Resolution 049-1976RESOLUTION NO49 -1976 WHEREAS, Monroe County, Florida, has become a self - insurer under the Florida Workmen's Compensation Act, and is now responsible for the payment of such benefits. to its employees, and WHEREAS, in order to properly administer this program so that all employees obtain the.full.measure of benefit and fur- ther to comply with the benefits of the law, it is necessary for the, Board of County Commissioners of Monroe County, Florida, to adopt procedures, now, therefore, BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF MONROE COUNTY, FLORIDA, that it hereby adopts the following rules concerning the administration of the Flori.da:.Workmen's Compensation Act: 1. All Constitutional Officers, and Heads of Depart- ments will be responsible for procedures for the completion of the State of Florida First Report of Injury Form. 2. Such procedures shall provide that when notified that an employee has sustained a job -connected injury, any Con- stitutional Officer, Department Head, Supervisor, or Foreman must immediately obtain all necessary information for the completion of the First Report of Injury Form. 3. All Report of Injury Forms must be submitted immedi- ately to the County Insurance Clerk, Office of the Clerk of the Circuit Court. 4. Any employee that alleged 'a job -connected accident should first obtain authorization to be furnished medical atten- tion where practical. The County Insurance Clerk will furnish a partial Medical Panel List from which Constitutional Officers, Department Heads, Supervisors or Foremen may authorize medical facilities for treatment.
