RESOLUTION NO. 415-1987
A RESOLUTION OF THE BOARD OF COUNTY COMMIS-
SIONERS OF MONROE COUNTY, FLORIDA, AUTHO-
RIZING THE MAYOR/CHAIRMAN OF THE BOARD TO
EXECUTE AN AGREEMENT BY AND BETWEEN THE BOARD
OF COUNTY COMMISSIONERS OF MONROE COUNTY,
FLORIDA, AND FREELAND WILLIAMS, M. D., CON-
CERNING PHYSICALS FOR PROSPECTIVE AND CURRENT
EMPLOYEES.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, that said Board hereby authorizes the
Mayor/Chairman of the Board to execute an Agreement by and
between the Board of County Commissioners of Monroe County,
Florida, and Freeland Williams, M. D., a copy of same being
attached hereto, concerning physicals for prospective and current
employees.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held
on the 3rd day of November, A.D. 1987.

A G R E E MEN T
THIS AGREEMENT entered into this
day of
A.D. 1987, by and between the BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, party of the first part, and FREELAND
WILLIAMS, M.D., party of the second part, whose mailing address
is Southern Medical Group, 1111 12th Street, Key West, Florida
33040.
WITNESSETH, that the party of the second part, for and
in consideration of the sum of $30.00 per patient, payable upon
receipt of invoice, agrees to furnish pre-employment physicals
for prospective employees and any physicals deemed necessary for
current employees of the party of the first part.
It is hereby further agreed by and between the parties
hereto that either party may cancel this agreement by giving
thirty (30) days notice in writing.
