Attached is a proposed Change Order #8 for Bert Bender for the Old Mariners
Hospital for approval.
According to the ordinance adopted by the Board of County Commissioners,
proposed change orders are to be presented to members of the Board of County
Commissioners prior to approval, assuming they are within the Administrator's
prescribed limits. Change orders not within the Administrator's authority are
placed on the BOCC agenda.
The Administrator intends to approve this change order on Friday, August 27,
2004.

Underaround work for Phone/Cable: Proposal Reauest No. 28 The existina telephone and cable lines are struna on

property. These poles will be removed because FKEC is currentlv re.routina their service to the neiahborhood
underaround alona Hiahpoint Road across the front of the property. It is our understandina that Bell South will not allow
Comcast to attach to their poles because they are competitors: therefore separate utility poles would be reauired for
each company. Installina these two utilities underaround is the most cost-effective and time-efficient way to resolve this
situation. The contractor's pricina for this is: $5.293.75

Dear Don:
I have received pricing from D. L. Porter on Proposal Request No. 26, extending the D.O.H.
sewer from its planned tie in location to the building.
1. lbis Proposal Request was issued because the Monroe County site work will be complete
before D.O.H. starts, and installing the line later will require cutting and patching the new
asphalt. This will result in an unsightly situation.
2. Monroe County will receive a better job if this work is completed now.
3. This work is cheaper if done now because future asphalt patch work will be avoided.
4. The work is required.
5. The work was planned under the D.O.H. contract, but the later timing of their work
necessitates proceeding now under the County contract.
6. The pricing is fair.
Based on all of the above, I recommend accepting Proposal Request No. 26 in the amount of
g6,899.J 1.

Attached is D.L. Porter's pricing for Proposal Request No. 28 regarding underground telephone
and cable. It is my opinion that trenching underground is the only viable option at this point.
The plans call for existing utility poles to be relocated; however, in doing so the following
problems occur:
1. The existing telephone and cable lines are strung on utility poles owned and
maintained by FKEC along the west property line from Highpoint Road to the rear
of the property. These poles are scheduled to be removed because FKEC is
currently re-routing their service to the neighborhood underground along
Highpoint Road across the front of the property rather than around and through
the property overhead.
2. There are only two existing utility poles located along the rear property line
owned and maintained by Bell South. Both of these poles would need to be
relocated and several additional poles would need to replace the existing FKEC
poles along the west property line to bridge the gap up to Highpoint Road. All
existing utility poles are wood, which would not be salvageable. Concrete poles
would need to replace all wood poles.
3. It is my understanding that Bell South will not allow Comcast to attach to their
poles because, they are competitors; therefore, separate utility poles would be
required for each company.
4. Easements across the property would need to be granted to the utility companies
along overhead lines.
5. Above ground lines and easements will interfere with trees in the hammock.
Underground utilities would require less clearing and less trees to be mitigated.

3'x2'x2' road rated pull box @ northwest comer of trench. All conduits to run through new trench and over top of
french drain totaling 450 I.f. each for telephone and cable TV lines. Provide 3' radius sweeps at all turns.

This is to confirm our revised agreements conceming customer provided support structures required to
accommodate BellSouth telephone facilities to provide network services to the demarcation point for the subject
project. The attached specification, 852-03-0101, reflects only the requirements of BellSouth. However, there may
be other vendor telecommunications requirements such as conduit, sleeves and equipment space that you may
need to consider in your plans. You should negotiate any additional requirements with your vendor(s).
It is necessary that the agreed upon support structures be in place and the Building Industry Consulting Group in
BellSouth be notified at least 60 days prior to your required service date, estimated to be October 15, 2004 . The
attached specifications reflect our agreements. If changes are required, prior to building completion, please inform
us as soon as possible. BellSouth recognizes that design changes are sometimes necessary and we will do our
best to accommodate your needs. Please understand, though, that such changes may result in delayed service,
and special charges may be applied in accordance with the general provisions of the tariff to recover any excessive
costs incurred by BellSouth.

Please take a look at the Proposal Request No. 26, which I have attached for your
convenience. It is our understanding the work was planned under the Department of Health
contract, but the later timing of their work necessitates proceeding now under the County
contract. By proceeding now we avoid costly and disruptive work to the asphalt and
concrete walks at a later date.
Monroe County requests you confirm in writing by June 28th, 2004 that you agree with this
work and that The Department of Health will accept responsibility to pay Monroe County for
the work in Proposal Request No. 26.
Additionally, please give Monroe County Construction Management an updated
Construction Schedule beginning when you anticipate going out to bid.

Dear Don:
I have received pricing from D. L. Porter on Proposal Request No. 26, extending the D.O.H.
sewer from its planned tie in location to the building.
1. This Proposal Request was issued because the Monroe County site work will be complete
before D. 0 .H. starts, and installing the line later will require cutting and patching the new
asphalt. This will result in an unsightly situation.
2. Monroe County will receive a better job if this work is completed now.
3. This work is cheaper if done now because future asphalt patch work will be avoided.
4. The work is required.
5. The work was planned under the D.O.H. contract, but the later timing of their work
necessitates proceeding now under the County contract.
6. The pricing is fair.
Based on all ofthe above, I recommend accepting Proposal Request No. 26 in the amount of
