This Task Order is entered into on the effective date noted above pursuant to the "Master
Agreement for Professional Services" between Monroe County, Florida ("Client") and JACOBS
PROJECT MANAGEMENT CO. ("Consultant"), dated February 16, 2022 ("Agreement"). The
Agreement is incorporated herein and forms an integral part of this Task Order.

Scope of Work
Under Task Order No. 12/1 S-D9, Jacobs Project Management Co. (Jacobs) provided construction
support and resident project representative (RPR) services for the "Aircraft Overflow Apron"
project at Key West International Airport (EYW). The project includes the construction of the
aircraft overflow apron and the associated mitigation areas on airport property (ONS-1, ONS-2,
and ONS-3) and off airport property at Summerland Key(OFS-1)
The original design for the EYW Aircraft Overflow Apron and Mitigation Areas project was
advertised in September 2021. For the proposed work on Summerland Key, the original design
utilized an existing haul route to access the mitigation area. Due to the USACE and SFWMD
permitting delays, the construction did not start until April 2024. Between September 2021 and
April 2024, the existing haul route to access the mitigation area overgrew with mangroves. An
SFWMD permit modification was required due to the change in site conditions and presence of
mangroves.
Additionally, to meet permit requirements it is necessary to complete a Time Zero Monitoring
Report. The Time Zero Monitoring Report includes evaluating the site post construction and
developing a report on the site and plant conditions at the mitigation areas.
This proposal includes the survey and design required for the Summerland Key (OFS-1) Haul
Route.Additionally, it includes the drainage design permitting efforts required for the SFWMD,the
SFWMD permitting fee, and the Time Zero Monitoring Report.
Assumptions
Jacobs will invoice on a percent complete, including expenses, to compensate the level of effort
required for each assignment.
The scope of services and pricing is provided based upon and shall be performed under base
agreement"Master Agreement for Professional Services".The Master Agreement for
Professional Services is between Monroe County Florida and Jacobs Project Management Co.
with an execution date of December 14, 2021 (together the base agreement Master Agreement
for Professional Services and any amendments thereto are defined as the "Agreement").
Notwithstanding anything to the contrary contained or referenced in this Scope of Services, the
terms and conditions of the Agreement shall govern all staff augmentation under this Scope of
Services.

ARTICLE B— DESIGN, SPECIFICATIONS,AND MEETINGS
1. Develop the following sheets for the SFWMD permit plans:

2. Prepare for and attend virtual pre-application meeting with SFWMD.
3. Prepare for and attend virtual pre-application meeting with FDEP to discuss the
haul route on TIFF lands.
4. Submit permit package to SFWMD on-line portal.
5. Respond to Requests for Additional Information (RAIs) from SFWMD. Assume 2
RAIs.
6. The proposal includes the additional funds needed to complete a detailed as-built survey
of the mitigation sites boundaries for the conservation easements.
Services for Article B will be provided under a lump sum basis.
ARTICLE D —CONSTRUCTION PHASE SERVICES
1. Inspect mitigation sites ONS-1, ONS-2, ONS-3, and OFS-1 (Summerland Key) post
construction and evaluate the post construction site and plant conditions. Develop a
Time Zero Monitoring Report of the mitigation areas for the permitting agencies. This
task will be completed by Birkitt Environmental Services, Inc.
Services for Article D will be provided under a lump sum basis.

