PCO#123:Furnish and install 5/8"fire-treated plywood on walls and ceilings to allow for installation the MP-3 and MP-4 wall panel
PCO#124:furnish and install(15)fifteen MCP-1 ceiling tiles and 54 SF of grid system for MCP-1 tiles at the new Gate#8 entry vestibule

This change order includes the costs associated with placing two 1.5 MVA transformers at designated locations for the KWIA project(20-2039-01).
Originally,the transformers were to be installed by KWIA and Florida Keys Energy,and this was not included in the JV's contract. However,at the
Owner's request,the JV installed the transformers.The cost breakdown and qualifications are attached.This change order supersedes all other
proposals for the same scope,thereby voiding PCO#107.

Terms and Conditions '
Bella Holdings of Key West,LLC.('Bella')agrees to perform the lifting operation,requested by Customer,subject to the following:
1. Compliance with Laws and Standards:Customer shall comply with and conform to all applicable laws,regulations,ordinances,rules and orders of any governmental entity
including,but not limited to OSHA and MSHA laws and regulations and shall comply with all applicable ANSI Standards.Bella agrees to provide lifting services as further
described in this Service Agreement. Customer is responsible for providing overall job site safety. Customer is responsible for providing a competent and experienced site
supervisor and lift director to oversee job site and lifting operations as set forth in ASME/ANSI 30.6-3.1.3,at seq.(as amended 2007). Customer is responsible for providing
Bella accurate load weights and accepts all liability resulting from its failure to do so.Customer assumes responsibility,control of,and supervision for rigging,hooking and
unhooking loads.Customer agrees to provide competent and qualified signal persons to direct Bella's equipment operators,Bella's work does not include rigging,signaling,
hooking or unhooking the loads(except where and to the extent Bella's employees or designated subcontractors are used as agreed in writing by the parties hereto).
2.Title:Any equipment used to perform the lifting operation shall remain the property of Bella at all times.Customer shall keep the equipment free from any and all liens,
encumbrances and claims whatsoever,and shall not perform or permit any act that may encumber or impair Bella's title or rights in the equipment.Upon Bella's request,
Customer shall promptly execute and/or deliver to Bella all documentation(such as estoppel certificates or a landlord waiver),as Maxim deems necessary or appropriate for
the preservation,perfection or enforcement of Bella's interests in the Equipment and Bella's rights under this Service Agreement,and if Customer fails to do so,Bella may
execute such documents on Customer's behalf and in Customer's name.
3.Indemnification:TO THE FULLEST EXTENT ALLOWED BY APPLICABLE LAW,CUSTOMER SHALL DEFEND,INDEMNIFY,AND HOLD HARMLESS BELLA,ITS
OFFICERS,DIRECTORS, PARTNERS, MANAGERS, EMPLOYEES,AFFILIATES, REPRESENTATIVES AND AGENTS(THE"INDEMNITEES")FROM ANY AND
ALL ACTIONS,CAUSES OF ACTION,CLAIMS,SUITS,DEMANDS,INVESTIGATIONS,OBLIGATIONS,JUDGMENTS,LOSSES,COSTS,LIABILITIES,DAMAGES,
FINES,PENALTIES AND EXPENSES INCLUDING ATTORNEY FEES WHICH ARE INCURRED BY ACCRUED ASSERTED OR MADE AGAINST OR
RECOVERABLE FROM ANY OF THE INDEMNITEES ARISING FROM OR RELATING TO,DIRECTLY OR INDIRECTLY,THE CUSTOMER'S NEGLIGENCE,WILLFUL OR
INTENTIONAL ACT OR ANY OTHER ACT OR OMISSION,WHETHER OR NOT THE SAME ARISES FROM DAMAGE TO PROPERTY(REAL OR PERSONAL) INJURY
OR DEATH TO PERSONS INCLUDING BUT NOT LIMITED TO CUSTOMER'S EMPLOYEES AGENTS AND REPRESENTATIVES.CUSTOMER EXPRESSLY AGREES
TO WAIVE ANY WORKERS'COMPENSATION IMMUNITY IT MAY OTHERWISE HAVE,THE DUTY TO DEFEND,INDEMNIFY AND HOLD HARMLESS INDEMNITIEES
EXISTS WHETHER BASED IN WHOLE OR IN PART UPON THE ACTIVE PASSIVE OR CONCURRENT NEGLIGENCE OF INDEMNITEES IN JURISDICTIONS IN
WHICH THE INDEMNIFICATION PROVIDED FOR IN THIS SECTION IS BROADER THAN THAT ALLOWED BY APPLICABLE LAW THIS SECTION SHALL BE
INTERPRETED AS PROVIDING THE BROADEST INDEMNIFICATION PERMITTED AND SHALL BE LIMITED ONLY TO THE EXTENT NECESSARY TO COMPLY
WITH SAID LAW.CUSTOMER'S OBLIGATION TO INDEMNIFY AND AND HOLD HARMLESS AND THE DUTY TO DEFEND THE INDEMNITEES SHALL SURVIVE
THE TERMINATION OF THIS AGREEMENT.IT IS EXPESSLYAGREED THAT THIS INDEMNIFICATION CLAUSE APPLIES TO BOTH THIRD PARTY CLAIMS AND TO
ALL CLAIMS BETWEEN BELLA AND CUSTOMER.
4. Payment Adjustments:Payments under this Service Agreement shall be subject to adjustment for excess hours of operator and/or crew time and/or excess hours of usage
of the equipment in accordance with the rates agreed to at the lime of the order,custom in the area where the lifting operation occurs,or by any applicable collective bargaining
agreement,as the case may be.The Customer shall not be entitled to any abatement,deduction,reduction,set-off,counterclaim,reooupment or defense against payment for
any reason.BELLA SHALL NOT BE LIABLE FOR ANY INCIDENTAL OR CONSEQUENTIAL DAMAGES RELATING TO THIS SERVICE AGREEMENT.
5.Payment:All payments required hereunder shall be due in full upon billing. In addition to the rates shown on the face of this Service Agreement,Customer shall pay the
following arising in connection with this Service Agreement:
(a)All demurrage,storage,switching,and drayage.
(b)All taxes,levies,assessment,fees and other public charges assessed as a result of this Service Agreement,including tangible personal property taxes if applicable.
(c)All judgments,awards,fines,penalties,forfeitures,court costs,expenses,attorneys'fees(unless prohibited by law)incurred by Bella in accordance with Item 3 above.
(d) Bella's costs and expenses including reasonable attorney's fees(unless prohibited by law)incurred in enforcing this Service Agreement and/or collecting any amounts due
hereunder.
(e) In the event of an accident resulting in damage to the equipment arising from the acts,omissions,misfeasance or malfeasance of Customer,including,but not limited to
Customer's employees,agents,representatives,subcontractors or independent contractors,Customer shall be responsible for costs of repairs and related expenses,all
consequential losses including,but not limited to loss of use of equipment,loss of profits and all reasonable attomey fees incurred in recovering such damages.
Any payments more than 30 days past due hereunder shall bear interest at the rate of 1.5%per month(or the maximum amount allowed by law whichever is higher).
6.Default:If Customer fails to pay when due any payment or other amount required herein to be paid by Customer,or if Customer makes an assignment for the benefit of
creditors,whether voluntary or involuntary,or if a petition is filed by or against Customer under the bankruptcy laws of the United States,in addition to other remedies Bella
may exercise to protect its interest,CUSTOMER AGREES THAT BELLA MAY TAKE POSSESSION OF ANY OR ALL ITEMS OF EQUIPMENT WHEREVER THESE ITEMS
MAY BE LOCATED WITHOUT DEMAND OR NOTICE WITHOUT ANY COURT ORDER OR OTHER PROCESS OF LAW AND WITHOUT LIABILITY TO CUSTOMER FOR
ANY DAMAGES OCCASIONED BY SUCH TAKINGS OR POSSESSION.
7.Notice:Customer shall immediately notify Bella of any event or occurrence involving personal injury and/or property damage of any kind relating to Bella personnel or
equipment.Customer shall immediately deliver to Bella any summons,pleading,notice or paper of any kind involving any claim,suit or proceeding relating to any event or
occurrence involving Bella personnel or equipment.Customer shall not aid or abet the assertion of any such claim,suit or proceeding,and shall fully cooperate with Bella in
investigating and defending the same.
8.Access&Site Conditions:Customer shall provide Bella with proper ingress to and egress from the job site and provide protection of all paving, curbs, real estate,
structures and/or improvements and Customer shall be responsible for all damage to any paving,curbs,real estate,structures and/or improvements caused by Customer's
failure to provide proper ingress and egress.Customer is responsible for providing an operating area that is suitable for operation of the crane with respect to levelness,surface
conditions,support capability,proximity to pawerlines,excavations,slopes,underground utilities,subsurface conditions,and obstructions to crane operations.Bella shall have
immediate access to and the light to retrieve and repair any equipment used in the lifting operation and shall be entitled to enter upon Customer's property at any time for those
purposes even in the event of a strike affecting Customer's facility or property.Customer shall perform or have performed all necessary inspections or testing to determine the
nature of the ground or soil and its ability to support the crane while in operation or otherwise.If the ground or soil condition is such that it cannot support the crane,Customer
shall take all necessary measures to insure that these conditions are remedied prior to the crane being placed on that ground or soil.These measures include,but are not limited
to,the provision of proper shoring or cribbing or other measures.

10. Assignment or Transfer:This is a personal services agreement that neither Bella nor Customer may assign or transfer to a third party without the combined prior
written consent of both parties hereto(which consent may be withheld for any reason).Any such consent shall not release Customer from any obligations under this Service
Agreement.
11. Lifting,Lugs,Rigging and Apparatus:Customer assumes all responsibility and liability to ensure the adequacy of the design and strength of any lifting lug or device embedded
in or attached to any object,and of any and all rigging or lifting apparatus failures or defects(even if such rigging or lifting apparatus is supplied by Bella tD Customer).
12. MSDS Requirements:Customer shall provide Bella,upon delivery of the equipment to the job location,copies of material safety data sheets(MSDS)for all hazardous chemicals in
use at said job location.or make such MSDS available at a central location at the job site,in order that Bella may comply with the requirements of all applicable local,state and Federal laws
and regulations,including but not limited to OSHA regulations,regarding hazardous materials.Furthermore,Customer shall inform Bella of all precautionary measures that need to be taken
to protect the operators and/or crew or other personnel during normal operating conditions and in foreseeable emergencies.
13. Full Agreement;Governing Law-,Waiver,Severability:This Service Agreement constitutes the full agreement between Bella and Customer.Any changes to this Service
Agreement must be in writing signed by Bella and Customer.No conduct by either party to this Service Agreement shall be deemed a modification of this Service Agreement.This Service
Agreement shall be binding upon and inure to the benefit of the parties hereto and their respective successors and permissible assigns.The laws of the state of the job site location shall
govern this Service Agreement.Headings are provided for convenience only,and not for Interpretation of this Service Agreement.Bella and Customer are independent contractors of each
other and Customer shall not be deemed to be the agent,servant or employee of Bella for any reason or purpose.No failure of Bella to enforce performance of any terms or covenants,or
failure to exercise or delay in exercising any right under this Service Agreement shall operate as a waiver thereof,nor shall any single or partial exercise of any right hereunder preclude any
other or further exercise thereof or the exercise of any other right If any provision of this Service Agreement is held to be invalid or illegal by a court of competent jurisdiction,the Invalid or
illegal term will be deemed excluded from this Service Agreement and will not invalidate the remaining terns of this Service Agreement.

