A RESOLUTION AUTHORIZING THE MAYOR AND CHAIRMAN
OF THE BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, TO EXECUTE AN AGREEMENT BY AND
BETWEEN THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, AND THE DADE-MONROE
MENTAL HEALTH BOARD, INC., DISTRICT XI,
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS
OF MONROE COUNTY, FLORIDA, as follows:
That the Mayor and Chairman be authorized to execute an
Agreement by and between the BOARD OF COUNTY CO~~1ISSIONERS OF
MONROE COUNTY, FLORIDA, and the DADE-MONROE MENTAL HEALTH BOARD,
INC., DISTRICT XI, a copy of same being attached hereto,
Passed and adopted by the Board of County Commissioners of
Monroe County, Florida, at a regular meeting held on the 23rd day
of October, 1979,

WHEREAS, the BOARD OF COUNTY COMMISSIONERS OF MONROE
COUNTY, FLORIDA, and the DADE-MONROE MENTAL HEALTH BOARD, INC.
desires to enter into an agreement wherein the Board of County
Commissioners contracts for services from said Mental Healt~
Board for purpose of administration of the Guidance Clinic
of the Upper Keys, the Guidance Clinic of the Middle Keys, and
the Guidance Clinic of the Lower Keys as required under state law
by the Department of Health and Rehabilitative Services In carry-
ing out their duties and functions thereto, and
WHEREAS, the Board of County Commissioners of Monroe County,
Florida, is vested and charged with certain duties and responsi-
bilities relating to the mental health and guidance of the
citizens of Monroe County, and
WHEREAS, such services have been invaluable to the citizens
of the Upper Keys, Middle Keys, and Lower Keys of ~lonroe County,
Florida, and
WHEREAS, it IS proper and fitting to enter into an agree-
ment for serVIces to be rendered in the forthcoming year 1979-80;
now, therefore,
THIS AGREEMENT, Made between the BOARD OF COUNTY COMMIS-
SIONERS OF MONROE COUNTY, FLORIDA, and the DADE-MONROE MENTAL
HEALTH BOARD, INC., each in conisderation of the premises, agree
as follows:
The said Board of County Commissioners of Monroe County,
F lor ida, in consideration of sa id Dade - ~Ionroe Hen tal Heal th
Board, Inc., substantially and satisfactorily performing and
carrying out the duties and obligations of the Board as to the
administration of the Guidance Clinic of the Upper Keys, the
Guidance Clinic of the Mi~dle Keys, and the Guidance Clinic of
the Lower Keys as required under state law by the Department of
Health and Rehabilitative Services in carrying out their duties
and functions thereto in matters of mental health and guidance,

agree to pay to the said Dade-Monroe Mental Health Board, Inc.
the sum of Two Thousand Two Hundred Ninety-five Dollars ($2,295,00)
for the fiscal year 1979-80, This figure will be paid monthly
commencing October 1, 1979, and terminating September 30, 1980,
as hereinafter set forth.
And the said Dade-Monroe Mental Health Board, Inc~, for
the consideration named, covenants and agrees with the Board of
County Commisisoners of Monroe County, Florida, to substantially
and satisfactorily perform and carry out part of the duties of
the said Board in the administration of the Guidance Clinic of
the Upper Keys, the Guidance Clinic of the Middle Keys, and the
Guidance Clinic of the Lower Keys as required under state law by
the Department of Health and Rehabilitative Services in carrying
out their duties and functions thereto in the matters of mental
health and guidance to the citizens of Monroe County, Florida,
In further consideration of said financial remuneration,
the said Dade-Monroe Mental llealth Board, Inc. agrees to provide
a clear financial audit trail to allow for full accountability
of funds received from said Board. Monthly report of the
District payroll disbursements will be made available to the
Board of County Commissioners of Monroe County, Florida, to vali-
date the delivery of services under this contract, The monthly
payroll report is due in tIle office of the Clerk of the Board
of County Commissioners of Monroe County, Florida, no later than
the 15th day of the following month. After the Clerk of the Board
pre-audits the certified report, the Board will reimburse said
Dade-Monroe Mental Health Board, Inc. for its monthly payroll
expenses, However, the total of said ~onth1y payments in the
aggregate sum shall not exceed the total amount of $2,295.00 dur-
ing the term of this contract.
IN WITNESS WHEREOF, the Board of County Commissioners of
Monroe County, Florida, has caused these presents to be signed in
its name by its Chairman, Board of County Commissioners, and th~
Clerk of the Board of County Commissioners, and tIle Dade-Monroe
