Attached is a proposed change order for D.L. Porter Constructors, Inc. for
Renovation of the Old Mariners Hospital for approval.
According to the ordinance adopted by the Board of County Commissioners,
proposed change orders are to be presented to members of the Board of County
Commissioners prior to approval, assuming they are within the Administrator's
prescribed limits. Change orders not within the Administrator's authority are
placed on the BOCC agenda.

Site Plan modifications are required in order to accommodate a request from the Village of Islamorada _ Fire Chief to retain the helipad
south of High Point Road. Sheet Al is revised and reissued with a date of 0718/03. Provide pricing to provide the modification s shown
described in general as follows:
1. Revise all parking and landscaping shown on the revised site plan, Leave the helicopter landing pad, fencing and landscaping as it
currently exists,
2. Demolish the metal building directly south of the sewage treatment plant and the metal building directly east of the current
construction office building, removing foundations as required, but not less than 12" below finish grade.
3. The total nwnber of plants and other landscape materials as previously bid will remain the same, however, they will be redistributed
into revised landscape areas as shown on revised site plan.
4. Landscape sprinkler system will be required to provide 100% coverage of landscape areas, including area surrounding new parking
south of Highpoint Road.