Bella Holdings of Key West,LLC.("Bejla)agrees to perform the lifting operation,requested by Customer,subject to the following:
1. Compliance with Laws and Standards:Customer shall comply with and conform to all applicable laws,regulations,ordinances,rules and orders of any governmental entity
including,but not limited to OSHA and MSHA laws and regulations and shall comply with all applicable ANSI Standards. Bella agrees to provide lifting services as further
described in this Service Agreement.Customer is responsible for providing overall job site safety. Customer is responsible for providing a competent and experienced site
supervisor and lift director to oversee job site and lifting operations as set forth in ASMEIANSI 30.5-3.1.3,at.seq.(as amended 2007). Customer is responsible for providing
Bella accurate load weights and accepts all liability resulting from its failure to do so.Customer assumes responsibility,control of,and supervision for rigging,hooking and
unhooking loads.Customer agrees to provide competent and qualified signal persons to direct Bella's equipment operators:Bella's work does not include rigging,signaling,
hooking or unhooking the loads(except where and to the extent Bella's employees or designated subcontractors are used as agreed in.writing by the parties hereto).
2.Title:Any equipment used to perform the lifting operation shall remain the property of Bella at all times.Customer shall keep the equipment free from any and all liens,
encumbrances and claims whatsoever,and shall not perform or permit any act that may encumber or impair Bella's title or rights in the equipment.Upon Bella's request,
Customer shall promptly execute and/or deliver to Bella all documentation(such as estoppel certificates or a landlord waiver),as Maxim deems necessary or appropriate for
the preservation,perfection or enforcement of Bella's interests in the Equipment and Bella's rights under this Service Agreement,and if Customer fails to do so,Bella may
execute such documents on Customer's behalf and in Customer's name.
3.Indemnification:TO THE FULLEST EXTENT ALLOWED BY APPLICABLE LAW,CUSTOMER SHALL DEFEND,INDEMNIFY,AND HOLD HARMLESS BELLA ITS
OFFICERS, DIRECTORS, PARTNERS, MANAGERS, EMPLOYEES,AFFILIATES, REPRESENTATIVES AND AGENTS (THE"INDEMNITEES") FROM ANY AND
ALL ACTIONS,CAUSES OF ACTION,CLAIMS,SUITS, DEMANDS,INVESTIGATIONS,OBLIGATIONS JUDGMENTS LOSSES COSTS LIABILITIES DAMAGES
FINES,PENALTIES AND EXPENSES,INCLUDING ATTORNEY FEES,WHICH ARE INCURRED BY,ACCRUED ASSERTED OR MADE AGAINST OR
RECOVERABLE FROM ANY OF THE INDEMNITEES ARISING FROM OR RELATING TO,DIRECTLY OR INDIRECTLY,THE CUSTOMER'S NEGLIGENCE WILLFUL OR
INTENTIONAL ACT OR ANY OTHER ACT OR OMISSION,WHETHER OR NOT THE SAME ARISES FROM DAMAGE TO PROPERTY(REAL OR PERSONAL) INJURY
OR DEATH TO PERSONS INCLUDING BUT NOT LIMITED TO CUSTOMER'S EMPLOYEES,AGENTS AND REPRESENTATIVES.CUSTOMER EXPRESSLY AGREES
TO WAIVE ANY WORKERS'COMPENSATION IMMUNITY IT MAY OTHERWISE HAVE.THE DUTY TO DEFEND,INDEMNIFY AND HOLD HARMLESS INDEMNITIEES
EXISTS WHETHER BASED IN WHOLE OR IN PART UPON THE ACTIVE, PASSIVE,OR CONCURRENT NEGLIGENCE OF INDEMNITEES.IN JURISDICTIONS IN
WHICH THE INDEMNIFICATION PROVIDED FOR IN THIS SECTION IS BROADER THAN THAT ALLOWED BY APPLICABLE LAW,THIS SECTION SHALL BE
INTERPRETED AS PROVIDING THE BROADEST INDEMNIFICATION PERMITTED AND SHALL BE LIMITED ONLY TO THE EXTENT NECESSARY TO COMPLY
WITH SAID LAW.CUSTOMER'S OBLIGATION TO INDEMNIFY AND AND HOLD HARMLESS AND THE DUTY TO DEFEND THE INDEMNITEES SHALL SURVIVE
THE TERMINATION OF THIS AGREEMENT.IT IS EXPESSLYAGREED THAT THIS INDEMNIFICATION CLAUSE APPLIES TO BOTH THIRD PARTY CLAIMS AND TO
ALL CLAIMS BETWEEN BELLA AND CUSTOMER.
4. Payment Adjustments:Payments under this Service Agreement shall be subject to adjustment for excess hours of operator and/or crew time and/or excess hours of usage
of the equipment In accordance with the rates agreed to at the lime of the order,custom in the area where the lifting operation occurs,or by any applicable collective bargaining
agreement,as the case may be.The Customer shall not be entitled to any abatement,deduction,reduction,set-off,counterclaim,recoupment or defense against payment for
any reason.BELLA SHALL NOT BE LIABLE FOR ANY INCIDENTAL OR CONSEQUENTIAL DAMAGES RELATING TO THIS SERVICE AGREEMENT.
5.Payment:All payments required hereunder shall be due in full upon billing. In addition to the rates shown on the face of this Service Agreement,Customer shall pay the
following arising In connection with this Service Agreement:
(a)All demurrage,storage,switching,and drayage.
(b)All taxes,levies,assessment,fees and other public charges assessed as a result of this Service Agreement,including tangible personal property taxes if applicable.
(c)All judgments,awards,fines,penalties,forfeitures,court costs,expenses,attomeys'fees(unless prohibited by law)incurred by Bella in accordance with Item 3 above.
(d) Bella's costs and expenses including reasonable attorney's fees(unless prohibited by law)incurred in enforcing this Service Agreement and/or collecting any amounts due
hereunder.
(e) In the event of an accident resulting in damage to the equipment arising from the acts,omissions,misfeasance or malfeasance of Customer,including,but not limited to
Customer's employees,agents,representatives,subcontractors or independent contractors,Customer shall be responsible for costs of repairs and related expenses,all
consequential losses Including,but not limited to loss of use of equipment,loss of profits and all reasonable attorney fees incurred in recovering such damages.
Any payments more than 30 days past due hereunder shall bear interest at the rate of 1.5%per month(or the maximum amount allowed by law whichever is higher).
S.Default:If Customer fails to pay when due any payment or other amount required herein to be paid by Customer,or if Customer makes an assignment for the benefit of
creditors,whether voluntary or involuntary,or if a petition is filed by or against Customer under the bankruptcy laws of the United States,in addition to other remedies Bella
may exercise to protect Its interest,CUSTOMER AGREES THAT BELLA MAY TAKE POSSESSION OF ANY OR ALL ITEMS OF EQUIPMENT,WHEREVER THESE ITEMS
MAY BE LOCATED,WITHOUT DEMAND OR NOTICE,WITHOUT ANY COURT ORDER OR OTHER PROCESS OF LAW AND WITHOUT LIABILITY TO CUSTOMER FOR
ANY DAMAGES OCCASIONED BY SUCH TAKINGS OR POSSESSION.
7.Notice:Customer shall immediately notify Bella of any event or occurrence involving personal Injury and/or property damage of any kind relating to Bella personnel or
equipment.Customer shall immediately deliver to Bella any summons,pleading,notice or paper of any kind involving any claim,suit or proceeding relating to any event or
occurrence involving Bella personnel or equipment Customer shall not aid or abet the assertion of any such claim,suit or proceeding,and shall fully cooperate with Bella in
investigating and defending the same.
8.Access&Site Conditions: Customer shall provide Bella with proper ingress to and egress from the job site and provide protection of all paving,curbs,real estate,
structures and/or improvements and Customer shall be responsible for all damage to any paving,curbs,real estate,structures and/or improvements caused by Customer's
failure to provide proper ingress and egress.Customer is responsible for providing an operating area that is suitable for operation of the crane with respect to levelness,surface
conditions,support capability,proximity to powerlines,excavations,slopes,underground utilities,subsurface conditions,and obstructions to crane operations.Bella shall have
immediate access to and the right to retrieve and repair any equipment used in the lifting operation and shall be entitled to enter upon Customer's property at any time for those
purposes even in the event of a strike affecting Customer's facility or property.Customer shall perform or have performed all necessary inspections or testing to determine the
nature of the ground or soil and its ability to support the crane while in operation or otherwise.If the ground or soil condition is such that It cannot support the crane,Customer
shall take all necessary measures to insure that these conditions are remedied prior to the crane being placed on that ground or soil.These measures include,but are not limited
to,the provision of proper shoring or cribbing or other measures.

