RESOLUTION NO. 325 -1986
A RESOLUTION AUTHORIZING THE MAYOR AND
CHAIRMAN OF THE BOARD OF COUNTY COMMISSIONERS
OF MONROE COUNTY, FLORIDA, TO EXECUTE AN
AGREEMENT BY AND BETWEEN THE BOARD OF COUNTY
COMMISSIONERS OF MONROE COUNTY, FLORIDA, AND
AVA STEGALL CONCERNING PRE-EMPLOYMENT
PHYSICALS FOR PROSPECTIVE EMPLOYEES AND
PHYSICALS DEEMED NECESSARY FOR CURRENT
EMPLOYEES.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, as follows:
That the Mayor and Chairman of the Board of County Commis-
sioners of Monroe County, Florida, is hereby authorized to
execute an Agreement by and between the Board of County Commis-
sioners of Monroe County, Florida, and. Ava Stegall, a copy of
same being attached hereto, concerning pre-employment physicals
for prospective employees and physicals deemed necessary for
current employees.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held

AGREEMENT
THIS AGREEMENT made this
day of
A.D. 1986, by and between the BOARD OF COUNTY COMMISSIONERS OF

MONROE COUNTY, FLORIDA, party of the first part, and AVA ~EGALL,
D.O., party of the second part, whose mailing address is
88583 Overseas Highway, Tavernier, Florida 33070.
WITNESSETH, that the party of the second part, for and in
consideration of the sum of $25.00 per patient, payable upon
receipt of invoice, agrees to furnish pre-employment physicals
for prospective employees and any physicals deemed necessary for
current employees of the party of the first part.
It is hereby further agreed by and between the parties
hereto that either party may cancel this agreement by giving
thirty (30) days notice in writing.
IN WITNESS WHEREOF, the parties hereto have executed this
agreement the day and year first above written.
