The project is currently in the construction phase and this amendment is for additional post design services task in the amount of$25,000.00. revising the total contract amount to $482,420.23. C7 3rd Change Order to the Contract with H&R Paving, Inc. for Key Largo III Roadway and Drainage Project to correct quantities for milling and resurfacing and establishing roadway crowns on roads being widened for bicycle lanes in the not to exceed amount of $249,778.00, for a revised total contract amount of$2,345,609.17 and adding 34 calendar days to the project for a revised substantial completion of August 26, 2025.. FOR KEY LARGO III ROADWAY AND DRAINAGE IMPROVEMENTS PROJECT In accordance with the Continuing Contract for On Call Professional Engineering Services made and entered on the 17th day of January 2018 between Monroe County hereinafter referred to as the "County" and WSP USA, INC. hereinafter referred to as "Consultant". All terms and conditions of the referenced Contract for On Call Professional Engineering Services apply to the Task Order, unless the Task Order modifies an Article of the Agreement of which will be specifically referenced in this Task Order and the modification shall be precisely described. WHEREAS Amendment 1 was executed by both parties on April 15, 2020, and added Standard Penetration Test (SPT) borings, coring and Dynamic Cone Penetration (DCP) tests to obtain California Bearing Ratio (CBR) Values for Burton Drive in the lump sum amount of$6,960.00; revising the total contract amount to $356,054.71; and WHEREAS Amendment 2 was executed by both parties on May 17, 2023; and added FDEP SLIP study, revised drainage and roadway design, and addressed FDOT SCOP grant requirements in the not to exceed amount of$26,365.52; revising the total contract amount to $382,420.23; and WHEREAS Amendment 3 was executed by both parties on March 20, 2024; and added scope to address the final round of FDOT comments not anticipated and complete design to 100% bid package in the amount of $25,000.00, revising the total contract amount to $407,420.23; and WHEREAS Amendment 4 was executed by both parties on October 16, 2024, and added design to provide construction plans to elevate a portion of Antigua Rd and added post design funds due to construction in progress in the amount of $50,000.00; revising the total contract amount to $457,420.23; and WHEREAS this Amendment 5 amends Article II Scope of Basic Services for additional design required to revise construction plans to address existing utility conflict and other contractor request for information (RFI) and amends Article VII Compensation, paragraph 7.1 adding not to exceed $25,000.00 to post design services for the construction currently in progress: revising the total contract amount to $482,420.23. The individual itemized task amounts are estimates and supported by hours of level of effort and On Call contract rates, (as attached). The individual task amounts are estimates and unused balances may be moved to other tasks as needed with prior approval by the County Engineer provided the Total Contract Amount of $482,420.23 is not exceeded. The original Task Order was effective on the 22nd Day of May 2019, amended on April 15, 2020, May 17, 2023, March 20, 2024, October 16, 2024, and here effective May 21, 2025.