(c)All judgments,awards,fines,penalties,forfeitures,court costs,expenses,attomeys'fees(unless prohibited by law)incurred by Bella in accordance with Item 3 above.
(d) Bella's costs and expenses including reasonable attorney's fees(unless prohibited by law)incurred in enforcing this Service Agreement and/or collecting any amounts due
hereunder.
(e) In the event of an accident resulting in damage to the equipment arising from the acts,omissions,misfeasance or malfeasance of Customer,including,but not limited to
Customer's employees,agents,representatives,subcontractors or independent contractors,Customer shall be responsible for costs of repairs and related expenses,all
consequential losses including,but not limited to loss of use of equipment,loss of profits and all reasonable attorney fees Incurred In recovering such damages.
Any payments more than 30 days past due hereunder shall bear interest at the rate of 1.5%per month(or the maximum amount allowed by law whichever is higher).
6.Default:If Customer fails to pay when due any payment or other amount required herein to be paid by Customer,or if Customer makes an assignment for the benefit of
creditors,whether voluntary or Involuntary,or If a petition is filed by or against Customer under the bankruptcy laws of the United States,in addition to other remedies Bella
may exercise to protect its interest,CUSTOMER AGREES THAT BELLA MAY TAKE POSSESSION OF ANY OR ALL ITEMS OF EQUIPMENT,WHEREVER THESE ITEMS
MAY BE LOCATED,WITHOUT DEMAND OR NOTICE,WITHOUT ANY COURT ORDER OR OTHER PROCESS OF LAW AND WITHOUT LIABILITY TO CUSTOMER FOR
ANY DAMAGES OCCASIONED BY SUCH TAKINGS OR POSSESSION.
7.Notice:Customer shall immediately notify Bella of any event or occurrence involving personal Injury and/or property damage of any kind relating to Bella personnel or
equipment.Customer shall immediately deliver to Bella any summons,pleading,notice or paper of any kind Involving any claim,suit or proceeding relating to any event or
occurrence involving Bella personnel or equipment.Customer shall not aid or abet the assertion of any such claim,suit or proceeding,and shall fully cooperate with Bella In
investigating and defending the same.
8.Access&Site Conditions: Customer shall provide Bella with proper ingress to and egress from the job site and provide protection of all paving,curbs, real estate,
structures and/or improvements and Customer shall be responsible for all damage to any paving,curbs,real estate,structures and/or improvements caused by Customer's
failure to provide proper ingress and egress.Customer is responsible for providing an operating area that is suitable for operation of the crane with respect to levelness,surface
conditions,support capability,proximity to powerlines,excavations,slopes,underground utilities,subsurface conditions,and obstructions to crane operations.Bella shall have
immediate access to and the right to retrieve and repair any equipment used in the lifting operation and shall be entitled to enter upon Customer's property at any time for those
purposes even in the event of a strike affecting Customer's facility or property.Customer shall perform or have performed all necessary inspections or testing to determine the
nature of the ground or soil and its ability to support the crane while in operation or otherwise. If the ground or soil condition is such that it cannot support the crane,Customer
shall take all necessary measures to insure that these conditions are remedied prior to the crane being placed on that ground or soil.These measures include,but are not limited
to,the provision of proper shoring or cribbing or other measures.
9. Jury Waiver:UNLESS PROHIBITED BYLAW,BELLA AND CUSTOMER EACH KNOWINGLY,UNCONDITIONALLY AND IRREVOCABLY WAIVE TRIAL BY JURY WITI
RESPECT TO ANY ACTION, CLAIM,SUIT OR PROCEEDING,WHETHER SOUNDING IN CONTRACT TORT OR OTHERWISE IN RESPECT OF ARISING OUT OF
RELATED TO,OR INCIDENTAL TO THE RELATIONSHIP ESTABLISHED BETWEEN THEM(OR AS TO ANY THIRD PARTIES)IN CONNECTION WITH THIS AGREE.
MENT,ANY OTHER DOCUMENT OR AGREEMENT EXECUTED OR DELIVERED IN CONNECTION HEREWITH OR THE PARTIES'CONDUCT
10. Assignment or Transfer.This is a personal services agreement that neither Bella nor Customer may assign or transfer to a third party without the combined prior
written consent of both parties hereto(which consent may be withheld for any reason).Any such consent shall not release Customer from any obligations under this Service
Agreement. .
11. Lifting,Lugs,Rigging and Apparatus:Customer assumes all responsibility and liability to ensure the adequacy of the design and strength of any lifting lug or device embedde
in or attached to any object,and of any and all rigging or lifting apparatus failures or defects(even if such rigging or lifting apparatus is supplied by Bella to Customer).
12. MSDS Requirements:Customer shall provide Bella,upon delivery of the equipment to the job location,copies of material safety data sheets(MSDS)for all hazardous chemicals ti
use at said job location.or make such MSDS available at a central location at the job site,In order that Bella may comply with the requirements of all applicable local,state and Federal laws
and regulations,Including but not limited to OSHA regulations,regarding hazardous materials.Furthermore,Customer shall Inform Bella of all precautionary measures that need to be take
to protect the operators and/or crew or other personnel during normal operating conditions and In foreseeable emergencies.
13. Full Agreement;Governing Law;Waiver, Severability:This Service Agreement constitutes the full agreement between Bella and Customer.Any changes to this Service
Agreement must be in writing signed by Bella and Customer.No conduct by either party to this Service Agreement shall be deemed a modification of this Service Agreement This Servic
Agreement shall be binding upon and inure to the benefit of the parties hereto and their respective successors and permissible assigns.The laws of the state of the job site location shall
govern this Service Agreement.Headings are provided for convenience only,and not for interpretation of this Service Agreement.Bella and Customer are independent contractors of each
other and Customer shall not be deemed to be the agent,servant or employee of Bella for any reason or purpose.No failure of Bella to enforce performance of any terms or covenants,or
failure to exercise or delay in exercising any right under this Service Agreement shall operate as a waiver thereof;nor shall any single or partial exercise of any right hereunder preclude any
other or further exercise thereof or the exercise of any other right.If any provision of this Service Agreement is held to be invalid or illegal by a court of competent jurisdiction,the invalid or
illegal term will be deemed excluded from this Service Agreement and will not invalidate the remaining terms of this Service Agreement

Terms and Conditions '
Bella Holdings of Key West,LLC.('Bella')agrees to perform the lifting operation,requested by Customer,subject to the following:
1. Compliance with Laws and Standards:Customer shall comply with and conform to all applicable laws,regulations,ordinances,rules and orders of any governmental entity
including,but not limited to OSHA and MSHA laws and regulations and shall comply with all applicable ANSI Standards.Bella agrees to provide lifting services as further
described in this Service Agreement. Customer is responsible for providing overall job site safety. Customer is responsible for providing a competent and experienced site
supervisor and lift director to oversee job site and lifting operations as set forth in ASME/ANSI 30.6-3.1.3,at seq.(as amended 2007). Customer is responsible for providing
Bella accurate load weights and accepts all liability resulting from its failure to do so.Customer assumes responsibility,control of,and supervision for rigging,hooking and
unhooking loads.Customer agrees to provide competent and qualified signal persons to direct Bella's equipment operators,Bella's work does not include rigging,signaling,
hooking or unhooking the loads(except where and to the extent Bella's employees or designated subcontractors are used as agreed in writing by the parties hereto).
2.Title:Any equipment used to perform the lifting operation shall remain the property of Bella at all times.Customer shall keep the equipment free from any and all liens,
encumbrances and claims whatsoever,and shall not perform or permit any act that may encumber or impair Bella's title or rights in the equipment.Upon Bella's request,
Customer shall promptly execute and/or deliver to Bella all documentation(such as estoppel certificates or a landlord waiver),as Maxim deems necessary or appropriate for
the preservation,perfection or enforcement of Bella's interests in the Equipment and Bella's rights under this Service Agreement,and if Customer fails to do so,Bella may
execute such documents on Customer's behalf and in Customer's name.
3.Indemnification:TO THE FULLEST EXTENT ALLOWED BY APPLICABLE LAW,CUSTOMER SHALL DEFEND,INDEMNIFY,AND HOLD HARMLESS BELLA,ITS
OFFICERS,DIRECTORS, PARTNERS, MANAGERS, EMPLOYEES,AFFILIATES, REPRESENTATIVES AND AGENTS(THE"INDEMNITEES")FROM ANY AND
ALL ACTIONS,CAUSES OF ACTION,CLAIMS,SUITS,DEMANDS,INVESTIGATIONS,OBLIGATIONS,JUDGMENTS,LOSSES,COSTS,LIABILITIES,DAMAGES,
FINES,PENALTIES AND EXPENSES INCLUDING ATTORNEY FEES WHICH ARE INCURRED BY ACCRUED ASSERTED OR MADE AGAINST OR
RECOVERABLE FROM ANY OF THE INDEMNITEES ARISING FROM OR RELATING TO,DIRECTLY OR INDIRECTLY,THE CUSTOMER'S NEGLIGENCE,WILLFUL OR
INTENTIONAL ACT OR ANY OTHER ACT OR OMISSION,WHETHER OR NOT THE SAME ARISES FROM DAMAGE TO PROPERTY(REAL OR PERSONAL) INJURY
OR DEATH TO PERSONS INCLUDING BUT NOT LIMITED TO CUSTOMER'S EMPLOYEES AGENTS AND REPRESENTATIVES.CUSTOMER EXPRESSLY AGREES
TO WAIVE ANY WORKERS'COMPENSATION IMMUNITY IT MAY OTHERWISE HAVE,THE DUTY TO DEFEND,INDEMNIFY AND HOLD HARMLESS INDEMNITIEES
EXISTS WHETHER BASED IN WHOLE OR IN PART UPON THE ACTIVE PASSIVE OR CONCURRENT NEGLIGENCE OF INDEMNITEES IN JURISDICTIONS IN
WHICH THE INDEMNIFICATION PROVIDED FOR IN THIS SECTION IS BROADER THAN THAT ALLOWED BY APPLICABLE LAW THIS SECTION SHALL BE
INTERPRETED AS PROVIDING THE BROADEST INDEMNIFICATION PERMITTED AND SHALL BE LIMITED ONLY TO THE EXTENT NECESSARY TO COMPLY
WITH SAID LAW.CUSTOMER'S OBLIGATION TO INDEMNIFY AND AND HOLD HARMLESS AND THE DUTY TO DEFEND THE INDEMNITEES SHALL SURVIVE
THE TERMINATION OF THIS AGREEMENT.IT IS EXPESSLYAGREED THAT THIS INDEMNIFICATION CLAUSE APPLIES TO BOTH THIRD PARTY CLAIMS AND TO
ALL CLAIMS BETWEEN BELLA AND CUSTOMER.
4. Payment Adjustments:Payments under this Service Agreement shall be subject to adjustment for excess hours of operator and/or crew time and/or excess hours of usage
of the equipment in accordance with the rates agreed to at the lime of the order,custom in the area where the lifting operation occurs,or by any applicable collective bargaining
agreement,as the case may be.The Customer shall not be entitled to any abatement,deduction,reduction,set-off,counterclaim,reooupment or defense against payment for
any reason.BELLA SHALL NOT BE LIABLE FOR ANY INCIDENTAL OR CONSEQUENTIAL DAMAGES RELATING TO THIS SERVICE AGREEMENT.
5.Payment:All payments required hereunder shall be due in full upon billing. In addition to the rates shown on the face of this Service Agreement,Customer shall pay the
following arising in connection with this Service Agreement:
(a)All demurrage,storage,switching,and drayage.
(b)All taxes,levies,assessment,fees and other public charges assessed as a result of this Service Agreement,including tangible personal property taxes if applicable.
(c)All judgments,awards,fines,penalties,forfeitures,court costs,expenses,attorneys'fees(unless prohibited by law)incurred by Bella in accordance with Item 3 above.
(d) Bella's costs and expenses including reasonable attorney's fees(unless prohibited by law)incurred in enforcing this Service Agreement and/or collecting any amounts due
hereunder.

