Change Order No. 5 includes the removal of abandoned structures from Airport property at the Hawk Missile Site. The
contract work currently being performed at this location inclludes the creation of wetlands in conformance with the US
Army Corps of Engineers(USAGE)and South Florida Water Management District (SFWMD) issued permit conditions.
Also located on this property is four abandoned structures. These structures are in disrepair and to prevent future
damage and safety concerns to the public, the airport has directed the contractor to demolish these structures and to
legally dispose of them off airport property.
This change order assumes the structures do not contain any hazardous materials, such as lead paint or asbestos. This
change order does include testing for both lead paint and asbestos. If the testing notes a presence of lead paint or
asbestos the contractor is entitled to adjust the fee to remove these structures.
Net change to contract Change Order 6: $132,163.28
This Change Order 5 does not increase contract time.
Contractor agrees to accept the increase in monetary compensation and hereby waives and releases any and all claims for
any additional compensation related to these bid items,
This Change Order 5 represents 1.8% of the original contract price.

Dear Mr. Pfahl:
Gallagher Bassett Technical Services (GBTS), a division of Gallagher Bassett Services, Inc., is pleased to submit this
proposal to Toppino and Sons, Inc. for an asbestos pre-demolition inspection and report of the four abandoned
structure son the north side of KWIA.
BACKGROUND
GBTS has prepared a proposal to conduct a pre-demolition asbestos survey of the subject interior/exterior/roof
finishes(subject areas)of the four abandoned structures as per the provided plans to satisfy local Monroe County
pre-demolition permitting requirements.
SCOPE
Task 1—Asbestos Inspection
GBTS asbestos inspector will conduct a walk-through of the areas to visually assess suspect friable and nonfriable
asbestos-containing materials (ACM) for condition, homogenous areas, and functional spaces. Upon receipt of
notice to proceed, GBTS will collect up to 40 samples/layers total from the interior/exterior/roofs subject
structures and transport the samples to a NVLAP-qualified asbestos laboratory for analysis.
Task 2—Bulk Laboratory Analysis
Bulk samples will be analyzed for asbestos content by polarized light microscopy (PLM) at GBTS's in-house
asbestos laboratory. The laboratory analysis report will be included in the final asbestos survey report. GBTS
anticipates that 40 samples will be adequate to complete the survey of the site. If needed, as per United States
Environmental Protection Agency (USEPA) regulations, any friable materials found to contain less than 10%
asbestos by visual estimation, may require analysis by the objective Point Count Method (PCM) for confirmation
of asbestos content. GBTS recommends only samples that are less than 1% asbestos (1 sample out of 3) for this
analysis.This analysis may be performed for an additional $40 per sample at the request of the Client.Any unused
sample fees from unanalyzed samples may be applied to any Point Counts Method required analysis.

Task 3—Final Report
GBTS will prepare one final written report containing our findings, and recommendations, based upon the results
of the inspection. Field diagrams and/or photographs will be provided in the report. The report will be reviewed
and signed by a Licensed Asbestos Consultant. For an additional fee, GBTS can provide additional consulting
services to support the removal of identified friable ACM, nonfriable ACM, including bid assistance, contractor
selection, and final air clearance monitoring.
LIMITATIONS AND ASSUMPTIONS
The following limitations and assumptions apply to this proposal:
• Our fee is based on the collection and analysis of up to maximum 40 samples/layers total from the four
structures.
• Some building materials may not be accessible or exposed due to renovation/modification activities that have
occurred since the original building construction. GBTS will not be held responsible for sampling, analysis, and
subsequent identification of ACBMs that are not "reasonably accessible". For the purpose of this proposal,
"reasonably accessible" includes, but is not limited to, "accessible without destruction of floors,walls,or hard
ceilings, capable of sampling with hand tools, and reachable with a 16' extension ladder."
• Any large scale destruction of walls or finishes in search of ACBMs will only be conducted by GBTS at the
direction of Client, and only after written permission is granted.
FEE
GBTS's proposed fees for completion of Tasks 1-3 will be a Fixed Fee of$3,500.00, based on the following:
Task 1—Asbestos Inspection (Techs/truck/lodging/per diem)....................................................................$2,450.00
Task 2—Bulk Sample Analysis @$15 .............................................................................. up to 40 samples—$600.00

GBTS's total estimated fee for completion of the proposed work is NOT TO EXCEED $3,500.00. This fee estimate
is expected to be sufficient to accomplish the Statement of Services described herein; however, no guarantee is
made or implied.This fee assumes that GBTS will have complete access to survey the site in one single visit. This
fee also assumes that each suspect material will be sampled and analyzed, and that a maximum 40 PLM samples
will be sufficient to adequately survey the interior and exterior/roof areas.
Payments are due 30 days from receipt of invoice. Payment is the sole responsibility of Charley Toppino and Sons,
Inc. and is not contingent upon third party funding, transactional closing, or insurance recovery.
