RESOLUTION NO. 271 -1978
RESOLUTION ESTABLISHING CHARGES FOR COPIES OF
MONROE GENERAL HOSPITAL MEDICAL RECORDS
WHEREAS, Monroe General Hospital is no longer in opera-
tion and by Special Act of the Legislature the records of said
Hospital have been turned over to Monroe County, and
WHEREAS, from time to time there are requests for copies
of said records, and
WHEREAS, a nominal charge should be set up for providing
of said copies, now, therefore,
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
MONROE COUNTY, FLORIDA, as follows:
1. That requests for medical records shall be processed
by the Social Services Department of Monroe County, and
2. That a charge of $4.00 for the first five (5) pages
and 50C for each additional page thereafter for copies of medical
or other records of Monroe General Hospital shall be charged to
any person requesting copies of said medical records.
