The successful bidder is New York Life Insurance Company. There is no employer contribution to fund this benefit and will be paid solely by the individual who chooses to purchase the product. Aguiar, Employee Services Di ct SUBJ: Approval of Resolution — Supplemental Life Insurance March 19`h BOCC meeting This item requests approval of a resolution to offer county personnel the option to purchase voluntary supplemental life or dependent life insurance with New York Life Insurance Company. In August, 2007, the County requested and received proposals to provide supplemental life and dependent life insurance products. We received 10 proposals and decided to offer the bid to New York Life Insurance Company. Attached is a summary of the products and services provided. New York Life makes arrangements directly with the individual thereby eliminating the County's involvement in the administrative process. There is no employer contribution to fund this benefit. It is my recommendation that the BOCC approve this resolution. If you have any questions on this item, please do not hesitate to contact me at X4458. Ms Aguiar, as requested, the following is a brief summary of the products and services that is provided by New York Life Insurance Company. Life Insurance: Three Whole Life Insurance contracts Five Term Life Insurance contracts Three Traditional Universal Life contracts Two Variable Universal Life contracts Annuities: Eight types of Fixed Annuities Four types of Variable Annuities Mutual Funds: 27funds in our Fund Family with access to most any fund offered in the market. (A prospectus is required). Long Term Care, designed to fit each individuals needs. Services included but not limited to: The Needs Analysis to identify individual and family concerns, to determine the amount of insurance that a family really needs. Estate Planning by registered and certified financial planners. (no fees) College Funding, Estate Conservation, Mortgage Protection, Supplemental Retirement Income, Supplemental Life Insurance for Retirement Planning, Charitable Giving, Business Continuation, Deferring Estate Taxes for Small Business Owners, and Pension Maximization Alternative. We discuss issues such as the importance of a Will, uses of a Trust, the importance of life insurance for the Stay -at -Home Spouse; we provide information on what to do when someone dies. SEE PAGE 2 The cost of individual coverage is determined by many factors which includes but is not limited to: Age, sex, health condition, built, amount of coverage, hazardous activity, or medical history. As stated in our conversation, if life insurance is included in a 125 Plan the proceeds may be taxable to the beneficiary. The Maximum amount of coverage is determined by suitability, insurability, and affordability. The minimum coverage is determined by the type of contract: Whole Life $10,000 Universal Life $25,000 Term minimum ranges from $100,000 to $250,000. WHEREAS, Monroe County does not currently offer supplemental life or dependent life insurance for its personnel; and WHEREAS, some individuals, at their own expense, may desire to possess additional life insurance coverage for specific needs; and WHEREAS, Monroe County received proposals to provide supplemental life and dependent insurance products to personnel and has selected New York Life Insurance Company as the provider; and WHEREAS, there is no employer contribution to fund this benefit. New York Life makes arrangements with the individual to direct the withdrawal and transfer of funds from the individual's designated account, eliminating the County's involvement in the administrative process. BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF MONROE COUNTY, FLORIDA That New York Life Insurance Company shall be offered as the voluntary supplemental life and dependent insurance provider for county personnel and the county shall assist in coordinating enrollment meetings with the provider.
