RESOLUTION NO. 206 -1984
RESOLUTION AUTHORIZING THE CHAIRMAN OF THE
BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, TO EXECUTE THE ADDENDUM TO
AGREEMENT FOR BAKER ACT MATCHING FUNDS BY AND
BETWEEN THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, AND THE GUIDANCE
CLINIC OF THE MIDDLE KEYS, INC.
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, as follows:
That the Mayor and Chairman of the Board of County Commis-
sioners of Monroe County, Florida, is hereby authorized to
execute the addendum to agreement for Baker Act matching funds by
and between the Board of County Commissioners of Monroe County,
Florida, and the Guidance Clinic of the Middle Keys, Inc., a copy
of same being attached hereto.
PASSED AND ADOPTED by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting of said Board held
on the 27th day of July, A.D. 1984.

ADDENDUM TO AGREEMENT FOR BAKER ACT MATCHING FUNDS
THIS ADDENDUM, entered into this
corrects paragraph one (1) of that certain agreement entered into
the lst day of October, 1983, by and between the Board of County
Commissioners of Monroe County, Florida, (Board), and the
Gu~dance Clinic of the Middle Keys, Inc. (Clinic) wherein the
'total reimbursement cost was incorrectly typed as "Twenty-four

thousand, four eighty dollars, and twenty-five cents
($23,484.25)". Said paragraph one of said agreement is hereby
amended to read as follows:
percent (25%) of total Clinic expenditures for Baker Act

hospital, physician, and crisis stabilization services, as billed
by the Clinic on state approved Baker Act Billing Forms (HRS 346)
for clients qualifying for such services under applicable state
and federal regulations and eligibility determination procedures.
This cost is not to exceed a total reimbursement of Twenty-four
Thousand, Four Hundred eighty-four dollars and twenty-five cents
($24,484.25) during the period beginning October I, 1983 and
en'di,ng September 30, 1984."
All other provisions of said agreement are to remain in full

force and effect.
IN WITNESS WHEREOF, the parties hereto have caused this
addendum to agreement to be executed as of the day and year first
above written.