Bella Holdings of Key West,LLC.("Bejla)agrees to perform the lifting operation,requested by Customer,subject to the following:
1. Compliance with Laws and Standards:Customer shall comply with and conform to all applicable laws,regulations,ordinances,rules and orders of any governmental entity
including,but not limited to OSHA and MSHA laws and regulations and shall comply with all applicable ANSI Standards. Bella agrees to provide lifting services as further
described in this Service Agreement.Customer is responsible for providing overall job site safety. Customer is responsible for providing a competent and experienced site
supervisor and lift director to oversee job site and lifting operations as set forth in ASMEIANSI 30.5-3.1.3,at.seq.(as amended 2007). Customer is responsible for providing
Bella accurate load weights and accepts all liability resulting from its failure to do so.Customer assumes responsibility,control of,and supervision for rigging,hooking and
unhooking loads.Customer agrees to provide competent and qualified signal persons to direct Bella's equipment operators:Bella's work does not include rigging,signaling,
hooking or unhooking the loads(except where and to the extent Bella's employees or designated subcontractors are used as agreed in.writing by the parties hereto).
2.Title:Any equipment used to perform the lifting operation shall remain the property of Bella at all times.Customer shall keep the equipment free from any and all liens,
encumbrances and claims whatsoever,and shall not perform or permit any act that may encumber or impair Bella's title or rights in the equipment.Upon Bella's request,
Customer shall promptly execute and/or deliver to Bella all documentation(such as estoppel certificates or a landlord waiver),as Maxim deems necessary or appropriate for
the preservation,perfection or enforcement of Bella's interests in the Equipment and Bella's rights under this Service Agreement,and if Customer fails to do so,Bella may
execute such documents on Customer's behalf and in Customer's name.
3.Indemnification:TO THE FULLEST EXTENT ALLOWED BY APPLICABLE LAW,CUSTOMER SHALL DEFEND,INDEMNIFY,AND HOLD HARMLESS BELLA ITS
OFFICERS, DIRECTORS, PARTNERS, MANAGERS, EMPLOYEES,AFFILIATES, REPRESENTATIVES AND AGENTS (THE"INDEMNITEES") FROM ANY AND
ALL ACTIONS,CAUSES OF ACTION,CLAIMS,SUITS, DEMANDS,INVESTIGATIONS,OBLIGATIONS JUDGMENTS LOSSES COSTS LIABILITIES DAMAGES
FINES,PENALTIES AND EXPENSES,INCLUDING ATTORNEY FEES,WHICH ARE INCURRED BY,ACCRUED ASSERTED OR MADE AGAINST OR
RECOVERABLE FROM ANY OF THE INDEMNITEES ARISING FROM OR RELATING TO,DIRECTLY OR INDIRECTLY,THE CUSTOMER'S NEGLIGENCE WILLFUL OR
INTENTIONAL ACT OR ANY OTHER ACT OR OMISSION,WHETHER OR NOT THE SAME ARISES FROM DAMAGE TO PROPERTY(REAL OR PERSONAL) INJURY
OR DEATH TO PERSONS INCLUDING BUT NOT LIMITED TO CUSTOMER'S EMPLOYEES,AGENTS AND REPRESENTATIVES.CUSTOMER EXPRESSLY AGREES
TO WAIVE ANY WORKERS'COMPENSATION IMMUNITY IT MAY OTHERWISE HAVE.THE DUTY TO DEFEND,INDEMNIFY AND HOLD HARMLESS INDEMNITIEES
EXISTS WHETHER BASED IN WHOLE OR IN PART UPON THE ACTIVE, PASSIVE,OR CONCURRENT NEGLIGENCE OF INDEMNITEES.IN JURISDICTIONS IN
WHICH THE INDEMNIFICATION PROVIDED FOR IN THIS SECTION IS BROADER THAN THAT ALLOWED BY APPLICABLE LAW,THIS SECTION SHALL BE
INTERPRETED AS PROVIDING THE BROADEST INDEMNIFICATION PERMITTED AND SHALL BE LIMITED ONLY TO THE EXTENT NECESSARY TO COMPLY
WITH SAID LAW.CUSTOMER'S OBLIGATION TO INDEMNIFY AND AND HOLD HARMLESS AND THE DUTY TO DEFEND THE INDEMNITEES SHALL SURVIVE
THE TERMINATION OF THIS AGREEMENT.IT IS EXPESSLYAGREED THAT THIS INDEMNIFICATION CLAUSE APPLIES TO BOTH THIRD PARTY CLAIMS AND TO
ALL CLAIMS BETWEEN BELLA AND CUSTOMER.
4. Payment Adjustments:Payments under this Service Agreement shall be subject to adjustment for excess hours of operator and/or crew time and/or excess hours of usage
of the equipment In accordance with the rates agreed to at the lime of the order,custom in the area where the lifting operation occurs,or by any applicable collective bargaining
agreement,as the case may be.The Customer shall not be entitled to any abatement,deduction,reduction,set-off,counterclaim,recoupment or defense against payment for
any reason.BELLA SHALL NOT BE LIABLE FOR ANY INCIDENTAL OR CONSEQUENTIAL DAMAGES RELATING TO THIS SERVICE AGREEMENT.
5.Payment:All payments required hereunder shall be due in full upon billing. In addition to the rates shown on the face of this Service Agreement,Customer shall pay the
following arising In connection with this Service Agreement:
(a)All demurrage,storage,switching,and drayage.
(b)All taxes,levies,assessment,fees and other public charges assessed as a result of this Service Agreement,including tangible personal property taxes if applicable.
(c)All judgments,awards,fines,penalties,forfeitures,court costs,expenses,attomeys'fees(unless prohibited by law)incurred by Bella in accordance with Item 3 above.
(d) Bella's costs and expenses including reasonable attorney's fees(unless prohibited by law)incurred in enforcing this Service Agreement and/or collecting any amounts due
hereunder.
(e) In the event of an accident resulting in damage to the equipment arising from the acts,omissions,misfeasance or malfeasance of Customer,including,but not limited to
Customer's employees,agents,representatives,subcontractors or independent contractors,Customer shall be responsible for costs of repairs and related expenses,all
consequential losses Including,but not limited to loss of use of equipment,loss of profits and all reasonable attorney fees incurred in recovering such damages.
Any payments more than 30 days past due hereunder shall bear interest at the rate of 1.5%per month(or the maximum amount allowed by law whichever is higher).
S.Default:If Customer fails to pay when due any payment or other amount required herein to be paid by Customer,or if Customer makes an assignment for the benefit of
creditors,whether voluntary or involuntary,or if a petition is filed by or against Customer under the bankruptcy laws of the United States,in addition to other remedies Bella
may exercise to protect Its interest,CUSTOMER AGREES THAT BELLA MAY TAKE POSSESSION OF ANY OR ALL ITEMS OF EQUIPMENT,WHEREVER THESE ITEMS
MAY BE LOCATED,WITHOUT DEMAND OR NOTICE,WITHOUT ANY COURT ORDER OR OTHER PROCESS OF LAW AND WITHOUT LIABILITY TO CUSTOMER FOR
ANY DAMAGES OCCASIONED BY SUCH TAKINGS OR POSSESSION.
7.Notice:Customer shall immediately notify Bella of any event or occurrence involving personal Injury and/or property damage of any kind relating to Bella personnel or
equipment.Customer shall immediately deliver to Bella any summons,pleading,notice or paper of any kind involving any claim,suit or proceeding relating to any event or
occurrence involving Bella personnel or equipment Customer shall not aid or abet the assertion of any such claim,suit or proceeding,and shall fully cooperate with Bella in
investigating and defending the same.
8.Access&Site Conditions: Customer shall provide Bella with proper ingress to and egress from the job site and provide protection of all paving,curbs,real estate,
structures and/or improvements and Customer shall be responsible for all damage to any paving,curbs,real estate,structures and/or improvements caused by Customer's
failure to provide proper ingress and egress.Customer is responsible for providing an operating area that is suitable for operation of the crane with respect to levelness,surface
conditions,support capability,proximity to powerlines,excavations,slopes,underground utilities,subsurface conditions,and obstructions to crane operations.Bella shall have
immediate access to and the right to retrieve and repair any equipment used in the lifting operation and shall be entitled to enter upon Customer's property at any time for those
purposes even in the event of a strike affecting Customer's facility or property.Customer shall perform or have performed all necessary inspections or testing to determine the
nature of the ground or soil and its ability to support the crane while in operation or otherwise.If the ground or soil condition is such that It cannot support the crane,Customer
shall take all necessary measures to insure that these conditions are remedied prior to the crane being placed on that ground or soil.These measures include,but are not limited
to,the provision of proper shoring or cribbing or other measures.
8. Jury Waiver:UNLESS PROHIBITED BYLAW,BELLA AND CUSTOMER EACH KNOWINGLY,UNCONDITIONALLY AND IRREVOCABLY WAIVE TRIAL BY JURY WITI
RESPECT TO ANY ACTION, CLAIM,SUIT OR PROCEEDING,WHETHER SOUNDING IN CONTRACT TORT OR OTHERWISE IN RESPECT OF ARISING OUT OF,
RELATED TO,OR INCIDENTAL TO THE RELATIONSHIP ESTABLISHED BETWEEN THEM(OR AS TO ANY THIRD PARTIES)IN CONNECTION WITH THIS AGREE
MENT,ANY OTHER DOCUMENT OR AGREEMENT EXECUTED OR DELIVERED IN CONNECTION HEREWITH OR THE PARTIES'CONDUCT
10. Assignment or Transfer.This is a personal services agreement that neither Bella nor Customer may assign or transfer to a third party without the combined prior
written consent of both parties hereto(which consent may be withheld for any reason).Any such consent shall not release Customer from any obligations under this Service
Agreement.
11. Lifting,Lugs,Rigging and Apparatus:Customer assumes all responsibility and liability to ensure the adequacy of the design and strength of any lifting lug or device embedde
in or attached to any object,and of any and all rigging or lifting apparatus failures or defects(even if such rigging or lifting apparatus is supplied by Bella to Customer).
12. MSDS Requirements:Customer shall provide Bella,upon delivery of the equipment to the job location,copies of material safety data sheets(MSDS)for all hazardous chemicals i
use at said job location.or make such MSDS available at a central location at the job site,In order that Bella may comply with the requirements of all applicable local,state and Federal law;
and regulations.Including but not limited to OSHA regulations,regarding hazardous materials.Furthermore,Customer shall inform Bella of all precautionary measures that need to be take
to protect the operators and/or crew or other personnel during normal operating conditions and in foreseeable emergencies.
13. Full Agreement;Governing Law;Waiver,Severability:This Service Agreement constitutes the full agreement between Bella and Customer.Any changes to this Service
Agreement must be In writing signed by Bella and Customer.No conduct by either party to this Service Agreement shall be deemed a modification of this Service Agreement This Servic
Agreement shall be binding upon and inure to the benefit of the parties hereto and their respective successors and permissible assigns.The laws of the state of the job site location shall
govern this Service Agreement.Headings are provided for convenience only,and not for Interpretation of this Service Agreement.Bella and Customer are Independent contractors of each
other and Customer shall not be deemed to be the agent,servant or employee of Bella for any reason or purpose.No failure of Bella to enforce performance of any terms or covenants,or
failure to exercise or delay in exercising any right under this Service Agreement shall operate as a waiver thereof;nor shall any single or partial exercise of any right hereunder preclude any
other or further exercise thereof or the exercise of any other right.If any provision of this Service Agreement is held to be invalid or Illegal by a court of competent jurisdiction,the invalid or
illegal term will be deemed excluded from this Service Agreement and will not invalidate the remaining terms of this Service Agreement.

