RESOLUTION NO. 555-1988
A RESOLUTION OF THE BOARD OF COUNTY COMMIS-
SIONERS OF MONROE COUNTY, FLORIDA, AUTHORIZ-
ING THE MAYOR/CHAIRMAN OF THE BOARD TO
EXECUTE AMENDMENT 12 OF THE TITLE III C-2
CONTRACT BETWEEN MONROE COUNTY AND THE AREA
AGENCY ON AGING FOR DADE AND MONROE COUNTIES/
UNITED WAY OF DADE COUNTY, INC., CONCERNING
THE MONROE COUNTY SOCIAL SERVICES NUTRITION
PROGRAM.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, that the Mayor/Chairman of the Board is
hereby authorized to execute Amendment 12 of the Title III C-2
Contract between Monroe County and the Area Agency on Aging for
Dade and Monroe Counties/United Way of Dade County, Inc., a copy
of same being attached hereto, concerning the Monroe County
Social Services Nutrition Program.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held
on the 1st day of November, A.D. 1988.

AMENDMENT #2 TO CONTRACT
THIS AGREEMENT, entered into on tbis 26tb day of Septe~ber, 1988, by

bereinafter referred to as tbe "Provider ,n and tbe Area Agency on
Aging for Dade and Monroe Counties/United Way of Dade County, Inc.
WITNESSETH:
Tbe above-naRed parties bereby ~utually agree:
I. That the contract between the said parties on lanuary 1, 1988,
covering tbe period fro~ 1/1/88 to 12/31)88 for Title III C-2
of the Older ~ericans Act.
A. Tbe total aRount of funds is increased as follows:
Original $ 60.043

For a Total of $ 64,582

B. A Special Provision is added as follows:
The original contract plus ~endRent 1, in the ~ount of
$61.900 are to be considered an attacbNent to
this aIIlendAent.
II. Tbe terRination date re~ains DeceMber 31. 1988.
III. The provision of service will re~ain the s~e throughout the
period.
IV. Tbe attacbed contract entered into on lanuary 1. 1988. is,
in fact, the contractural agree~ent between tbe Provider and
tbe Area Agency and tbis is tbe agree~ent under wbicb pa~ent
has been taking place.
V. Only iteNs in tbe contract or attachnents to the contract in
conflict with this ~endJqent to contract shall be and are
bereby changed to conforM with the contract.
VI. A. The Provider agrees to hire Project Independence.
participants in a MiniMUR of five percent of all entry-
level openings, which require a high school education or
less and which be~oMe available during tbe course of tbis
contract. Project Independence participants are defined
as persons or bousebold MeMbers receiving Public
Assistance in the forM of AFDC or food staMsps, or
bousebolds applying for AFDC. Tbese participants will be
identified (or verified) to the Provider by the
DepartMent. Entry-level jobs are defined as tbose jobs
requiring education at or below bigh scbool level.

B. The Provider agrees to sub~it a quarterly report showing:
total nUMber o~ entry-level vacancies ~illed during the
quarter and year to date; total nUNber of Project
Independence clients hired during the quarter and year to
date; and such other in~or~tion as the Depart~ent Ray
deter~ine necessary.
IN WITNESS THEREOF, the parties hereto have caused this
agreeRent to be executed by their o~~icials thereunto duly
authorized.
