Change Order No. 7 is issued to incorporate additional costs associated with asbestos abatement identified after execution
of Change Order No. 5. At the time of Change Order No. 5, asbestos testing had not yet been completed; thenafora,
provisions were included allowing for a future adjustment should asbestos-containing mabahe|s (ACM) be confirmed.
Subsequent testing results confirmed the presence of asbestos within the project area, See attached for detailed findings
from the Contractor.
In accordance with these findings, the Contractor obtained a proposal from a licensed environmental subcontractor for the
removal and proper disposal of identified ACM, including approximately 300 square feet of asbestos-containing floor tile.
This Change Order includes the Contractor's costs for asbestos abetement, associated overhead and profit, and related
project costs as supported by the submitted and reviewed quotations, totaling $8,430.24.
This adjustment is necessary to address unforeseen hazardous materials encountered during the work and to ensure
compliance with applicable environmental regulations and project requirements.
Net change to contract Change Order 7: $8.430.24
This Change Order 7 does not increase contract time.
Contractor agrees to accept the increase in monetary compensation and hereby waives and na|aames any and all claims for
any additional compensation related tothese bid items.

PROPOSED SCOPE OF WORK: Simpson Environmental Services, LLC dba EIS—Environmental Infrastructure Solutions ("EIS")
hereby proposes to:
Remove and dispose of a total of approximately 300 square feet of asbestos-containing floor tile located in the office area of shed#1.
NOTES TO THE SCOPE OF WORK—This Proposal is subject to and provided based upon the following assumptions:
1. This proposal is based on the premise Charley Toppino and Son, Inc. (Toppino)will provide all necessary water
and power for the project.
2. Simpson's proposal is based on completing the work in one mobilization.
3. Simpson's offer does not include any provisions for any Davis Bacon, Florida Prevailing wages, or overtime.
4. This proposal is for the removal of accessible ACM only. Simpson's proposal does not include any demolition or
disassembly which may be necessary to gain access to ACM that is trapped, concealed, or otherwise inaccessible.
5. This offer is based on the premise that the structure is sound and does not pose a safety hazard to Simpson
employees.
6. This proposal is provided on the premise that the structure is to be demolished following completion of the
abatement work, therefore, Simpson's proposal does not include any protection, temporarily or otherwise, of the
building or its contents following removal of the ACM.
7. Simpson's offer is based on being provided access to park both a truck and trailer proximal to the structure.

8. With regards to the movable objects, Simpson's proposal includes provisions for miscellaneous trash to be
relocated but does not include disposal.
9. Simpson will utilize the most expedient method of tile method without regards to the structured based on the
premise that it is schedule for demolition upon completion of the work.
10. Simpson's offer does not include any weather or security protection.
11. Simpson's proposal includes air monitoring for contractor's OSHA compliance only. Toppino shall be responsible
for all other air monitoring. Toppino's air monitor/consultant shall provide Simpson with copies of all air monitoring
data generated on this project at no additional charge to Simpson.
12. Simpson will dispose of all ACM waste generated on this project at a landfill authorized to accept asbestos wastes;
Owner will receive a waste manifest(within a few weeks of the projects' completion)to evidence the proper
disposal of the ACM generated on this project.
13. All asbestos work will be completed using all federal, state, local regulations as well as Simpson's standard
operating procedures.
EXCLUSIONS—This Proposal is subject to and provided based upon the following exclusions:
1. Daily air monitoring and final clearances.
2. Badging and Airport Security Check
3. Performing this work under a specification
DOCUMENTS INCORPORATED BY REFERENCE, if any:
NESHAP Pre-Demolition Survey performed by Gallagher Bassett(GBTS Project:26007-0050)and dated May 8, 2026,
PROPOSED DURATION OF WORK:
Simpson anticipates a complete project duration of approximately two working days.
CONTRACT PRICE—"EIS" proposes to provide the above-described Scope of Work for the sum of:

Additional Terms
Unless noted otherwise above,the following items apply:"EIS",the Contractor,proposes to provide supplies,materials,equipment,documentation,environmental
insurance, labor, supervision, and disposal to complete this project. State notification fees are excluded for this proposal, as well as third party consulting fees.
The contractor requires the following to be provided by the Owner or Client:access,electricity,water,third party consulting services,location for equipment/supply
storage/trailer, and parking,as needed. Bonds and applicable sales and use taxes are not included in pricing, unless specifically noted above. Performance and
Payment Bonds can be added at a rate of 2.5%.A mobilization fee equal to 10%of the total contract value will be invoiced at project start to cover initial project
setup, materials,staffing, and logistics.All invoices issued under this agreement, including mobilization, progress, and final invoices, are subject to the payment
terms stated herein.Payment is due thirty(30)days from submission of invoice.If payment is not received in full within thirty(30)days of the due date,Contractor
reserves the right to file a Mechanic's Lien or any other legal claim against the property, in accordance with state lien laws.In the event payment is not made as
required herein,the Contractor is entitled to recover all costs of collection, including, but not limited to, reasonable attorney fees.The obligation of payment to
the Contractor is independent of the Prime/Owner payment to the Client.The Client remains responsible for full payment within the agreed upon terms
regardless of any delay or non-payment from the Prime/Owner.A 3%processing fee will apply to credit card payments.The Client and Contractor may agree
to changes in the Scope of Work.Any modifications to the Scope of Work must be approved in writing by both Parties.Any alterations or deviation from the above
specifications involving extra costs will be executed only upon written orders and will become an extra charge over and above the estimate. Any claim for
damages or back charges, including but not limited to those arising from alleged delays, defects, or deficiencies in performance, must be submitted to the
Contractor in writing within five(5)business days of the occurrence giving rise to the claim. Failure to provide written notice within this period shall constitute a
waiver of such claims.No back charges shall be accepted without prior written approval from the Contractor.Any retainage will be paid within thirty(30)days of
completion of the Scope of Work as defined above. Owner must carry fire, tornado, or other necessary loss insurance. The Contractor is not the generator of
waste—Owner is the generator of waste and hereby gives the Contractor authority to sign waste disposal documents on Owner's behalf. Owner also gives the
Contractor the authority to sign asbestos/renovation/demolition notifications on behalf of the owner.To the extent this proposal is incorporated by reference into
or conflicts with other project or contract documents,this agreement will take precedence.
This Proposal shall be incorporated into any final agreement signed by the parties in furtherance of performing the work contemplated herein. If any
provision of the final agreement entered into by the parties conflicts with any provision set forth in this Proposal,the provision of this Proposal shall
control and prevail.