PART 1GENERAL
1.01 SUMMARY
A. Section Includes:
1. Adhered ketone ethylene ester(KEE) roofing system.
2. Substrate board.
3. Roof and perimeter wall insulation.
4. Cover board.
5. Walkway pads.
6. Expansion joints.
1.02 DEFINITIONS
A. Roofing Terminology: Definitions in ASTM D1079 and glossary in NRCA's"The NRCA Roofing
Manual: Membrane Roof Systems" apply to work of this Section.
1.03 PREINSTALLATION MEETINGS
A. Preliminary Roofing Conference: Conduct conference at Project site to comply with Division 01
requirements and the following:
1. Meet with Owner, Consultant, Construction Manager, Owner's representative, testing and
inspecting agency representative, roofing installer, roofing system manufacturer's
representative, deck installer, air barrier installer, and installers whose work interfaces with or
affects roofing, including installers of roof accessories and roof-mounted equipment.
2. Review methods and procedures related to roofing installation, including manufacturer's written
instructions.
3. Review and finalize construction schedule.
4. Examine deck substrate conditions and finishes for compliance with requirements.
5. Review structural loading limitations of roof deck during and after roofing.
6. Review base flashings, special roofing details, roof drainage, roof penetrations, equipment
curbs, and condition of other construction that affects roofing system.
7. Review governing regulations and requirements for insurance and certificates.
8. Review temporary protection requirements for roofing system during and after installation.
9. Review roof observation and repair procedures after roofing installation.
1.04 SUBMITTALS, GENERAL
A. See Section 01 3000-Administrative Requirements for submittal procedures.
1.05 ACTION SUBMITTALS
A. Product Data: For each type of product.
1. For insulation and roof system component fasteners, include copy of SPRI's Directory of Roof
Assemblies listing.
2. Provide Florida Building Code product approvals and additional engineering documentation as
required by Authorities Having Jurisdiction.
B. Shop Drawings: Include the following:
1. Roof plans indicating wind zones per Structural Drawings and ASCE 7 and fastening and
adherence requirements.
2. Manufacturer's standard details for typical conditions.
3. Layout and thickness of insulation.
4. Base flashings and membrane terminations.
5. Flashing details at penetrations; coordinate with Section 07 62 00 "Sheet Metal Flashing and
Trim."
6. Tapered insulation, including slopes.

performance requirements.
1.06 INFORMATIONAL SUBMITTALS
A. Manufacturer Certificates:
1. Performance Requirement Certificate: Signed by roof membrane manufacturer, certifying that
roofing system complies with requirements specified in "Performance Requirements"Article.
a. Submit evidence of compliance with performance requirements.
2. Special Warranty Certificate: Signed by roof membrane manufacturer, certifying that all
materials supplied under this Section are acceptable for special warranty.
B. Product Test Reports: For roof membrane and insulation, for tests performed by a qualified testing
agency, indicating compliance with specified requirements.
C. Research reports.
D. Field Test Reports:
1. Concrete internal relative humidity test reports.
2. Fastener-pullout test results and manufacturer's revised requirements for fastener patterns.

1.07 QUALITY ASSURANCE
A. Manufacturer Qualifications: A qualified manufacturer that is listed in SPRI's Directory of Roof
Assemblies for roofing system identical to that used for this Project.
B. Installer Qualifications: A qualified firm that is approved, authorized, or licensed by roofing system
manufacturer to install manufacturer's product and that is eligible to receive manufacturer's special
warranty.
1.08 DELIVERY, STORAGE, AND HANDLING
A. Deliver materials in manufacturer's original containers, dry and undamaged, with seals and labels
intact.
B. Store materials in weather protected environment, clear of ground and moisture.
C. Ensure storage and staging of materials does not exceed static and dynamic load-bearing capacities
of roof decking.
D. Protect insulation from direct exposure to sunlight.
1.09 FIELD CONDITIONS
A. Weather Limitations: Proceed with installation only when existing and forecasted weather conditions
permit roofing system to be installed according to manufacturer's written instructions and warranty
requirements.
B. Do not expose materials vulnerable to water or sun damage in quantities greater than can be
weatherproofed the same day.
C. Schedule applications so that no partially completed sections of roof are left exposed at end of
workday.

1.10 WARRANTY
A. Manufacturer's System Warranty: Manufacturer agrees to repair or replace, without monetary
limitation, components of roofing system that do not remain watertight; that fail in adhesion,
cohesion, or general durability; or that deteriorate in manner not clearly specified by submitted
roofing-system manufacturer's data as inherent quality of material for application indicated within the
specified warranty period.
1. Warranty includes roof membrane, base flashings, roof insulation, fasteners, cover boards,
substrate boards, walkway pads, and other components of roofing system.
2. Warranty Period: Stated in Warranty Form at the end of this Section.
a. Include a 150 mph windspeed rider.
B. Roofing Installer's Warranty:
1. Completed Warranty Form at end of this Section, signed by Roofing Installer.
a. Repair or replace components of roofing system that do not comply with requirements;
that do not remain watertight; that fail in adhesion, cohesion, or general durability; or that
deteriorate in manner not clearly specified by submitted roofing-system manufacturer's
data as inherent quality of material for application indicated. Warranty includes defects
such as blisters, ridging, and excessive surfacing loss.
b. Removal and replacement of base sheets, insulation, cover boards, walkway products,
and other components of roofing system. Warranty includes replacing materials and labor
as necessary.
2. Warranty Period: Stated in Warranty Form at the end of this Sectio.
PART 2 PRODUCTS
2.01 PERFORMANCE REQUIREMENTS
A. Accelerated Weathering: Roof membrane shall withstand 2000 hours of exposure when tested
according to ASTM G152, ASTM G154, or ASTM G155.
B. Impact Resistance: Roof membrane shall resist impact damage when tested according to
ASTM D3746 or ASTM D4272/D4272M, or the"Resistance to Foot Traffic Test" in FM
Approvals 4470.
C. Wind Uplift Resistance: Design roofing system to resist the wind uplift pressures indicated on the
Components and Cladding Wind Pressures table in the Drawings, with a 1.67 safety factor, when
tested according to UL 580 and UL 1897.
D. Solar Reflectance Index (SRI): Three-year-aged SRI not less than 64 or initial SRI not less than 82
when calculated according to ASTM E 1980, based on testing identical products by a qualified
testing agency.
E. Exterior Fire-Test Exposure: ASTM E108 or UL 790, Class A; for application and roof slopes
indicated; testing by a qualified testing agency. Identify products with appropriate markings of
applicable testing agency.
2.02 KETONE ETHYLENE ESTER(KEE)/STYRENE-BUTADIENE-STYRENE (SBS) MODIFIED BITUMEN
MEMBRANE ROOFING - DRAWING DESIGNATION: MR-1
A. To establish standards of manufacture, operation, performance, and appearance, Drawings and
Specifications are based on products of FiberTite (Seaman Corporation) Multi-Ply Roofing System.
Provided compliance with Project requirements and prior approval by the Owner and the Architect
products of the following manufacturer will also be acceptable:
1. Tremco TremPly Kee FB.
B. Roofing System:
1. Base Ply: ASTM D 6164/D66164M SBS modified bitumen sheet, reinforced with a non-woven
polyester mat.
a. Average Weight: 91 Ibs/100 sq. ft(4.4 kg/m2).
b. Polyolefin burn-off film; both faces for torch application.
c. Product: FiberTite SBS 190 TG Base.

a. Ketone Ethylene Ester(KEE) Content: Not less than 50 percent by weight of the polymer
content of the sheet when tested in accordance with ASTM D8154.
b. Thickness: 50 mils (1.2 mm), nominal.
C. Exposed Face Color: White.
d. Product: FiberTite-XT FB Membrane.
2.03 AUXILIARY ROOFING MATERIALS
A. General: Auxiliary materials recommended by roofing system manufacturer for intended use and
compatible with other roofing components.
1. Adhesives and Sealants: Comply with VOC limits of Authorities Having Jurisdiction.
B. Sheet Flashing: Manufacturer's standard sheet flashing of same material, type, reinforcement,
thickness, and color as KEE sheet.
C. Prefabricated Pipe Flashings: As recommended by roof membrane manufacturer.
D. Bonding Adhesive: Manufacturer's standard.
E. Slip Sheet: Manufacturer's standard, of thickness required for application.
F. Metal Termination Bars: Manufacturer's standard, predrilled stainless steel or aluminum bars,
approximately 1 by 1/8 inch (25 by 3 mm) thick; with anchors.
G. Fasteners: Manufacturer's standard No. 14-13, heavy duty threaded steel No. 3 Phillips truss, self-
tapping corrosion resistant fastener.
H. Miscellaneous Accessories: Provide single-component gun-grade polyether, preformed cone and
vent sheet flashings, preformed inside and outside corner sheet flashings, T joint covers, lap
sealants, termination reglets, and other accessories required for a complete, weather-tight
installation.
2.04 SUBSTRATE AND COVER BOARDS
A. Substrate Board: ASTM C1177/C1177M, glass-mat, water-resistant gypsum substrate, or
ASTM C1278/C1278M, fiber-reinforced gypsum board.
1. Subject to compliance with requirements, provide products by one of the following:
a. Georgia-Pacific Gypsum, LLC.
b. National Gypsum Company.
c. USG Corporation.
B. Fasteners: Factory-coated steel fasteners and metal or plastic plates complying with corrosion-
resistance provisions in FM Approvals 4470, designed for fastening substrate board to roof deck.
2.05 ROOF INSULATION
A. General: Preformed roof insulation boards manufactured or approved by KEE roof membrane
manufacturer
B. Polyisocyanurate Board Insulation: ASTM C1289, Type II, Class 1, Grade 3, felt or glass-fiber mat
facer on both major surfaces.
1. Manufacturers: Subject to compliance with requirements, provide products by one of the
following:
a. Carlisle SynTec Incorporated.

