AMENDMENT TO AGREEMENT PHOENIX PROGRAMS

Five originally signed copies of the above-referenced Amendment to Agreement were received by this office
on 3/22/05. Apparently due to a scriveners error in Paragraph 2. of the document actually presented and
approved by the BOCC on 3/16/05, and executed by the President of Phoenix Programs, Inc., the date of
"February 19,2003" was now "whited-out" and the date October 20,2004 handwritten in it's place.
A review by Assistant County Attorney Suzanne Hutton found that the content of the first "WHEREAS"
cfause of the Amendment supports approval of this document by the BOCC as presented to this office on
3/22/05 with the changed date in Paragraph 2, substantiating a scriveners error in the original document. In
that the document was signed by the President of Phoenix Programs of Florida, Inc. in its original state,
however, the revised document must be re-executed by the President of the Phoenix Programs. This can be
accomplished by:

Preferably. the document being completely re-typed to include the correction and then
executed by the President of Phoenix Programs of Florida, Inc.
