The following items have been executed and added to the record:
S 1 Ratification and approval of 2nd Amendment to the Alzheimer's Disease Initiative
(ADI) Contract Number KZ 2597 between the Alliance for Aging, Inc., (Alliance) and Monroe
County to increase funding by $68,928.00 to cover expenses for four(4) additional clients for the
contract period of 7/01/2025-6/30/2026. This amendment increases the contract total funding
from $86,160.00 to $155,088.00; and no match is required by the County; and correction of a
scrivener's error to revise the CFDA 4.
S2 Ratification and approval of 3rd Amendment to the Alzheimer's Disease Initiative
(ADI) Contract Number KZ 2597 between the Alliance for Aging, Inc., (Alliance) and Monroe
County to clarify insurance obligations between the parties during the contract period of
7/01/2025-6/30/2026.

Amendment 003 CONTRACT KZ 2597
. "This Amendment entered into between the Alliance for Aging, Inc. (Alliance) and Monroe County Board of
County. Commissioners, Social Services/In-Home Services (Provider), collectively. referred to as the "Parties"
hereby amends contract KZ-2597.
WHEREAS, any modification to the Contract shall be executed by the parties thereto; and
WHEREAS, the Parties wish to amend the Contract as set forth herein.
NOW THEREFORE,the Parties hereby amend the Contract as follows:
1. The first paragraph in Section 17.1 (page 6)is hereby replaced to read:
17.1 Provider must provide continuous adequate liability insurance and worker's compensation insurance

coverage,.during the existence of this contract, and duringanyrenewal(s) or extension(s) of it. In the

event there is a need.:to process any claims that may arise that the same protection will:be.afforded the

Alliance. Supporting documentation should be either a Certificate of Insurance naming the Alliance as
an additional insured on the provider's liability insurance policy (or policies) or if the Provider is self •
insured or a local municipality or county,and to be in compliance with and subject to the limitations of
Florida Statutes, Section 768..2.8.and Chapter 440, a signed letters from the municipality or county:stating
that it will indemnify the Alliance against any claims. The Provider accepts responsibility for
identifying and determining the ty.pe(s).and extent of liability insurance necessary to provide reasonable
financial protections.for the Provider and the clients to be served under this contract. Upon.execution of
this .Contract, the Provider shall furnish the. Alliance written verification. supporting both the

determination and existence of such insurance coverage. The limits of coverage under each policy
maintained by the Provider do not limit the Provider's liability and obligations under this contract The
•
Provider shall ensure that the Alliance has the most.current written verification of insurance coverage
throughout the term of this contract.. Such coverage may be •provided by a self-insurance 'program
ram
established and operating under the laws of the State of Florida:The Department and the Alliance reserves
• the right to require additional insurance as specified by this contract. . •
• This Amendment does not amend any other term or provision of the Contract or previous amendments to:the
Contract(except for any term or provision of the Contract that is inconsistent with the express amendments set forth •
in this Amendment),which shall continue to be in full force and effect.
This Amendment may be:executed in one or more counterparts,:any one'of which need not contain the signature
of more than one Party,but all such counterparts taken together will constitute one and the same instrument. •