PART 3 EXECUTION
3.01 EXAMINATION
A. Examine substrates, areas, and conditions, with Installer present, for compliance with requirements
and other conditions affecting performance of the Work.
1. Verify that metal roof deck is properly secured and free of damage.
3.02 PREPARATION
A. Clean substrate of dust, debris, moisture, and other substances detrimental to roofing system
installation according to roofing system manufacturer's written instructions. Remove sharp
projections.
B. Prevent materials from entering and clogging roof drains and conductors and from spilling or
migrating onto surfaces of other construction. Remove roof-drain plugs when no work is taking place
or when rain is forecast.

3.03 INSTALLATION OF ROOFING, GENERAL
A. Install roofing system according to roofing system manufacturer's written instructions, SPRI's
Directory of Roof Assemblies listed roof assembly requirements, and FM Global Property Loss
Prevention Data Sheet 1-29.
B. Complete terminations and base flashings and provide temporary seals to prevent water from
entering completed sections of roofing system at the end of workday or when rain is forecast.
Remove and discard temporary seals before beginning work on adjoining roofing.
3.04 INSTALLATION OF SUBSTRATE BOARD OVER METAL DECK
A. Install substrate board with long joints in continuous straight lines, with end joints staggered not less
than 24 inches (610 mm) in adjacent rows.
1. At steel roof decks, install substrate board at right angle to flutes of deck locating joints over
crests of roof deck.
2. Tightly butt substrate boards together.
3. Cut substrate board to fit tight around penetrations and projections, and to fit tight to
intersecting sloping roof decks.
4. Fasten substrate board to top flanges of steel deck according to recommendations in SPRI's
Directory of Roof Assemblies listed roof assembly requirements for specified Wind Uplift Load
Capacity and FM Global Property Loss Prevention Data Sheet 1-29.
3.05 INSTALLATION OF INSULATION
A. Coordinate installing roofing system components so insulation is not exposed to precipitation or left
exposed at the end of the workday.
B. Comply with roofing system and insulation manufacturer's written instructions for installing roof
insulation.
C. Installation Over Metal Decking:
1. Install base layer of insulation with joints staggered not less than 24 inches (610 mm) in
adjacent rows.
a. Trim insulation neatly to fit around penetrations and projections, and to fit tight to
intersecting sloping roof decks.
b. Make joints between adjacent insulation boards not more than 1/4 inch (6 mm) in width.
c. Fill gaps exceeding 1/4 inch (6 mm)with insulation.
d. Cut and fit insulation within 1/4 inch (6 mm) of nailers, projections, and penetrations.
e. Adhere base layer of insulation and substrate board using insulation manufacturer's
proprietary two-part foam adhesive according to requirements in SPRI's Directory of Roof
Assemblies for specified Wind Uplift Load Capacity.
2. Install upper layers of insulation and tapered insulation with joints of each layer offset not less
than 12 inches (305 mm) from previous layer of insulation.
a. Staggered end joints within each layer not less than 24 inches (610 mm) in adjacent rows.
b. Install with long joints continuous and with end joints staggered not less than 12 inches
(305 mm) in adjacent rows.
c. Trim insulation neatly to fit around penetrations and projections, and to fit tight to
intersecting sloping roof decks.
d. Make joints between adjacent insulation boards not more than 1/4 inch (6 mm) in width.
e. Provide prefabricated sloped insulation at internal roof drains to create a square drain
sump with each side equal to the diameter of the drain bowl plus 24 inches (610 mm).
1) Trim insulation so that water flow is unrestricted.
f. Fill gaps exceeding 1/4 inch (6 mm)with insulation.
g. Cut and fit insulation within 1/4 inch (6 mm) of nailers, projections, and penetrations.

direction.
1. Trim cover board neatly to fit around penetrations and projections, and to fit tight to intersecting
sloping roof decks.
2. At internal roof drains, conform to slope of drain sump.
a. Trim cover board so that water flow is unrestricted.
3. Cut and fit cover board tight to nailers, projections, and penetrations.
4. Adhere cover board to substrate using adhesive according to SPRI's Directory of Roof
Assemblies listed roof assembly requirements for specified Wind Uplift Load Capacity and FM
Global Property Loss Prevention Data Sheet 1-29, as follows:
a. Set cover board in a uniform coverage of full-spread insulation adhesive, firmly pressing
and maintaining insulation in place.
3.07 INSTALLATION OF ADHERED ROOF MEMBRANE
A. Adhere roof membrane over area to receive roofing according to roofing system manufacturer's
written instructions. Unroll roof membrane and allow to relax before installing.
B. Start installation of roofing in presence of roofing system manufacturer's technical personnel and
Owner's testing and inspection agency.
C. Accurately align roof membrane and maintain uniform side and end laps of minimum dimensions
required by manufacturer. Stagger end laps.
D. Fabric-Backed Roof Membrane Adhesive: Apply to roof membrane and substrate at rate required by
manufacturer and install fabric-backed roof membrane.
E. Apply roof membrane with side laps shingled with slope of roof deck where possible.
F. Extend base membrane vertically up parapet walls and projections to a minimum height equal to
height of insulation and cover board.
G. Seams: Clean seam areas, overlap roofing, and hot-air weld side and end laps of roof membrane
and sheet flashings to ensure a watertight seam installation.
1. Test lap edges with probe to verify seam weld continuity. Apply lap sealant to seal cut edges of
roof membrane and sheet flashings.
2. Verify field strength of seams a minimum of three times daily or for every 1000 square feet of
membrane, and repair seam sample areas.
3. Repair tears, voids, and lapped seams in roof membrane that do not comply with requirements.
H. Spread sealant bed over deck-drain flange at roof drains, and securely seal roof membrane in place
with clamping ring.
3.08 INSTALLATION OF BASE FLASHING
A. Install sheet flashings and preformed flashing accessories and adhere to substrates according to
roofing system manufacturer's written instructions.
B. Apply bonding adhesive to substrate and underside of sheet flashing at required rate, and allow to
partially dry. Do not apply to seam area of flashing.
C. Flash penetrations and field-formed inside and outside corners with cured or uncured sheet flashing.
D. Clean seam areas, overlap, and firmly roll sheet flashings into the adhesive. Hot-air weld side and
end laps to ensure a watertight seam installation.

E. Terminate and seal top of sheet flashings and mechanically anchor to substrate through termination
bars.
3.09 INSTALLATION OF WALK PADS
A. Flexible Walkways: Install walkway products according to manufacturer's written instructions.
1. Install flexible walk pads as indicated on the Drawings.
2. Provide 6-inch (76-mm) clearance between adjoining pads.
3. Trim walk pads so that valleys along crickets are not obstructed.
4. Adhere walkway products to substrate with compatible adhesive according to roofing system
manufacturer's written instructions.
3.10 FIELD QUALITY ASSURANCE
A. The Construction Manager will engage a qualified testing agency to perform tests and to inspect
substrate conditions, surface preparation, roof membrane application, sheet flashings, protection,
and drainage components, and to furnish reports to the Owner and the Architect.
B. Low-Voltage Electrical Conductance Testing: The testing agency will survey the entire roof area and
flashings to locate discontinuity in the roof membrane using an exposed metal electrical loop to
create an electrical field tested with handheld probes or a scanning platform with integral perimeter
electrical loops creating a complete electrical field.
1. Testing will be performed before overlying construction is placed.
2. After testing, repair areas of discontinuities, repeat tests, and make further repairs, at no
additional cost to the Owner, until roofing and flashing installations are contiguous.
3. The testing agency will prepare survey report indicating locations of initial discontinuities, if any.
C. Arrange for the roofing manufacturer's technical representative to inspect roofing system installation
during construction at intervals appropriate to progress, but no less than five times for each roofing
system type. Include at a minimum start insulation, start of base sheet, start of cap sheet, and
completion of cap sheet.
D. Final Roof Inspection: Arrange for roofing system manufacturer's technical personnel to inspect
roofing installation on completion, in presence of Owner's representative, and to prepare inspection
report.
E. At Construction 's discretion perform field uplift testing in accordance with FM 1-52 /ASTM E907 to
validate uplift performance.
F. Repair, or remove and replace with new, components of roofing system where inspections indicate
that they do not comply with specified requirements.
G. Additional testing and inspecting, at Contractor's expense, will be performed to determine if replaced
or additional work complies with specified requirements.
3.11 PROTECTING AND CLEANING
A. Protect roofing system from damage discoloration, and wear during remainder of construction
period. When remaining construction does not affect or endanger roofing, inspect roofing system for
deterioration and damage, describing its nature and extent in a written report, with copies to
Architect and Owner.
B. Correct deficiencies in or remove roofing system that does not comply with requirements, repair
substrates, and repair or reinstall roofing system to a condition free of damage and deterioration at
time of Substantial Completion and according to warranty requirements.
C. Clean overspray and spillage from adjacent construction using cleaning agents and procedures
recommended in writing by manufacturer of affected construction.
D. Clean roof membrane as recommended in writing by manufacturer following construction to remove
discoloration that may impact SRI rating.

3.12 ROOFING INSTALLER'S WARRANTY
A. WHEREAS of herein
called the "Roofing Installer," has performed roofing and associated work("work") on the following
project:
1. Owner: Key West International Airport.
2. Address: 3491 S Roosevelt Blvd., Key West, Florida 33040.
3. Building Name/Type: EYW Concourse A.
4. Address: 3491 S Roosevelt Blvd., Key West, Florida 33040.
5. Area of Work: As indicated on Drawings.

