RESOLUTION NO. 022-1986
A RESOLUTION AUTHORIZING THE MAYOR AND
CHAIRMAN OF THE BOARD OF COUNTY COMMIS-
SIONERS OF MONROE COUNTY, FLORIDA, TO APPROVE
AND EXECUTE AN AGREEMENT BY AND BETWEEN THE
BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, AND THE GUIDANCE CLINIC OF
THE MIDDLE KEYS, INC., IN CONNECTION WITH
FUNDING FOR MENTAL HEALTH CARE SERVICES AND
BAKER ACT TRANSPORTATION FOR THE CITIZENS OF
THE MIDDLE KEYS, MONROE COUNTY, FLQRIDA.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, as follows:
That the Mayor and Chairman of the Board of County Commis-
sioners of Monroe County, Florida, is hereby authorized to
approve and execute an Agreement by and between the Board of
County Commissioners of Monroe County, Florida, and the Guidance
Clinic of the Middle Keys, Inc., a copy of same being attached
hereto, in connection with funding for mental health care
services and Baker Act transportation for the citizens of the
Middle Keys, Monroe County, Florida.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held
on the 24th day of January, A.D. 1986.

A G R E E MEN T
WHEREAS, the BOARD OF COUNTY COMMISSIONERS OF MONROE COUNTY,
FLORIDA, and the GUIDANCE CLINIC OF THE MIDDLE KEYS, INC. desire
to enter into an agreement wherein the Board of County Commis-
sioners contracts for services from said Clinic for the rendering
of mental health services to the citizens of the Middle Keys,
Monroe County, Florida, and
WHEREAS, the Board of County Commissioners of Monroe County,
Florida is vested and charged with certain duties and
responsibilities relating to the mental health and guidance of
the citizens of Monroe County, and
WHEREAS, such services have been invaluable to the citizens
of the Middle Keys, Monroe County, Florida, and
WHEREAS, it is proper and fitting to enter into an agreement
for services to be rendered in the forthcoming year 1985-86; now,
therefore,
THIS AGREEMENT made between the BOARD OF COUNTY COMMIS-
SIONERS OF MONROE COUNTY, FLORIDA, and the GUIDANCE CLINIC OF THE
MIDDLE KEYS, INC. each in the consideration of the premises,
agree as follows:
The said Board of County Commissioners of Monroe County,
Florida, in consideration of said Guidance Clinic of the Middle
Keys, Inc. substantially and satisfactorily performing and
carrying out the duties and obligations of the Board as to
rendering counsel to the citizens of the Middle Keys, Monroe
County, Florida, in matters of mental health and guidance, and
providing transportation to treatment facilities as required by
Florida Statute 394.459 for Monroe County patients, agrees to:
1. Pay to the said GUIDANCE CLINIC OF THE MIDDLE KEYS, INC.
the sum of Twenty Three Thousand Six Hundred Eighteen Dollars
($23,618.00) for the fiscal year 1985-86.
2. Pay an additional sum not to exceed Twenty Four Thousand
Four Hundred Five Dollars ($24,405.00) for the providing of
transportation of patients in Monroe County to treatment
facilities.
3. Pay an additional sum not to exceed Fifty Nine Thousand
One Hundred Twenty Seven Dollars ($59,127.00) for the providing
of services for the MARC House in Marathon, as provided for under
separate agreement.
These figures will be paid monthly commencing October 1,
1985, as hereinafter set forth.
And the said Guidance Clinic of the Middle Keys, Inc. for
the consideration named, covenants and agrees with the Board of
County Commissioners of Monroe County, Florida, to substantially
and satisfactorily perform and carry out the duties of the said
Board in rendering counsel in the matters of mental health and
guidance to the citizens of the Middle Keys, Monroe County,
Florida, and transporting patients in Monroe County to treatment
facilities in accordance with Florida Statute 394.459.
In further consideration of said financial remuneration, the
said Guidance Clinic of the Middle Keys, Inc. agrees to provide a
clear financial audit trail to allow for full accountability of
funds received from said Board. Certified monthly financial and
service load reports will be made available to the Board of
County Commissioners of Monroe County, Florida, to validate the
delivery of services under this contract. The monthly financial
report is due in the office of the Clerk of the Board of County
Commissioners of Monroe County, Florida, no later than the 15th
day of the following month. After the Clerk of the Board
pre-audits the certified report, the Board will reimburse said
Guidance Clinic of the Middle Keys, Inc. for its monthly
expense.s. However, the total of said monthly payments in the
aggregate sum shall not exceed the total amount of $23,618.00 for
counseling, $24,405.00 for transportation cost and $59,127.00 for
costs of the MARC House during the term of this contract.
IN WITNESS WHEREOF, the Board of County Commissioners of
Monroe County, Florida, has caused these presents to be signed in
its name by its Chairman, Board of County Commissioners, and the

presents to be signed in its name by its Clinic Director and
President of the Board, all as of the ____ day of
