RESOLUTION NO. 344 -1987
A RESOLUTION OF THE BOARD OF COUNTY COMMIS-
SIONERS OF MONROE COUNTY, FLORIDA, AUTHORIZ-
ING THE MAYOR/CHAIRMAN OF THE BOARD TO
EXECUTE AN ADDENDUM TO AGREEMENT BY AND
BETWEEN THE BOARD AND THE GUIDANCE CLINIC OF
THE MIDDLE KEYS, INC. CONCERNING ADDITIONAL
FUNDING FOR BAKER ACT TRANSPORTATION SERVICES
FOR FISCAL YEAR 1986-87.
WHEREAS, at a regular meeting held on the 4th day of August,
1987, the Board of County Commissioners of Monroe County,
Florida, authorized the transfer of $1,800 from Contingency funds
to the Baker Act Transportation Budget for the Guidance Clinic of
the Middle Keys, Inc., and
WHEREAS,
in accordance with said additional funding
mentioned above, an Addendum to Agreement was prepared in order
to reflect said additional funding for Baker Act Transportation
Services, now, therefore,
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, that the Mayor/Chairman of the Board of
County Commissioners of Monroe County, Florida, is hereby
authorized to execute an Addendum to Agreement by and between the
Board and the Guidance Clinic of the Middle Keys, Inc., a copy of
same being attached hereto, concerning additional funding for
Baker Act Transportation Services for fiscal year 1986-87.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held
on the 15th day of September, A.D. 1987.

ADDENDUM TO AGREEMENT FOR
BAKER ACT TRANSPORTATION FUNDS
THIS ADDENDUM, made this
day of

by and between the BOARD OF COUNTY COMMISSIONERS OF MONROE,

FLORIDA, hereinafter referred to as "BOARD", and the GUIDANCE
CLINIC OF THE MIDDLE KEYS, INC., hereinafter referred to as
"CLINIC".
For and in consideration of the provisions of transportation
to treatment facilities as required by Florida Statutes, ~394.459
for Monroe County patients, the parties hereby agree as follows:
(1) To amend paragraph 2. of its Agreement entered into on
January 7, 1987, a copy attached hereto, by adding to the amount
payable by the BOARD a sum not to exceed $1,800 for providing
transportation of patients in Monroe County to treatment
facilities, for a total of $34,968.00.
IN WITNESS WHEREOF, the BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, has caused these presents to be signed in
its name by its Chairman and Clerk of the Board of County
Commissioners, and the GUIDANCE CLINIC OF THE MIDDLE KEYS, INC.,
has caused these presents to be signed in its name by its Clinic
Director and President of the Board, all as hereinabove set
forth.
