A RESOLUTION OF THE BOARD OF COUNTY COMMIS-
SIONERS OF MONROE COUNTY, FLORIDA, AUTHORIZ-
ING THE MAYOR/CHAIRMAN OF THE BOARD TO
EXECUTE AMENDMENT 12 OF THE TITLE III C-1
CONTRACT BETWEEN MONROE COUNTY AND THE AREA
AGENCY ON AGING FOR DADE AND MONROE COUNTIES/
UNITED WAY OF DADE COUNTY, INC., CONCERNING
THE MONROE COUNTY SOCIAL SERVICES NUTRITION
PROGRAM.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, that the Mayor/Chairman of the Board is
hereby authorized to execute Amendment 12 of the Title III C-1
Contract between Monroe County and the Area Agency on Aging for
Dade and Monroe Counties/United Way of Dade County, Inc" a copy
of same being attached hereto, concerning the Monroe County
Social Services Nutrition Program.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held
on the 1st day of November, A.D. 1988.

AMENDMENT # 2' TO CONTRACT
THIS AGREEMENT, entered into on this 26th day of September, 1988 by
and between Monroe County (Social Services - Nutrition Proszram)
hereinafter referred to as the "Provider," and the Area Agency on
Aging for Dade and Monroe Counties/United Way of Dade County, Inc.
WITNESSETH:
The above-nallled parties hereby Nutually agree:

That the contract between the said parties on

aRount of $157.566 are to be considered an
attachRent to this aIIIendlllent.
II. The terlllination date reNains Decelllber 31. 1999 .
III. The provision of service will reNain the sallie throughout the
period.
IV. The attached contract entered into on January 1. 1999. is,
in fact, the contractural agreelllent between the Provider and
the Area Agency and this is the agreeNent under which paYlllent
has been taking place.
V. Only itellls in the contract or attachlllents to the contract in
conflict with this aIIlendJl'lent to contract shall be and are
hereby changed to conforlll with the contract.
VI. A. The Provider agrees to hire Project Independence
participants in a lIliniNUIIl of" five percent of all entry-
level openings, which require a high school education or
less and which becollle available during the course of this
contract. Project Independence participants are defined
as persons or household lIleNbers receiving Public
Assistance in the forN of" AFDC or food stalllSpS, or
households applying for AFDC. These participants will be
identified (or verified) to the Provider by the
DepartNent. Entry-level jobs ara defined as those jobs
requiring education at or below high school level.

B. The Provider agrees to subNit a quarterly report showing:
total n~ber or entry-level vacancies filled during the
quarter and year to date; total n~ber of Project
Independence clients hired during the quarter and year to
dale; and such other inforNalion as the DepartNent Nay
deterNine necessary.
IN WITNESS THEREOF, the parties hereto have caused this
agreeNent to be executed by their officials thereunto duly
authorized.
