Resolution 304-1983 ./ i RESOLUTION NO.304 -1983 RESOLUTION AUTHORIZING THE CHAIRMAN OF THE BOARD OF COUNTY COMMISSIONERS OF MONROE COUNTY, FLORIDA, TO EXECUTE AN AGREEMENT BY AND BETWEEN THE BOARD OF COUNTY COMMISSIONERS OF MONROE COUNTY, FLORIDA, AND ANITA SALLAS. BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF MONROE COUNTY, FLORIDA, as follows: That the Mayor and Chairman of the Board of County Commis- sioners of Monroe County, Florida, is hereby authorized to execute an Agreement by and between the Board of County Commissioners of Monroe County, Florida, and Anita Sallas, a copy of the same being attached hereto, for the purpose of providing nutrition consulta- tion in the Older Americans Act, Title III-C, Nutrition Program. Passed and adopted by the Board of County Commssioners of Monroe County, Florida, at a regular meeting of said Board held on the lOth day of November, A.D. 1983. AGREEMENT THIS AGREEMENT, made and entered into this 10th day of November ,A.D. 1983, by and between the BOARD OF COUNTY COMMIS- SIONERS OF MONROE COUNTY, FLORIDA, a body corporate and politic existing under the laws of the State of Florida, hereinafter called the SPONSOR, and ANITA SALLAS, whose mailing address is Apartment 114, Second Avenue & l16th Street, Marathon, Florida 33050, herein- after called the CONTRACTOR. WITNESSETH WHEREAS, the SPONSOR desires to utilize the services of the CONTRACTOR for the purpose of providing nutrition consultation in the Older Americans Act, Title III-C, Nutrition Program in Monroe County and the CONTRACTOR is willing to provide for said services upon the terms and conditions of this Agreement, NOW, THEREFORE, the parties agree and undertake as follows: I The CONTRACTOR hereby agrees to furnish consultation services in accordance with Attachment "A" of this Agreement and to be paid $l5.00 per hour for said services. II The SPONSOR agrees to reimburse the CONTRACTOR for consulta- tion services according to the aforesaid schedule, payment to be made to the CONTRACTOR no more than thirty (30) days after receipt by the SPONSOR of the CONTRACTOR'S claim. Claims for reimbursement shall be rendered as promptly as practicable after the lOth day of each month. III If additional services are requested by the SPONSOR, the parties shall mutually negotiate for additional services including cost of providing same to be paid to the CONTRACTOR. IV The CONTRACTOR shall provide at least thirty (30) days of notice to terminate this AGREEMENT. v The obligations of the SPONSOR under this Agreement are subject to the availability of funds lawfully appropriated annually for its purpose by Monroe County, Florida, and on the availability of funds through contract or grant programs. VI This Agreement shall co~tinue in effect to and including ~ the 31st day of December 198~ at which time the CONTRACTOR'Sfrl~~ obligations hereunder shall cease, unless terminated sooner as provided for herein. This Agreement may be terminated by either party by giving written notice to the other party of its intention so to do thirty (30) days prior to said termination IN WITNESS WHEREOF, the parties hereto have executed this Agreement the day and year first above written. P~,D.C d 0 Clerk ANITA SALLAS all.~7:\ 7)) /~'-A~U-<l E.:!) . ., -- ~e( Witnesses -2- ~~VEDAS TO FORM .,y1z::~T I o ~", Attorney's Office ~.~, Attachment "A" Consultation Services , Relative to Fishermen's Hospital, Inc., providing nutrition consultation to the Nutrition Program the following items are agreed to: The CONTRACTOR will: 1. plan monthly menus in coordination with caterer and Nutrition Program Director which meet one-third the recommended dietary allowance for elderly adults and follow the State mandated menu pattern; 2. submit menus to Nutrition Program Director at least 45 days prior to utilization; 3. develop nutrition education packages monthly for presenta- tion at each of six (6) congregate meal sites; 4. present nutrition education within at least two (2) congregate meal sites each month; 5. evaluate effects of nutrition education among participants at least quarterly; 6. report to Nutrition Program Director results of nutrition education on a regular basis; 7. inspect food service preparation and serVlce including portion and temperature controls at each congregate meal site at least twice yearly; 8. otherwise provide nutrition consultation as requested by the Nutrition Program Director; 9. provide no more than sixty four (64) hours of service during the contract period.