7. Warranty Period: Two years from date of Substantial Completion.

B. AND WHEREAS Roofing Installer has contracted (either directly with Owner or indirectly as a
subcontractor) to warrant said work against leaks and faulty or defective materials and workmanship
for designated Warranty Period,
C. NOW THEREFORE Roofing Installer hereby warrants, subject to terms and conditions herein set
forth, that during Warranty Period Roofing Installer will, at Roofing Installer's own cost and expense,
make or cause to be made such repairs to or replacements of said work as are necessary to correct
faulty and defective work and as are necessary to maintain said work in a watertight condition.
D. This Warranty is made subject to the following terms and conditions:
1. Specifically excluded from this Warranty are damages to work and other parts of the building,
and to building contents, caused by:
a. Lightning;
b. Peak gust wind speed exceeding mph (include wind speed rider);
c. Fire;
d. Failure of roofing system substrate, including cracking, settlement, excessive deflection,
deterioration, and decomposition;
e. Faulty construction of parapet walls, copings, chimneys, skylights, vents, equipment
supports, and other edge conditions and penetrations of the Work;
f. Vapor condensation on bottom of roofing; and
g. Activity on roofing by others, including construction contractors, maintenance personnel,
other persons, and animals, whether authorized or unauthorized by Owner.
2. When work has been damaged by any of foregoing causes, Warranty shall be null and void
until such damage has been repaired by Roofing Installer and until cost and expense thereof
have been paid by Owner or by another responsible party so designated.
3. Roofing Installer is responsible for damage to work covered by this Warranty but is not liable
for consequential damages to building or building contents resulting from leaks or faults or
defects of work.
4. During Warranty Period, if Owner allows alteration of work by anyone other than Roofing
Installer, including cutting, patching, and maintenance in connection with penetrations,
attachment of other work, and positioning of anything on roof, this Warranty shall become null
and void on date of said alterations, but only to the extent said alterations affect work covered
by this Warranty. If Owner engages Roofing Installer to perform said alterations, Warranty shall
not become null and void unless Roofing Installer, before starting said work, shall have notified
Owner in writing, showing reasonable cause for claim, that said alterations would likely damage
or deteriorate work, thereby reasonably justifying a limitation or termination of this Warranty.
5. During Warranty Period, if original use of roof is changed and it becomes used for, but was not
originally specified for, a promenade, work deck, spray-cooled surface, flooded basin, or other
use or service more severe than originally specified, this Warranty shall become null and void
on date of said change, but only to the extent said change affects work covered by this
Warranty.

6. Owner shall promptly notify Roofing Installer of observed, known, or suspected leaks, defects,
or deterioration and shall afford reasonable opportunity for Roofing Installer to inspect work and
to examine evidence of such leaks, defects, or deterioration.
7. This Warranty is recognized to be the only warranty of Roofing Installer on said work and shall
not operate to restrict or cut off Owner from other remedies and resources lawfully available to
Owner in cases of roofing failure. Specifically, this Warranty shall not operate to relieve Roofing
Installer of responsibility for performance of original work according to requirements of the
Contract Documents, regardless of whether Contract was a contract directly with Owner or a
subcontract with Owner's General Contractor.

1.07 QUALITY ASSURANCE
A. Manufacturer Qualifications: A qualified manufacturer that is listed in SPRI's Directory of Roof
Assemblies for roofing system identical to that used for this Project.
B. Installer Qualifications: A qualified firm that is approved, authorized, or licensed by roofing system
manufacturer to install manufacturer's product and that is eligible to receive manufacturer's special
warranty.
1.08 DELIVERY, STORAGE, AND HANDLING
A. Deliver materials in manufacturer's original containers, dry and undamaged, with seals and labels
intact.
B. Store materials in weather protected environment, clear of ground and moisture.
C. Ensure storage and staging of materials does not exceed static and dynamic load-bearing capacities
of roof decking.
D. Protect insulation from direct exposure to sunlight.
1.09 FIELD CONDITIONS
A. Weather Limitations: Proceed with installation only when existing and forecasted weather conditions
permit roofing system to be installed according to manufacturer's written instructions and warranty
requirements.
B. Do not expose materials vulnerable to water or sun damage in quantities greater than can be
weatherproofed the same day.
C. Schedule applications so that no partially completed sections of roof are left exposed at end of
workday.

1.10 WARRANTY
A. Manufacturer's System Warranty: Manufacturer agrees to repair or replace, without monetary
limitation, components of roofing system that do not remain watertight; that fail in adhesion,
cohesion, or general durability; or that deteriorate in manner not clearly specified by submitted
roofing-system manufacturer's data as inherent quality of material for application indicated within the
specified warranty period.
1. Warranty includes roof membrane, base flashings, roof insulation, fasteners, cover boards,
substrate boards, walkway pads, and other components of roofing system.
2. Warranty Period: Stated in Warranty Form at the end of this Section.
a. Include a 150 mph windspeed rider.
B. Roofing Installer's Warranty:
1. Completed Warranty Form at end of this Section, signed by Roofing Installer.
a. Repair or replace components of roofing system that do not comply with requirements;
that do not remain watertight; that fail in adhesion, cohesion, or general durability; or that
deteriorate in manner not clearly specified by submitted roofing-system manufacturer's
data as inherent quality of material for application indicated. Warranty includes defects
such as blisters, ridging, and excessive surfacing loss.
b. Removal and replacement of base sheets, insulation, cover boards, walkway products,
and other components of roofing system. Warranty includes replacing materials and labor
as necessary.
2. Warranty Period: Stated in Warranty Form at the end of this Sectio.
PART 2 PRODUCTS
2.01 PERFORMANCE REQUIREMENTS
A. Accelerated Weathering: Roof membrane shall withstand 2000 hours of exposure when tested
according to ASTM G152, ASTM G154, or ASTM G155.
B. Impact Resistance: Roof membrane shall resist impact damage when tested according to
ASTM D3746 or ASTM D4272/D4272M, or the"Resistance to Foot Traffic Test" in FM
Approvals 4470.
C. Wind Uplift Resistance: Design roofing system to resist the wind uplift pressures indicated on the
Components and Cladding Wind Pressures table in the Drawings, with a 1.67 safety factor, when
tested according to UL 580 and UL 1897.
D. Solar Reflectance Index (SRI): Three-year-aged SRI not less than 64 or initial SRI not less than 82
when calculated according to ASTM E 1980, based on testing identical products by a qualified
testing agency.
E. Exterior Fire-Test Exposure: ASTM E108 or UL 790, Class A; for application and roof slopes
indicated; testing by a qualified testing agency. Identify products with appropriate markings of
applicable testing agency.

PART 3 EXECUTION
3.01 EXAMINATION
A. Examine substrates, areas, and conditions, with Installer present, for compliance with requirements
and other conditions affecting performance of the Work.
1. Verify that metal roof deck is properly secured and free of damage.
3.02 PREPARATION
A. Clean substrate of dust, debris, moisture, and other substances detrimental to roofing system
installation according to roofing system manufacturer's written instructions. Remove sharp
projections.
B. Prevent materials from entering and clogging roof drains and conductors and from spilling or
migrating onto surfaces of other construction. Remove roof-drain plugs when no work is taking place
or when rain is forecast.

3.03 INSTALLATION OF ROOFING, GENERAL
A. Install roofing system according to roofing system manufacturer's written instructions, SPRI's
Directory of Roof Assemblies listed roof assembly requirements, and FM Global Property Loss
Prevention Data Sheet 1-29.
B. Complete terminations and base flashings and provide temporary seals to prevent water from
entering completed sections of roofing system at the end of workday or when rain is forecast.
Remove and discard temporary seals before beginning work on adjoining roofing.
3.04 INSTALLATION OF SUBSTRATE BOARD OVER METAL DECK
A. Install substrate board with long joints in continuous straight lines, with end joints staggered not less
than 24 inches (610 mm) in adjacent rows.
1. At steel roof decks, install substrate board at right angle to flutes of deck locating joints over
crests of roof deck.
2. Tightly butt substrate boards together.
3. Cut substrate board to fit tight around penetrations and projections, and to fit tight to
intersecting sloping roof decks.
4. Fasten substrate board to top flanges of steel deck according to recommendations in SPRI's
Directory of Roof Assemblies listed roof assembly requirements for specified Wind Uplift Load
Capacity and FM Global Property Loss Prevention Data Sheet 1-29.
3.05 INSTALLATION OF INSULATION
A. Coordinate installing roofing system components so insulation is not exposed to precipitation or left
exposed at the end of the workday.
B. Comply with roofing system and insulation manufacturer's written instructions for installing roof
insulation.
C. Installation Over Metal Decking:
1. Install base layer of insulation with joints staggered not less than 24 inches (610 mm) in
adjacent rows.
a. Trim insulation neatly to fit around penetrations and projections, and to fit tight to
intersecting sloping roof decks.
b. Make joints between adjacent insulation boards not more than 1/4 inch (6 mm) in width.
c. Fill gaps exceeding 1/4 inch (6 mm)with insulation.
d. Cut and fit insulation within 1/4 inch (6 mm) of nailers, projections, and penetrations.
e. Adhere base layer of insulation and substrate board using insulation manufacturer's
proprietary two-part foam adhesive according to requirements in SPRI's Directory of Roof
Assemblies for specified Wind Uplift Load Capacity.
2. Install upper layers of insulation and tapered insulation with joints of each layer offset not less
than 12 inches (305 mm) from previous layer of insulation.
a. Staggered end joints within each layer not less than 24 inches (610 mm) in adjacent rows.
b. Install with long joints continuous and with end joints staggered not less than 12 inches
(305 mm) in adjacent rows.
c. Trim insulation neatly to fit around penetrations and projections, and to fit tight to
intersecting sloping roof decks.
d. Make joints between adjacent insulation boards not more than 1/4 inch (6 mm) in width.
e. Provide prefabricated sloped insulation at internal roof drains to create a square drain
sump with each side equal to the diameter of the drain bowl plus 24 inches (610 mm).
1) Trim insulation so that water flow is unrestricted.
f. Fill gaps exceeding 1/4 inch (6 mm)with insulation.
g. Cut and fit insulation within 1/4 inch (6 mm) of nailers, projections, and penetrations.

