RESOLUTION NO. 615 -1988
A RESOLUTION OF THE BOARD OF COUNTY COMMIS-
SIONERS OF MONROE COUNTY, FLORIDA, AUTHORIZ-
ING THE MAYOR/CHAIRMAN OF THE BOARD TO
EXECUTE AN AGREEMENT BETWEEN THE BOARD OF
COUNTY COMMISSIONERS OF MONROE COUNTY,
FLORIDA, AND THE GUIDANCE CLINIC OF THE
MIDDLE KEYS, INC. CONCERNING FUNDING FOR
FY88-89.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, that the Mayor/Chairman of the Board is
hereby authorized to execute an Agreement between the Board of
County Commissioners of Monroe County, Florida, and the Guidance
Clinic of the Middle Keys, Inc., a copy of same being attached
hereto, concerning funding for FY88-89.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held
on the~ day of November, A.D. 1988.

AGREEMENT
WHEREAS, the BOARD OF COUNTY COMMISSIONERS OF MONROE COUNTY,
FLORIDA, and the GUIDANCE CLINIC OF THE MIDDLE KEYS, INC. wish to
enter into an agreement wherein the Board contracts for services
from the Clinic for the rendering of mental health services to
the citizens of the Middle Keys, Monroe County, Florida, and
WHEREAS, the Board is vested and charged with certain duties
relating to the mental health of the citizens of Monroe County,
and
WHEREAS, such services have been invaluable to the citizens
of the Middle Keys, and
WHEREAS, it is appropriate to enter into an agreement for
services to be rendered in the forthcoming year 1988-89; now,
therefore,
The Board and the Clinic, each in the consideration of the
premises, agree as follows:
1. The Board, in consideration of the Clinic performing
the duties of the Board as to rendering mental health counsel to
the citizens of the Middle Keys and providing transportation to
treatment facilities as required by F.S. 394.459 for Monroe
County patients shall pay to the Clinic the following amounts for
the fiscal year beginning October 1, 1988 and ending September
30, 1989:
(a) $32,226.00 for mental health counseling services
rendered by the Clinic,
(b) not more than $37,386.00 for providing transportation
of patients in Monroe County to treatment facilities, and
(c) not more than $76,642.00 for providing services to the
MARC House in Marathon, as provided for under separate agreement.
2. The Clinic, for the consideration named, shall perform
the duties of the Board in rendering mental health counsel to the
citizens of the Middle Keys and in transporting patients in
Monroe County to treatment facilities pursuant to F.S. 394.459.

3. The Clinic shall also provide a clear financial audit
trail to allow for full accountability of funds received from the
Board. In order to validate the delivery of services under this
contract, certified monthly financial and service load reports
shall be made available to the Board. The monthly financial
report is due in the office of the Clerk of the Board no later
than the 15th day of the following month. After the Clerk of the
Board pre-audits the certified report, the Board shall reimburse
the Clinic for its monthly expenses. However, the monthly
payments shall not exceed in the aggregate the total amount of
$32,226.00 for counseling, $37,386.00 for transportation and
$76,642.00 for the MARC House during the term of this contract.
IN WITNESS WHEREOF, the Board has caused these presents to
be signed in its name by its Chairman, and the Clinic has caused
these presents to be signed in its name by its Clinic Director
and President of the Board, all as of the
