This change order increases the final contract sum by$32,780.79. The substantial completion date is increased by Seventy-One (71) days. Monroe County Ordinance 027-1998 and subsequent amendments provide authority for the Administrator to approve change orders within specified limits after providing Board of County Commissioners with at least 24 hours to review them and pose any questions or objections that they may have. It is the Administrator's intent to approve the attached change order under that authority after the 24- hour BOCC notification time. These materials were not shown on the plans and was unforeseen. The compensation for labor and equipment to complete this task is shown below. These materials were not shown on the plans and were unforeseen. The removal of these materials resulted in additional time, labor, and equipment to complete this phase of work. PH 2 Sidewalk/Approach Slab/Moment Slab Revision Dear Mr. Briggs: WSP has received American Empire Builders, Inc (AEB) request for additional compensation due to unforeseen Delays. A. AEB was subject to delay and permit rejections on shipment of the Precast Beams for PH2 at Harbour Drive bridge. This delay was encountered from 1/10/2025 to 2/11/25. This denial of travel permit impacted the contractor's ability to continue work productive to their critical path. AEB is requesting 32 Contract Days of additional time from the delay of their schedule. B. AEB submitted RFI07 for clarification on the reinforcement of the sidewalk and PH2 moment/approach slabs for the PH3 temporary condition of the bridge and its TCP. A revision was needed to be made to the plans that affected the ability to move forward on the contractor's critical path. Because of this, the contractor's sequence and ability to move forward continuing PH2/3 construction of the bridge was impacted. AEB is requesting 27 Contract Days of additional time for complement of this task. The total amount of the work order is $0 and 59 Additional Contract Days. End Bent#1 Unforeseen Conditions (PH 3) Dear Mr. Briggs: WSP has received American Empire Builders, Inc (AEB) request for additional compensation due to unforeseen conditions at End Bents 91 &92 during Phase III of construction. A. AEB encountered large amounts of reinforced concrete,wooden pilings, and organic material that had been placed to support the existing bridge. This condition impacted the contractor's ability to stabilize the work area to remove the existing foundations to install the permanent work per plan in phase three construction of the bridge. AEB also encountered large amounts of reinforced concrete debris including concrete that had been pumped under the existing foundation to assist in stabilization of the existing bridge. This placed concrete and reinforced concrete debris was encountered during the initial demolition and impacted the contractor's sequence in phase three construction of the bridge. AEB is requesting 12 Contract Days and a $32,780.79 payment for labor and equipment to complete this task. The total amount of the work order is $32,780.79 and 12 Additional Contract Days.
