St., Opalocka, Florida 33054 ("Contractor"), and Monroe County Board of County Commissioners ("County" or`BOCC"). WHEREAS the parties entered into Agreement for tire disposal services on February 19, 2025 ("Agreement"); and WHEREAS, the Agreement incorrectly listed the Contractor corporate name as "FP Tire Disposal, Inc."instead of the correct name of"FP Junk Tire Inc"; and WHEREAS, the Contractor also wants to amend the Agreement to include a remittance address of P.O. Box 681096, Miami, Florida 33168; and WHEREAS, the Agreement did not specifically include removal and pricing for "oversized tires" meaning tires that will not fit on standard size passenger car, truck or SUV; and WHEREAS,the County wants to add this service and pricing in order to remove oversized tires at the County's transfer stations; NOW THEREFORE, in consideration of the mutual promises as stated herein, the County and Contractor agree to amend the Agreement as follows: 1. All references in the Agreement to "FP Tire Disposal, Inc.", shall be amended to and reference as, "FP Junk Tire Inc". 2. Paragraph 4.2.1 shall be amended to add the following: All Payments shall be remitted to FP Tire Disposal Inc, P.O. Box 681096,Miami,FL 33168. 3. Attachment A shall be revised to add the following under Scope of Work and Itemized Costs: The Contractor will remove, haul and properly disposed of"oversized tires" and rims from the County's Transfer Stations (Cudjoe, Long Key, and Key Largo). FP Junk Tires, Inc. is the only vendor authorized and approved to remove tires and rims from the transfer station, unless agreed upon by both parties in writing prior to removal.