3.06 INSTALLATION OF COVER BOARDS

staggered between rows. Offset joints of insulation below a minimum of 6 inches (150 mm) in each
direction.
1. Trim cover board neatly to fit around penetrations and projections, and to fit tight to intersecting
sloping roof decks.
2. At internal roof drains, conform to slope of drain sump.
a. Trim cover board so that water flow is unrestricted.
3. Cut and fit cover board tight to nailers, projections, and penetrations.
4. Adhere cover board to substrate using adhesive according to SPRI's Directory of Roof
Assemblies listed roof assembly requirements for specified Wind Uplift Load Capacity and FM
Global Property Loss Prevention Data Sheet 1-29, as follows:
a. Set cover board in a uniform coverage of full-spread insulation adhesive, firmly pressing
and maintaining insulation in place.
3.07 INSTALLATION OF ADHERED ROOF MEMBRANE
A. Adhere roof membrane over area to receive roofing according to roofing system manufacturer's
written instructions. Unroll roof membrane and allow to relax before installing.
B. Start installation of roofing in presence of roofing system manufacturer's technical personnel and
Owner's testing and inspection agency.
C. Accurately align roof membrane and maintain uniform side and end laps of minimum dimensions
required by manufacturer. Stagger end laps.
D. Fabric-Backed Roof Membrane Adhesive: Apply to roof membrane and substrate at rate required by
manufacturer and install fabric-backed roof membrane.
E. Apply roof membrane with side laps shingled with slope of roof deck where possible.
F. Extend base membrane vertically up parapet walls and projections to a minimum height equal to
height of insulation and cover board.
G. Seams: Clean seam areas, overlap roofing, and hot-air weld side and end laps of roof membrane
and sheet flashings to ensure a watertight seam installation.
1. Test lap edges with probe to verify seam weld continuity. Apply lap sealant to seal cut edges of
roof membrane and sheet flashings.
2. Verify field strength of seams a minimum of three times daily or for every 1000 square feet of
membrane, and repair seam sample areas.
3. Repair tears, voids, and lapped seams in roof membrane that do not comply with requirements.
H. Spread sealant bed over deck-drain flange at roof drains, and securely seal roof membrane in place
with clamping ring.
3.08 INSTALLATION OF BASE FLASHING
A. Install sheet flashings and preformed flashing accessories and adhere to substrates according to
roofing system manufacturer's written instructions.
B. Apply bonding adhesive to substrate and underside of sheet flashing at required rate, and allow to
partially dry. Do not apply to seam area of flashing.
C. Flash penetrations and field-formed inside and outside corners with cured or uncured sheet flashing.
D. Clean seam areas, overlap, and firmly roll sheet flashings into the adhesive. Hot-air weld side and
end laps to ensure a watertight seam installation.

E. Terminate and seal top of sheet flashings and mechanically anchor to substrate through termination
bars.
3.09 INSTALLATION OF WALK PADS
A. Flexible Walkways: Install walkway products according to manufacturer's written instructions.
1. Install flexible walk pads as indicated on the Drawings.
2. Provide 6-inch (76-mm) clearance between adjoining pads.
3. Trim walk pads so that valleys along crickets are not obstructed.
4. Adhere walkway products to substrate with compatible adhesive according to roofing system
manufacturer's written instructions.
3.10 FIELD QUALITY ASSURANCE
A. The Construction Manager will engage a qualified testing agency to perform tests and to inspect
substrate conditions, surface preparation, roof membrane application, sheet flashings, protection,
and drainage components, and to furnish reports to the Owner and the Architect.
B. Low-Voltage Electrical Conductance Testing: The testing agency will survey the entire roof area and
flashings to locate discontinuity in the roof membrane using an exposed metal electrical loop to
create an electrical field tested with handheld probes or a scanning platform with integral perimeter
electrical loops creating a complete electrical field.
1. Testing will be performed before overlying construction is placed.
2. After testing, repair areas of discontinuities, repeat tests, and make further repairs, at no
additional cost to the Owner, until roofing and flashing installations are contiguous.
3. The testing agency will prepare survey report indicating locations of initial discontinuities, if any.
C. Arrange for the roofing manufacturer's technical representative to inspect roofing system installation
during construction at intervals appropriate to progress, but no less than five times for each roofing
system type. Include at a minimum start insulation, start of base sheet, start of cap sheet, and
completion of cap sheet.
D. Final Roof Inspection: Arrange for roofing system manufacturer's technical personnel to inspect
roofing installation on completion, in presence of Owner's representative, and to prepare inspection
report.
E. At Construction 's discretion perform field uplift testing in accordance with FM 1-52 /ASTM E907 to
validate uplift performance.
F. Repair, or remove and replace with new, components of roofing system where inspections indicate
that they do not comply with specified requirements.
G. Additional testing and inspecting, at Contractor's expense, will be performed to determine if replaced
or additional work complies with specified requirements.
3.11 PROTECTING AND CLEANING
A. Protect roofing system from damage discoloration, and wear during remainder of construction
period. When remaining construction does not affect or endanger roofing, inspect roofing system for
deterioration and damage, describing its nature and extent in a written report, with copies to
Architect and Owner.
B. Correct deficiencies in or remove roofing system that does not comply with requirements, repair
substrates, and repair or reinstall roofing system to a condition free of damage and deterioration at
time of Substantial Completion and according to warranty requirements.
C. Clean overspray and spillage from adjacent construction using cleaning agents and procedures
recommended in writing by manufacturer of affected construction.
D. Clean roof membrane as recommended in writing by manufacturer following construction to remove
discoloration that may impact SRI rating.

3.12 ROOFING INSTALLER'S WARRANTY
A. WHEREAS of herein
called the "Roofing Installer," has performed roofing and associated work("work") on the following
project:
1. Owner: Key West International Airport.
2. Address: 3491 S Roosevelt Blvd., Key West, Florida 33040.
3. Building Name/Type: EYW Concourse A.
4. Address: 3491 S Roosevelt Blvd., Key West, Florida 33040.
5. Area of Work: As indicated on Drawings.
6. Acceptance Date:
7. Warranty Period: Two years from date of Substantial Completion.
8. Expiration Date:
B. AND WHEREAS Roofing Installer has contracted (either directly with Owner or indirectly as a
subcontractor) to warrant said work against leaks and faulty or defective materials and workmanship
for designated Warranty Period,
C. NOW THEREFORE Roofing Installer hereby warrants, subject to terms and conditions herein set
forth, that during Warranty Period Roofing Installer will, at Roofing Installer's own cost and expense,
make or cause to be made such repairs to or replacements of said work as are necessary to correct
faulty and defective work and as are necessary to maintain said work in a watertight condition.
D. This Warranty is made subject to the following terms and conditions:
1. Specifically excluded from this Warranty are damages to work and other parts of the building,
and to building contents, caused by:
a. Lightning;
b. Peak gust wind speed exceeding mph (include wind speed rider);
c. Fire;
d. Failure of roofing system substrate, including cracking, settlement, excessive deflection,
deterioration, and decomposition;
e. Faulty construction of parapet walls, copings, chimneys, skylights, vents, equipment
supports, and other edge conditions and penetrations of the Work;
f. Vapor condensation on bottom of roofing; and
g. Activity on roofing by others, including construction contractors, maintenance personnel,
other persons, and animals, whether authorized or unauthorized by Owner.
2. When work has been damaged by any of foregoing causes, Warranty shall be null and void
until such damage has been repaired by Roofing Installer and until cost and expense thereof
have been paid by Owner or by another responsible party so designated.
3. Roofing Installer is responsible for damage to work covered by this Warranty but is not liable
for consequential damages to building or building contents resulting from leaks or faults or
defects of work.
4. During Warranty Period, if Owner allows alteration of work by anyone other than Roofing
Installer, including cutting, patching, and maintenance in connection with penetrations,

and void on date of said alterations, but only to the extent said alterations affect work covered
by this Warranty. If Owner engages Roofing Installer to perform said alterations, Warranty shall

Owner in writing, showing reasonable cause for claim, that said alterations would likely damage
or deteriorate work, thereby reasonably justifying a limitation or termination of this Warranty.
5. During Warranty Period, if original use of roof is changed and it becomes used for, but was not
originally specified for, a promenade, work deck, spray-cooled surface, flooded basin, or other
use or service more severe than originally specified, this Warranty shall become null and void
on date of said change, but only to the extent said change affects work covered by this
Warranty.

6. Owner shall promptly notify Roofing Installer of observed, known, or suspected leaks, defects,
or deterioration and shall afford reasonable opportunity for Roofing Installer to inspect work and

Please see the KWIA RFI 512 Attachment, indicating the existing conditions in the existing terminal first floor holdroom.The
items requiring clarification are listed below:
1. Non-load bearing masonry walls(noted as stud walls,structural columns to remain), please confirm this is to be
demolished.
2. Existing Artwork;Please confirm this is to be preserved.
3. Concrete beam at 8'-4"to bottom of beam.(New ceiling to be at 8'-4"AFF)Please advise if ceiling height shall be
lowered in this area.
4. LVT behind old gate counters not shown on demo plans,please confirm this is to be demolished.
5. Slab elevation differences at restau rant/hold room where kneewall was,please confirm concrete shall be chipped out as
the finished floor is to be flush here.
6. Area has ceramic tile flooring, not shown on demo plan, please confirm this is to be demolished.
7. 2 windows not in field,please confirm no further action required.
8. Right area has tile and LVT not shown on demo plan,please confirm this is to be demolished.
9. LVT not shown on demo plan, please confirm this is to be demolished.
10. Existing column is 4 1/2"thick.Wall is type SA3 to be installed in this area, please confirm if the wall shall be upsized to
conceal column in wall.

90% Specification 09 6500-3 3.02 notes that existing flooring and adhesive should be
removed prior to installation of resilient flooring. Demolition Plan General Notes #3 on
90% notes the removal of all items that interfere with final construction'.
5. Concrete finish floor should be flushed with top of concrete slab of surrounding area.
6. Tile should be demolished. 90% Specification 09 6500-3 3.02 notes that existing
flooring and adhesive should be removed prior to installation of resilient flooring.
7. As noted in keyed notes, exterior of the windows are covered in plywood, and interior
is furred out.
8. Tile and LVT should be demolished. 90% Specification 09 6500-3 3.02 notes that
existing flooring and adhesive should be removed prior to installation of resilient flooring.

SUBCONTRACT PRICE AND SCOPE OF WORK
2.1 SUBCONTRACT PRICE
The Subcontractor agrees to furnish all labor, materials, equipment, tools, supervision, technical
and professional services, transportation, hoisting, submittals, shop drawings, samples, insurance
and taxes necessary and incidentally required to fully perform the following work (the "Work")
and per the terms of this Agreement.
A. SUBCONTRACT PRICE.................................................................. $12,022.92

2.2 SCOPE OF WORK
This scope and description of Scope of Work is intended to be read in conjunction with all other
provisions of the Contract. It is not intended to limit, replace, supersede or otherwise amend other
descriptive terms of the Contract. It is intended to provide a more succinct, subcontractor-specific
summary of the details contained in the drawings, plans and specifications associated with the
Scope of Work to be performed under this Contract. The specific performance results intended
for the Subcontractor to achieve are those indicated in the Contract. The entire Scope of Work is
to be in accordance with the Contract in its entirety and shall include all labor, materials, services,
supervision, coordination, support, commissioning, inspections, permits, licenses, fees,
engineering, shop drawings, work plans, quality, safety, security, temporary utilities, taxes,
insurance, overhead, profit, and any other ancillary or collateral resources and activities necessary
to achieve proper performance and strict compliance with the terms of the Contract. The Scope of
Work includes, but is not limited to, the following:
A. General Scope Items
1. Visited the site and understands the constraints imposed by the logistics of the site.
2. Aware the Work Schedule for this Project is based upon a 5-day 8-hour workweek.