O'Neal Surveying and Mapping, Inc. (]8K8) is pleased to provide you with a scope nfservices and proposal for the above-
referenced project in Key West, Florida. This proposal is to supplement the original proposal(dated October 27, 2021, for
$25,000). Portions of the original proposal were allocated for 2 tasks that were not included:
° Sketch and Legal Descriptions for Mitigation Areas ONS-1. O/VS-2. ONS-3 and Summorland Key-$&66600
° OF3-1 Haul Route Survey-$f,00800
For a total of$14,665.00 used for scope of services outside the original contract. The total funds remaining un the original
contract is$10,335. This proposal covers the remaining fee needed to backfill the original contract.
SCOPE OF SERVICES—NEW CONTRACT
HORIZONTAL&VERTICAL CONTROL
O8K8 team will verify positions on published Primary and Secondary Airport Control points and on control points which
were previously established by O8K8 and others. O8K8 will establish new control as may be required for this survey. All
survey control will be referenced to North American Datum 1083 (NAO83) (2011 Adjustment) and North American
Vertical Datum 1088 (NAVO88).
° Locate and verify existing site control as previously established and/or establish new control as necessary for this
project.
TOPOGRAPHIC SURVEY
The O8K8 team will provide a TnpngnophicAs'Bui|t Survey for(5) sites including the Aircraft Overflow Apron as shown
nn the attached Exhibit Plan Sheet C'GO2and (4) sepanote Mitigation Sites asshown on attached Exhibit Plan Sheets
C'502thru C'505 (provided by the client).
The attached Exhibit Sheets depict dashed "Limits of Disturbance" lines which will be the limits of the areas to surveyed.
Any additions to these limits may require additional fees, to be determined.
Tasks for this proposal include:
° Pavement elevations measured nna25'Grid.
° Concrete pavement elevations measured at4 corners at each joint.
° Ground elevations measured nna5O'ghd.
° Provide edge nfpavement.
° Survey significant grade break lines within limits of survey. NOTE: This includes as-built location of the various
proposed grade breaks depicted within the Mitigation Site area on Plan Sheet C-505. If significant grade breaks
are discovered during the as-bult survey in Mitigation Sites on Plan sheets C-502 thru C-504 they will be located,
otherwise, as-built grade contours within Mitigation Sites on Plan sheets C-502 thru C-504 will be determined
from the 50'grid.
• Vegetation and tree lines within limits of survey. NOTE: This location is for overall vegetation outlines of existing
or planted areas and does not include individual plant or line location of various species such as those noted in
the Planting Schedule on Sheet C-502.
• Fence lines
• Provide locations of visible major structures such as manholes, taxiway edge lights, drainage structures,
taxiway signs/foundations, concrete duct markers, pavement markings, aircraft tie downs.
• Elevations for light fixtures to be measured at top of foundations
• Elevation of sign foundations shall be measured at four corners
• Elevations of structures shall be measured at the center
• Provide drainage structure invert elevations and bottom of structure elevations
PLEASE NOTE:
• This project is scheduled as daytime operations with pull backwhen required.Jacobs will provide badged Airport
escort or arrange for Airport Operations Escort for OSM survey crews.
• This As-Built survey is scheduled to follow construction completion on the (5) project sites. This As-built survey
for the (5) sites is to be scheduled together as one continuous survey operation/deployment.
• This proposal excludes deployment of survey crews within a week of any Federal holiday and during Lobster
season.
• This proposal is valid only for Notice To Proceed with within one year of acceptance.
Remaining Lump Survey Fee needed to cover the original contract $ 20,053.00
WORK ZONE SAFETY:
All work zone and Airport safety, rules and procedures as required by the Key West Airport will be observed and strictly
adhered to by OSM.All survey activity will be coordinated with Jacobs staff prior to and during commencement. Any and all
vehicles to be utilized in said survey activities will be authorized by and registered as may be required. Said vehicles to
have current decal, display company name/identification and maintain proper lighting requirements.
QUALITY CONTROL/QUALITY ASSURANCE AND FIELD REVIEW:
Verification of the field conditions as related to the collected survey data prior to any topographic survey data submittal.
All activities required to supervise and coordinate the survey project will be performed by the OSM project supervisor, a
Florida P.S.M. or their delegate.
OSM will implement their QA/QC plan to include review, response to comments and any resolution meetings if required,
preparation of submittals for reviews, etc.
Deliverables will include the ACAD Civil 3D digital files of the topographic survey, Signed and sealed copies of survey as
required.

Birkitt will conduct the Time Zero monitoring upon completion of the mitigation areas

Haul Road Restoration) as required by SFWMD permit conditions.
