copies ofLE's Daily Work logs, Exposure Logs, OSHA personal air monitoring results, and a copy ofthe applicable asbestos waste manifest/waste shipment receipt forms (WSRs). 13. Coordinate and perform all asbestos abatement work in conjunction with the stipulated demolition schedule. Work Bv Others: 1. Provide restrooms, staging area, water and electric service during the entirety of all asbestos removal operations. 2. Supply an independent testing laboratory and State of Florida licensed Asbestos Consultant (EE&G) to perform a final visual observation/inspection and final clearance air sampling and analysis, with acceptable levels being less than or equal to 0.01 fibers/cc, as measured by Phase Contrast Microscopy. Lang Environmental has an active safety program with a dedicated safety manager who will monitor the project and conduct weekly safety meetings, as appropriate. 2. Lang Environmental has instituted a "Drug Free Work Place" Program, in accordance with State of Florida requirements. 3. Personnel air monitoring analysis will be performed by an independent testing laboratory at Lang Environmental's cost. 4. All personnel working on this project will have pre-employment physicals, and have the medical examinations available for your review, in accordance with OSHA requirements. 5. The supervisor for this project will have completed the one week "Asbestos Abatement Project Management and Supervision." This is in accordance with State of Florida legislation. The supervisor for the project will also have the OSHA 1 0 Hour Certification. 6. All personnel working on this project will have completed a training program covering the key aspects of asbestos and safe removal. 7. All personnel shall complete a respirator training program as part of their overall training. All personnel shall wear disposable protective coveralls for removal and disposal operations, and shall wear the appropriate respirator for removal. 8. All personnel will complete a certificate of worker's release form, outlining his understanding of the risks associated with asbestos removal. Insurance: Lang Environmental will provide a $1 million Occurrence policy for asbestos special liability insurance and also a $1 million policy for general liability, automobile and workers compensation. Additionally, we also have a $5 Million dollar excess liability policy. Licensine: Lang Environmental has met all of the requirements for licensing as an asbestos contractor, and Robert A. Lang holds the license for the company. We were one of the first companies licensed by the State of Florida (CJ C046268). We are also a licensed Class "A" General Contractor (CG CA23888) in Florida. Estimated Costs: Lang Environmental will supply all transportation, labor, materials, equipment, transportation, insurance, lodging, per diem, and insurance required to complete the above-mentioned scope of work for a total of $14, 138.00. EE&G's Statement of Services and Fee are presented below: STATEMENT OF SERVICES EE&G will provide environmental consulting and air monitoring services to the KWIA for the Floor 1 renovations in Key West, Florida. The areas measure approximately 2,500 square feet for containment area. See plans and previos surveys provided by KWIA. Task 1 - ScoDe of Work/Bid Assistance An EE&G Certified Abatement Designer will conduct a pre-solicitation meeting and will compile a Scope of Work and distribute meeting minutes to qualified abatement contractors. Other duties may include senior project management and pre-construction meetings/teleconferences. Up to 5 additional bulk samples will be analyzed for asbestos content to further delineate the scope of work. Mr. Peter J. Air samples will be collected before, during, and after removal of ACM. Air samples will be collected and analyzed using Phase Contrast Microscopy (PCM) during abatement and for Final clearances. EE&G's duties shall include the collection and analysis of PCM samples during each 8 hour shift. Other duties include in-office project management and periodic site visits by a senior project manager and/or licensed asbestos consultant. Task 3: Closeout Packaae An EE&G senior project manager will compose a "Letter of Completion" as necessary to facilitate completion of the project phase(s) and a final report based on the data collected during removal of the ACM. The final report will include: . A descriptive narrative of the abatement project. . Technician field notes & printed data sheets. . Photographic documentation. The report will be reviewed, signed, and sealed by a Florida licensed Asbestos Consultant. The fee includes all administrative overhead costs for one original and four copies of the report.
