A RESOLUTION AUTHORIZING THE CHAIRMAN OF THE
BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, TO APPROVE AND EXECUTE
ADDENDUM NO. 6 TO THE AUGUST 5, 1983, BASIC
SERVICES AGREEMENT BY AND BETWEEN POST,
BUCKLEY, SCHUH & JERNIGAN, INC. AND THE BOARD
OF COUNTY COMMISSION OF MONROE COUNTY,
FLORIDA.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, that the Chairman of the Board of County
Commissioners of Monroe County, Florida, is hereby authorized to
approve and execute Addendum No. 6 to the August 5, 1983, Basic
Services Agreement by and between Post, Buckley, Schuh &
Jernigan, Inc. and the Board of County Commissioners of Monroe
County, Florida, a copy of same being attached hereto,
concerning additional services to be provided by and additional
compensation to be paid to Post, Buckley, Schuh & Jernigan, Inc.
PASSED AND ADOPTED by the Chairman of the Board of County
Commissioners of Monroe County, Florida, at a regular meeting of
said Board held on the 8th day of February, A.D. 1985.

THIS ADDENDUM to the Agreement dated Au ust 5, 1983 between Post, Buckley, Schuh &:
Jernigan, Inc., (PBS&:J) and the Client identified her in i'i for the Additional Services described in
Item 5 of this Addendum. This document constitutes dendum No.6 to the August 5, 1983 Basic
Services Agreement, A.I.A. Document B 141 Standard OI'ill-Agreement (Lump Sum Contract of
$100,000.00).

4. PBS&:J shall begin work promptly on the requested Additional Services, however, completion
of the services may, at PBS&:J's option, be delayed until receipt of a fully executed copy of
this Addendum. (Client retain one copy, return remaining copies).
5. Description of Additional Services to be provided by PBS&:J (attach additional pages, if
necessary):
The A/E Design Team (PBS&J, Inc. with Wolfberg, Alvarez and Associates and Charles
McCoy and Associates as professional consultants) will prepare Construction Documents,
consisting of working drawings, specifications and bid documents, for construction of a
Criminal Justice Facility Building: Jackson Square Master Plan: Modified Phase II;
(approximately 7,500 to 8,500 S.F. building with parking provided on the first level)
consistent with the Master Plan of Jackson Square, as shown in the amended Monroe County
Jail/Government Facility Planning Study, incorporating the requirements and regulations of
the Old Island Restoration Commission.
The A/E Design Team will provide necessary working drawings for the site work, on-site
utility extensions and -landscaping. Demolition of existing structures and necessary soils
testing will be provided by the Owner. A survey of the U.S.O. Property will be provided to
the A/E Design Team by the Owner.
The A/E Design Team will also perform construction phase services, i.e., contract
administration, construction observation and final certification. The proposed
improvements should meet the requirements of Chapter 84-350 of the Laws of Florida for
disbursement of additional 1 % sales tax funds.
The A/E Design Team will complete the Preliminary and Final Design Construction
Documents within 90 days. Construction phase services will be rendered for a 270 day
constructed period only.
6. The compensation to be paid PBS&:J for providing the requested services shall be:

Direct personnel expense plus a surcharge of 150%, plus reimbursable costs.

A Lump-Sum charge of $ 85,000.00 , eKcluding out-of-town meetings, travel
and printing which is reimbursable as outlined in the Basic Services Agreement
dated August 5, 1983 between PBS&J and Monroe County.
Unit Cost/Time Charges identified in Exhibit A, attached.
In accordance with the provisions for Additional Services compensation set
forth in the aforementioned Agreement.
* Schematic Design Phase (15%); Design Development Phase (20%); Construction Documents
Phase (40%); Bidding/Negotiating Phase (5%); and Construction Phase (20%) of the Lump
Sum. .
IN WITNESS WHEREOF, this Addendum is accepted this day of
subject to the terms and conditions of the aforementioned Agreement.
